OBJECTIVETo verify the accuracy of the nursing diagnosis, decreased cardiac tissue perfusion, in patients with coronary artery disease.METHODSClinical validation consisted of evaluation by 75 participants. Sensitivity and specificity measurements were performed by means of latent class analysis.RESULTSThe prevalence of the diagnosis was 64.5%. Nausea, dizziness, and vomiting were sensitive and specific; altered heart rate was specific.CONCLUSIONFour indicators were accurate for the nursing diagnosis.IMPLICATIONS FOR NURSINGAccurate defining characteristics contribute to reliable and precise assessment and enable nurses to diagnose and intervene in the outcome of the patient's clinical condition.
Objetivou-se identificar diagnósticos de enfermagem Risco de perfusão tissular cardíaca diminuída, Risco de função cardiovascular prejudicada e Risco de intolerância à atividade em pacientes com Síndrome Coronária Aguda (SCA) atendidos em um hospital público e terciário de Campinas-SP. Estudo de prevalência entre agosto de 2018 e fevereiro de 2019. Para coleta de dados, foi utilizado um questionário e consulta ao prontuário. Foram avaliados 75 pacientes com SCA, 72% do sexo masculino, idade média de 60 anos. Com diagnóstico de RFCP em 89,3%, RIA em 84% e RPCD em 80%;Constatou-se uma alta vulnerabilidade cardiovascular dos pacientes com SCA aos diagnósticos de enfermagem do domínio e classe 4 da NANDA-I, recomendamos que novos estudos sejam realizados para determinar o poder de predição dos diagnósticos de risco avaliados para os DE com foco no problema.
Objective: To identify the nursing diagnoses of risk for decreased cardiac tissue perfusion and risk for activity intolerance and establish the association between the components of both diagnoses and the signs and symptoms of acute coronary syndromes. Materials and methods: Observational and associative study with 75 patients diagnosed with acute coronary syndrome in a Brazilian public hospital. Inferential statistics and multiple Poisson regression models were applied. Results: 84% of the patients showed risk for activity intolerance and 80% reported risk for decreased cardiac tissue perfusion. Risk factors were present in more than 50% of the sample. The diagnoses showed a high frequency in patients with acute coronary syndrome. Pharmacological agents, presence of comorbidities, and family and personal history were associated with the diagnoses. Absence of evidence of decreased cardiac tissue perfusion as dyspnea, radiation to shoulder and jaw, and pain time less than ten hours acted as protective factors. Conclusion: There is a high cardiovascular vulnerability of patients with acute coronary syndrome to the proposed nursing diagnoses. Therefore, we recommend further studies to determine the predictive power of the assessed risk diagnoses for those focusing on this health problem.
Este estudo teve como objetivos caracterizar o perfil medicamentoso de pacientes com DM2 atendidos na atenção primária e elaborar planos de ação educativa focalizado no aconselhamento medicamentoso dos principais medicamentos utilizados por estes pacientes. Trata-se de um estudo transversal conduzido em unidades de atenção primária. A prescrição médica do último mês foi solicitada ao paciente e, em seguida, registradas as seguintes informações: nome genérico do medicamento, dose e posologia (frequência/dia). O sistema Micromedex ® 2.0 foi utilizado para identificar e analisar os cuidados envolvendo as orientações clínicas a serem ofertadas aos pacientes. Participaram deste estudo 88 usuários com DM2. Identificou-se predomínio de mulheres com idade entre 42 e 75 anos com baixa renda e escolaridade. A maioria dos participantes fez uso de polifarmácia. As classes de medicamentos mais prescritas foram: antidiabéticos orais, hipolipemiantes e antiagregantes plaquetários.O esclarecimento de eventos adversos, o modo de tomar os medicamentos, a associação ou não com alimentos e o uso concomitante de bebida alcoólica foram os planos de aconselhamento mais prevalentes. O aconselhamento medicamentoso realizado por enfermeiros na atenção básica pode ser uma estratégia educativa voltada para promoção mais seguro do uso de medicamentos.
