Introdução: De acordo com dados do Instituto Brasileiro de Geografia e Estatística (IBGE), considera-se população pediátrica indivíduos na faixa etária de até 17 anos. Estima-se que a população pediátrica brasileira seja de 61,5 milhões, o que corresponde a 29% da população total do país, a qual foi estimada no ano de 2020 em cerca de 210 milhões de pessoas. De acordo com a Demografia Médica no Brasil, publicada em 2020, 10,1% dos médicos especialistas no Brasil são pediatras. Ao traçar o perfil de pacientes pediátricos para pesquisa clínica,encontramos uma escassez de estudos que contemplem essa população, impedindo, assim, a utilização de medicamentos seguros e eficazes em pediatria ou com formulações específicas baseadas em estudos clínicos e farmacoepidemiológicos adequados para esse público-alvo. Assim sendo, em suas diversas faixas etárias, crianças e adolescentes são expostos a medicamentos cujo benefício clínico só é conhecido em adultos. Objetivo: Com base nos desafios para o desenvolvimento de estudos clínicos pediátricos e na grande importância e necessidade depesquisas clínicas para este público-alvo, este estudo buscou identificar o perfil das pesquisas clínicas pediátricas no Brasil nos últimos três anos (2018–2021) e discutir seus desafios e oportunidades. Método: A presente pesquisa consistiu em um estudo observacional, transversal e descritivo, e considerou-se pesquisas clínicas e acesso expandido exclusivamente em população pediátrica. Realizou-se um levantamento na base de dados no Clinical Trials.gov® das pesquisas clínicas pediátricas no Brasil, incluindo programas de acesso expandido, registradasde 06 de janeiro de 2018 a 01 de junho de 2021. Para esse levantamento, a base de dados Clinical Trials.gov® foi acessada em 30 de julho de 2021, às 19 horas e 22 minutos. Utilizou-se as seguintes palavras-chave como filtro de busca: “Brazil, Child (birth–17) and Study Start (MM/DD/YYYY): from 06/01/2018 to 01/06/2021”. Os dados extraídos da base de dados Clinical Trials.gov® foram registrados em uma planilha com uso do programa Microsoft Excel® 2010. Após a checagem da consistência dos dados, realizou-se a análise descritiva. Resultados: Triageminicial reuniu 264 estudos relacionados a ensaios clínicos em pediatria no Brasil, porém apenas 107 destes enquadravam-se nos critérios de delineamento propostos. De acordo com os resultados entre os anos de 2018 e 2021, houve queda de mais de 34% no número total de estudos para pacientes pediátricos. Neste mesmo período, é possível afirmar que em mais de 97% dessas pesquisas não há diferenciação de gêneros, e a concentração da localização do centro de estudo foi na região Sudeste do Brasil, sendo a cidade de São Paulo responsável por abrigar 28% deles. Ainda, de acordo com dados obtidos, 41% desses estudos estão em fase de recrutamento e apenas 24% foram concluídos. A área de maior interesse das pesquisas pediátricas foi em Odontologia, enquanto a área de Doenças Raras não alcançou nem 1% dos temas estudados. No entanto, durante o período entre 2018 e 2021, houve uma mudança do tipo predominante de patrocínio desse tipo de pesquisa, passandoa ser o patrocínio privado o mais frequente. Conclusão: Em decorrência da queda significativa de estudos clínicos em pediatria no Brasil nos últimos anos, conforme apresentado nos resultados, os prescritores e pacientes ficarão sem alternativas de tratamentos terapêuticos precisos e seguros para a população pediátrica brasileira. Considerando que 43% dos estudos clínicos pediátricos em fase de recrutamento no Brasilse concentram no ano 2019, a falta de novas pesquisas clínicas em pediatria nos últimos anos pode sugerir um aumento do uso off label de medicamentos em pediatria por causa da ausência de pesquisas com comprovação científica suficientes nessa área terapêutica. Quase a totalidade dos estudos clínicos em pediatria no Brasil se realizou com a participação de todos os gêneros, o que demonstra que doenças específicas de cada sexo não fazem parte do escopo desses estudos pediátricos, assim como doenças raras que não estavam nos principaistemas pesquisados. Com relação à distribuição geográfica dos estudos clínicos em pediatria no Brasil, observou-se que mais de 60,7% dessas pesquisas clínicas foram conduzidas na região Sudeste, principalmente em São Paulo, seguida pela região Sul, com 27% da concentração dessas pesquisas. Esses resultados reafirmam dados do IBGE sobre a desigualdade regional que se reflete na concentração de investimentos, centros de pesquisas e investigadores nessas regiões do país. When profiling pediatric patients for clinical research, we found a shortage ofstudies that address this population, thus preventing the use of safe and effective drugs in pediatrics with specific formulations based on clinical and pharmacoepidemiological studies that are suitable for this target audience. Therefore, in their different age groups, children and adolescents are exposed to drugs whose clinical benefit is known only in adults. Objective: Based on the challenges for the development of pediatric clinicalstudies and the great importance and need for clinical research for this target audience, this study sought to identify the profile of pediatric clinical research in Brazil in the past 3 years (2018–2021) and discuss its challenges and opportunities. Method: This is an observational, crosssectional, and descriptive study and considered clinical trials and expanded access exclusively to the pediatric population. A survey was carried out in the Clinical Trials.gov® database of pediatric clinical trials in Brazil, including expanded access programs, registered from January 6, 2018 to June 1, 2021. For this survey, the Clinical Trials database.gov® was accessed on July 30, 2021 at 7:22 p.m. The following keywords were used as a search filter: “Brazil, Child (birth–17) and Study Start (MM/DD/YYYY): from 01/06/2018 to 06/01/2021.” Data extracted from the Clinical Trials database.gov® were recorded in a spreadsheet using the Microsoft Excel® 2010 program. After checking the consistency of the data, adescriptive analysis was performed. Results: The research gathered 264 studies related to clinical trials in pediatrics in Brazil, but only 107 of the studies met the proposed design criteria. According to the results between the years 2018 and 2021, there was a decrease of more than 34% in the total number of studies on pediatric patients. In this same period, it is possible to affirm that more than 97% of the studies do not have gender differentiation, and the concentration of the study centers is located in the southeast region of Brazil, with the city of São Paulo having28% of them. In addition, according to the data obtained, 41% of the studies are still in the recruitment phase and only 24% have been completed. The area of greatest research interest is in dentistry, and research on rare diseases did not reach even 1% of the topics studied. However, during the period between 2018 and 2021, there was a shift in research sponsorship sectors, moving from public to private. Conclusion: As a result of the significant decrease in clinical studies in pediatrics in Brazil in recent years, as shown in the results, prescribers and patients will be left withno alternatives for accurate and safe therapeutic treatments for the Brazilian pediatric population. Considering that 43% of pediatric clinical studies in the “recruitment” phase in Brazil are concentrated in the year 2019, the lack of new clinical research in pediatrics in recent years may suggest an increase in the “off label” use of medicines in pediatrics due to the absence of research with sufficient scientific evidence in this therapeutic area. Almost all pediatric clinical studies in Brazil were carried out with the participation of all genders, which demonstratesthat specific diseases of each gender are not part of the scope of these pediatric studies, as well as rare diseases that were not included in the research topics. Regarding the geographical distribution of clinical studies in pediatrics in Brazil, it was observed that more than 60.7% of these clinical studies were conducted in the southeast region, mainly in São Paulo, followed by the south region, with 27% of the concentration of these studies. These results reaffirm IBGE data on regional inequality which is reflected in the concentration of investments, research centers,and researchers in these regions of the country.
