This study aimed at determining the dietary patterns and investigating their association with cardiometabolic risk markers in a brazilian population at risk. This transversal study was carried out with data of 265 patients (n = 123 M/172 W, age 42 ± 16 years) of the Cardiovascular Health Care Program—PROCARDIO-UFV, Brazil—who had their first appointment between 2012 and 2017. A 24-hour recall was applied. The dietary patterns were determined by Principal Component Analysis. Anthropometric, clinical-metabolic, sociodemographic, and lifestyle data were collected through medical record analysis. Five patterns were identified: “Traditional”, “Caloric”, “Unhealthy”, “Healthy,” and “Healthy Snacks”. In bivariate analysis, the “Healthy” pattern was negatively associated with WC (waist circunference), BMI (body mass index), WHR (waist-to-hip ratio), SBP (systolic blood pressure), fasting glucose, TG/HDL, LDL/HDL, and TG/HDL values and positively to HDL. The “Traditional” pattern was positively associated with adiposity indicators (WC, BMI, and WHR) and negatively associated with body fat, TyG (triglyceride-glucose index), HDL, and LDL (P < 0.05). However, in adjusted models of Poisson regression, individuals with positive factor score (higher adherence) in the “Traditional” and “Healthy” patterns had less occurrence of abdominal obesity (PR 0.85; 95% CI 0.74–0.99/PR 0.88; 95% CI 0.02–0.76), as well as dyslipidemia (PR 0.06; 95% CI 0.02–0.51/PR 0.03; 95% CI 0.01–0.27), diabetes (PR 0.05; 95% CI 0.01–0.45/PR 0.02; 95% CI 0.01–021), and hypertension (PR 0.06; 95% CI 0.02–0.50/PR 0.02; 95% CI 0.01–0.21). A greater adherence to the “Healthy” pattern was associated with lower values to cardiometabolic risk markers and less occurrence of chronic diseases, while the “Traditional” pattern presented contradictory results.
INTRODUCTION/OBJECTIVE: Risk factors for metabolic disorders in childhood are likely to persist into adulthood. It is, therefore, important to identify these factors early to reduce the incidence of cardiovascular outcomes. Thus, this study evaluated the following factors associated with MS in children aged four to seven years: sociodemography, lifestyle, food consumption and nutritional status. METHODS: The study is a cross-sectional study conducted with 397 children (4-7 years) who were monitored during their early months of life via the Lactation Support Program (PROLAC). The continuous risk score for MS was assessed using Principal Component Analysis (PCA) and the MS risk factors used to calculate the score were Waist circumference (WC), High Density Lipoprotein (HDL), Triglycerides (TG), Mean Arterial Pressure (MAP) and blood sugar. In order to evaluate the continuous score’s cut-off point capable of predicting MS risk, a Receiver Operating Characteristics (ROC) curve was employed. The factors associated with MS were analyzed by multiple logistic regression analysis with the addition of variables by levels according to the hierarchical theoretical model. RESULTS: The chance of MS was 1.58 times higher among overweight children (OR: 1.58; 95% CI: 1.07 - 2.32). A higher calorie intake (OR: 1.0005; 95%CI: 1.0002 - 1.0008) was found to increase the chance of MS. Also, a higher percentage of lean body mass reduced the chance of presenting MS among the children (OR: 0.96; 95%CI: 0.94 - 0.98). CONCLUSION: In children, higher calorie intake, excess weight and lower percentage of lean mass were associated with a greater chance of MS.
A síndrome metabólica (SM) é caracterizada pela presença de pelo menos três das seguintes alterações: perímetro da cintura elevado; hipertensão arterial; altas concentrações de triglicerídeos; glicemia de jejum elevada e baixas concentrações de lipoproteína de alta densidade (HDL). Esse agrupamento de fatores de risco cardiometabólico ainda permanecem poucos explorados na literatura e um consenso sobre a sua definição para o público pediátrico ainda não foi estabelecido. Objetivo: Avaliar a prevalência de síndrome metabólica através do escore de risco e os fatores associados em crianças de quatro a sete anos do município de Viçosa, Minas Gerais. Metodologia: Trata-se de um estudo transversal, realizado com crianças de idades entre 4 e 7 anos, que foram acompanhadas pelo Programa de Apoio à Lactação (PROLAC) do município de Viçosa-MG. A pontuação do escore de risco contínuo para SM foi avaliada por meio da Análise de Componentes Principais (ACP). Os fatores de risco para SM utilizados no cálculo do escore foram o perímetro da cintura, HDL, triglicerídeos, pressão arterial média e glicemia. A ACP foi aplicada aos fatores de risco para derivar componentes principais e em seguida, o escore de risco contínuo para SM foi calculado somando os escores- Z. A análise da curva Receiver Operating Characteristics (ROC) foi realizada para a avaliar o ponto de corte capaz de predizer risco de SM pelo escore contínuo. A SM foi associada com as variáveis de interesse: estilo de vida, consumo alimentar, características socioeconômicas e demográficas, estado nutricional, composição corporal e histórico familiar. A normalidade das variáveis contínuas foi avaliada pelo teste de Shapiro-Wilk. Foi utilizada a análise de regressão logística múltipla com adição de variáveis por níveis de acordo com o modelo teórico hierarquizado para avaliar os fatores associados à SM. O nível de siginificância estatística considerado foi α = 5%. Resultados: Observou-se uma elevação progressiva nos valores do escore da SM de acordo com o aumento no número dos seus fatores de risco. Os pontos de corte do escore obtidos para predizer a SM foi de >0,09, com a sensibilidade de 100% e especificidade de 72,67% (crianças de quatro a cinco anos) e de >0,14, sensibilidade 100% e especificidade de 64,65% (crianças de seis a sete anos). Considerando essa proposta de ponto de corte, foi observado uma prevalência de 34,3% para a SM e 49,8% das crianças apresentaram pelo menos um fator de risco. A chance de SM foi maior entre as crianças com excesso de peso e maior consumo calórico. Além disso, as crianças com maior percentual de massa magra tiveram menor chance de apresentarem a SM (OR: 0.96; IC95%:0.94 - 0.98). Conclusão: A pontuação do escore de risco calculada apresentou uma boa acurácia para prever a SM, além de uma alta sensibilidade e uma razoável especificidade. Ademais, o maior consumo calórico, o excesso de peso e o menor percentual de massa magra foram associados à ocorrência de SM em crianças de idades precoces, mostrando a necessidade de avaliar e intervir nesses fatores de risco para prevenir eventos cardiovasculares no futuro.
