Iodine content in drinking water samples ranged from 1.8-2.6 microg/l showing that the studied region is environmentally iodine deficient. Inspite of the consumption of adequate iodine, the existing goiter prevalence among school children during post salt iodization phase ensures that environmental factors other than iodine deficiency may have the possible role in the persistence of endemic goiter in the population. The role of thiocyanate in this regard may not be ruled out.

Endemic goitre and associated iodine deficiency disorders (IDDs) are a major concern in public health even in the period of post salt iodization in many regions. Among others the consumption of cyanogenic plants found responsible for the persistence of such diseases. Bamboo shoots (BS) is one such cyanogenic plant food that caused disruption of certain thyroid hormone synthesizing regulatory element as has already been reported in our earlier study. In this investigation the possible mechanism of thyrocytes disruption along with interruption of thyroid hormone biosynthesis by BS has been worked out. Commonly consumed BS, Bambusa Balcooa Roxb (BBR) water extract was analysed by GC MS; three doses below IC50 were administered to thyrocytes in culture with and without iodine. Expressions of thyroglobulin (Tg), pendrin (PDS) and monocarboxylate transporter 8 (MCT8) were evaluated in thyrocytes with cell cycle analysis, reactive oxygen species (ROS) generation, DNA oxidation and apoptotic regulation through Bax, Bcl-2 and p53. Phytochemical analysis of BBR extract revealed the presence of precursors and metabolic end products of cyanogenic glycosides. Dose dependent decrease in expression of Tg and PDS with concomitant decrease in gene expression of these with MCT8 were observed. Increased ROS, DNA oxidation and associated imbalance were found through increased Bax and p53 with decreased Bcl-2 that perturbed thyrocytes cell cycle. Cyanogenic constituents of BBR generates ROS associated oxidative changes in thyrocytes with DNA damage and oxidation and cell cycle disruption followed by inhibition of thyroid hormone synthesizing regulatory elements; addition of extra iodine showed partial prevention.

Objective: Natural goitrogens present in plant foods or antithyroidal drugs both alter the morphology and normal functional status of the thyroid gland interfering the synthesis and secretion of thyroid hormones. In this study, the goitrogenic potential of bamboo shoots (BS) containing natural goitrogens has been compared against a potent synthetic antithyroidal drug thiourea.Methods: BS was fed to 6 rats by 1/3rdreplacement of 180grams (g) of their food i.e.60g of BS containing 35g of goitrogens of cyanogenic origin such that each rat ideally consumed 6 mg/100g of body weight per day and thiourea at a near equal dose of 6 mg/100g of body weight per day was force fed to another 6 rats for a period of 45 d. Changes in food consumption pattern, serum toxicity marker SGOT and SGPT, morphological changes like thyroid gland weight and histology and functional aspects like TPO activity, Na+-K+ATPase activity, urinary iodine excretion and serum T3,T4 and TSH levels induced by this natural and synthetic agent respectively were investigated. Results analyzed by ANOVA followed by post hoc tests.Results: Food consumption pattern was similar in all groups while serum toxicity markers SGOT, SGPT showed a non-significant increase in experimental groups. Increase in body and thyroid gland weight as compared to control was seen on treatment with both antithyroidal agents, a greater percentage of increase was seen in thiourea treated group. TPO activity, Na+-K+ATPase activity, serum T3 and T4levels were decreased significantly in both treated groups as compared to normal, with maximum inhibition in the thiourea treated group. Histological plates of BS treated thyroid revealed hypertrophy and thyroid follicular cell disruption with microcytic infiltration. Thiourea treated gland histological plate revealed complete abolition of normal histoarchitecture with marked changes in colloidal area and hyperplasia with folding and papillae on the epithelium.Conclusion: In overall, the data indicate that though the goitrogenic potentiality of BS cannot be considered having similar efficacy to that of thiourea but as a food entity by itself, it has potent antithyroidal activity even in presence of adequate iodine intake.

PurposeInformation on thyroid functions in populations consuming potent antithyroidal bamboo-shoots (BS) was found scanty. Therefore, to assess thyroid function in BS consuming children was found relevant.MethodsThis cross-sectional study included 127 children from 3 villages. Thyroid volume (TV), free thyroxine (FT4), free triiodothyronine (FT3), thyroid stimulating hormone (TSH), thyroglobulin antibody (Tg-Ab), thyroid peroxidase antibody (TPO-Ab), urinary iodine (UI), urinary thiocyanate (USCN), salt iodine (SI), water iodine (WI), and BS consumption pattern were assessed. ResultsVariable-wise overall mean±SDs/medians/interquartile ranges (IQR) were - age: 8.69±1.69/8.77/7.22-9.74 y, TVol: 0.82±0.27/0.82/0.67-0.96 ml, FT4: 19.5+5.6/20.1/16.9-22.0 pmol/L, FT3: 4.27+1.24/4.22/3.35-4.96 pmol/L, TSH: 2.44+1.39/2.33/1.60-3.15 mIU/L, Tg-Ab: 15.2+10.0/12.5/11.1-14.5 IU/ml, TPO-Ab: 5.58+12.25/3.89/2.90-5.11 IU/ml, UI: 117.4±58.5/108/73-160.5 µg/L, USCN: 0.99+0.67/0.8/0.5-1.2 mg/dl, WI: 4 .69±4.56/3.0/1.35–7µg/L, and BS consumption: 153.8±01.4/119/71.4–214.3 g/person/day respectively. Only 80.3% salt samples had ≥15 ppm iodine. Thyroid dysfunction prevalence was 6.3%. There were positive correlations between TV and FT4 (r=0.2466, p=0.005), UI and TSH (r=0.2633, p=0.003), TSH and FT4 (r=0.2135, p=0.016), TSH and FT3 (r=0.1898, p=0.033), USCN and FT4 (r=0.2477, p=0.005), Tg-Ab and TPO-Ab (r=0.3768, p=<0.001), and negative correlations between Tg-Ab and TSH (r=-0.2024, p=0.023), Tg-Ab and FT4 (r=-0.1869, p=0.035). In boys, USCN had a positive correlation with TPO-Ab (r=0.3069, p=0.018). The village having highest levels of BS consumption (p=0.037) and median UI showed higher TSH levels (p=0.037) and higher FT3 level (p=0.001) compared to the village of lowest BS consumption.ConclusionsThe BS consuming children appear at risk of developing hypothyroidism by Tg-Ab, which may partly be diminished by reducing BS consumption.
