A nutrição parenteral (NP) surgiu por volta de 1970 e é utilizada em pacientes que não estão aptos para realizar a alimentação via oral de forma efetiva. Assim como todo procedimento, possui riscos e benefícios que devem ser analisados pela equipe multiprofissional responsável pelo paciente. Durante sua utilização, deve-se fazer um monitoramento contínuo do paciente, visando diminuir a incidência das principais alterações metabólicas decorrentes da NP. Metodologia: Realizar uma revisão da narrativa acerca das principais complicações metabólicas após utilização da NP. Os dados foram retirados das fontes "PubMed", "Scielo" e "Google Scholar" em português e inglês, publicados nos últimos 10 anos. Discussão e Conclusão: A monitorização contínua do paciente submetido a NP é a melhor forma de prevenir possíveis complicações decorrentes desse procedimento. As principais intercorrências encontradas na NP são relacionadas a hidratação, balanço de eletrólitos, alterações lipídicas, infecção de cateter e trombose.
A pancreatite aguda é uma doença inflamatória do pâncreas, considerada autolimitada, podendo acometer tecido peripancreáticos e causar complicações como necrose local e até mesmo falência de múltiplos órgãos. A litíase biliar é responsável por 7% dos casos de pancreatite aguda, sendo mais comum em mulheres. O tratamento precoce da litíase biliar é responsável pela diminuição dos casos de pancreatite aguda biliar e consequentemente os números de internação por esta patologia. Dessa forma, este estudo objetiva analisar os prontuários dos pacientes com quadro de pancreatite biliar internados no HRAN durante o ano de 2018. Descrevendo as comorbidades, o quadro clínico, tempo de evolução da doença biliar, diagnóstico e tratamento dos pacientes portadores de pancreatite aguda biliar.Metodologia: Trata-se de um estudo qualitativo, quantitativo, descritivo, retrospectivo, transversal e documental extraído de prontuários eletrônicos de pacientes internados no HRAN durante o ano de 2018. Para compor a amostra, foram considerados como critérios de inclusão pacientes do sexo masculino e feminino, de todas as faixas etárias admitidos na UCG do HRAN que tiveram diagnóstico de pancreatite biliar durante o ano analisado. Foram excluídos todos os pacientes submetidos a colecistectomia que não tiveram diagnóstico de pancreatite biliar descrito no prontuário eletrônico.Resultados: Foram analisados 136 prontuários, dos quais 23 eram prontuários de pacientes que tiveram quadro de pancreatite aguda biliar no ano de 2018. Houve uma prevalência de pacientes do sexo feminino (77%). Em relação a comorbidades associadas, 26% dos pacientes eram portadores de HAS. O tempo de evolução da doença biliar foi entre 1 e 2 anos em 30% dos pacientes. O quadro clínico mais prevalente foi a presença de dor em hipocôndrio direito. A USG de abdome foi realizada em todos os pacientes analizados. A colecistectomia VLP foi realizada em 95% da amostragem. 39% dos pacientes teve a resolução do quadro em um tempo médio maior que um ano.Considerações finais: A pancreatite biliar é uma patologia com incidência importante no púlbico geral. O quadro clínico característico é a presença de dor em hipocôndrio direito. O exame diagnóstico mais comum é a realização da USG de abdome e a resolução total do quadro supera 1 ano na maioria dos pacientes.
Introdução: O Adenoma Pleomórfico é o mais comum dentre os tumores benignos, desenvolvendo-se em áreas de tecido glandular, apesar de ser uma importante patologia glandular salivar, o acometimento da glândula submandibular não é o mais prevalente, e sim o da glândula parótida, o que acaba por negligenciar tal manifestação. Apresentação do Caso: Paciente com 38 anos, sexo feminino, melanoderma, procurou o serviço de otorrinolaringologia devido aparecimento de nódulo em região cervical há cerca de 04 anos, associado à limitação de movimento, com prejuízo estético há 02 meses. Nega dor. Ao exame físico local, evidenciou-se lesão nodular de consistência firme, unilocular. À análise histológica, evidenciou ausência de sinais de malignidade. Discussão: O Adenoma Pleomórfico tem crescimento lento, normalmente, perceptível previamente à consulta médica, de anos a meses, apresentando-se de forma muito semelhante a uma linfonodomegalia cervical, indolor e de consistência firme, podendo se manifestar em todas as idades, com predominância entre os 40 e 50 anos, sendo a excisão cirúrgica com margem de segurança seu tratamento de referência, na maioria das vezes, com um excelente prognóstico e baixo índice de recidivas. Conclusão: A evolução lenta é determinante para a percepção atrasada da lesão em questão, e consequentemente, para a necessidade de excisão cirúrgica, para que se tenha um bom prognóstico, além do aumento nas chances de malignização do tumor, o que torna necessário que haja uma maior exploração do tema ao longo da graduação médica, a fim de repercutir na otimização do diagnóstico e tratamento em níveis de saúde menos avançados e mais acessíveis à população em geral.
INTRODUÇÃO: A Síndrome Stickler é uma colagenopatia, rara, hereditária e caráter autossômica dominante, sua incidência é de 1/7500 nascimentos. APRESENTAÇÃO CASO: GPS, sexo masculino, 5 anos, admitido no Hospital das Clínicas de Goiânia, portador de miopia importante e fissura palatina desde o nascimento, queixava-se de baixa acuidade visual há 3meses. Durante a inspeção, foram evidenciados face plana, maxilar curto, olhos proeminentes e pregas epicantais. DISCUSSÃO: A síndrome é caracterizada por um artro-oftalmopatia, ou seja, anormalidades esqueléticas, anomalias orofaciais, perda auditiva, osteoartrite prematura, perda neuro-sensorial,fenda palatina, miopatia grave e cegueira. É classificada de acordo com a mutação em Tipo I, II e III. CONCLUSÃO: Por se tratar de uma síndrome genética rara, é de difícil diagnóstico, necessita de uma análise fenotípica, um quadro clínico favorável e história familiar. Não existe um tratamento curativo. Portanto, quanto mais precoce seu diagnóstico, a intervenção será mais rápida e melhor será o prognóstico.
