O presente estudo teve como objetivo avaliar nas capitais brasileiras a quantidade de Centros de Especialidades Odontológicas (CEOs) habilitados como “Rede de Cuidados à Pessoa com Deficiência” (CEO-RCPD), e se esse número é compatível para atendimento da demanda existente de pacientes com necessidades especiais (PNEs) em cada uma das capitais. Trata-se de um estudo transversal, quantitativo, de caráter avaliativo normativo da relação entre o número de CEOs e o número de PNEs das capitais brasileiras no ano de 2017. De acordo como Ministério da Saúde, o Brasil possui 133 CEOs atuando nas capitais brasileiras. Dentre estes, apenas 79 estão habilitados a atender a demanda dos pacientes com necessidades especiais. A região Centro-Oeste foi a que apresentou a maior oferta de atendimento a PNEs nos CEOs (87,5%), e a região Sul, a menor (11,1%). Os resultados sugerem a necessidade de aumentar o número de CEOs que atendam a PNEs, de forma a assistir integral e universalmente a população brasileira em suas necessidades de saúde bucal, bem como reduzir o tempo de espera dos usuários e a necessidade de deslocamento para outros municípios ou Estados.
Background Childhood cancer is one of the main causes of child mortality and its treatment has debilitating effects on the oral cavity. Several oral mucositis (SOM) is one of the most common and may cause undesirable symptoms such as pain and risk of systemic infection. Material and Methods This was a longitudinal, retrospective, and observational study determining the incidence of severe oral mucositis (SOM) and its occurrence sites in pediatric oncologic patients, in João Pessoa, Brazil, between 2013 and 2018. Data from 56 patients aged 1 to 18 years were collected from their medical records and through an oral mucosa examination, from the 1st to 5th week of chemotherapy treatment (CT) using the modified Oral Assessment Guide, by previously calibrated examiners (Kappa index > 0.7). The data were analyzed by the Chi-square test, and Odds Ratios were calculated. Results Most patients were females (54.5%), aged 8.8 years (± 4.8), with hematologic tumors (73.2%), predominantly Acute Lymphoid Leukemia (50.0%). An increase in the occurrence of SOM was observed throughout the CT ( P = 0.05), ranging from 12.5% in the 1st to 35.7% in the 5th CT week. In the 1st CT week, there was a predominance of alterations in the lips (5.5%) and saliva (5.5%), while in the 5th, the jugal / palate mucosa (21.4%) remained the most affected site by SOM. Differences in the severity of SOM in the jugal / palate mucosa ( P = 0.01) and labial mucosa ( P = 0.04) were observed over time. In the 5th CT week, the likelihood of developing SOM was 13.3-fold higher (95% CI: 1.5 - 105.6) in patients with hematologic tumors. Conclusions The incidence of SOM was higher in the 5th CT week, most commonly affecting the jugal / palate mucosa, and patients with hematologic tumors were more prone to develop SOM. To identify the profiles of patients with special needs attended at a Center for Dental Specialties. Material and Methods: This was a cross-sectional and quantitative study using an inductive approach, and a comparative and statistical procedure for analysis of the patients with special needs. Information concerning socioeconomic, medical and dental conditions was collected. The data were analyzed using IBM SPSS, adopting a significance level of 5%. Results: It was observed that 58.6% of the users were males, single (54.7%), in the age group from 19 to 59 years (41.1%), being 97.4% without schooling. The most frequent clinical diagnoses were: deviations in intelligence (18.4%), behavioral deviations (18.4%) and physical defects (17.9%). We observed for oral health the presence of gingivitis (33.0%), and healthy gums (47.8%); the presence of caries (64.9%), with restored teeth (28.5%), and edentulism (12.3%). Dental intervention procedures were initiated with emphasis on: fluoride applications (39.7%) and subgingival scraping (34.9%). Topical fluoride applications (p=0.010) and prophylaxis (p=0.010) were realized in patients without autism. Also, prophylaxis (p=0.007) was more frequently performed and gingival alterations were more often verified (p=0.020) in patients without Down's syndrome. Conclusion: The users of the patients with special needs dental service can be generally described as male, single, aged between 19 and 54 years, with the special conditions of intelligence and behavioral deviation.
Objective: to ascertain the profile of home care service (HCS) users with speech and language complaints in João Pessoa, Paraíba (PB). Methods: a descriptive and quantitative study using an indirect documentation technique based on an analysis of primary source documents, characterized as a documentary study, conducted at a HCS, in João Pessoa, PB, that stores user information in the form of medical records. All patient records included by the HCS from June 2012 to June 2016 were analyzed. After the eligibility criteria were applied, the sample consisted of 114 medical records. The collected data were entered into a Microsoft Excel spreadsheet to generate descriptive statistics using IBM SPSS (version 21.0). Results: most patients were males (59.6%), elderly (62.3%), had been diagnosed with a stroke (57.9%),had histories of dysphagia (76.3%), speech problems (58.8%), swallowing disorders (80.7%), and abnormal chewing (77.2%). Dysphagia is often associated with stroke, predominantly among males and the elderly. Conclusion difficulty in swallowing, or dysphagia, is often associated with stroke, predominantly in the presence of the sociodemographic and clinical variables associated with men and the elderly.
O objetivo deste trabalho foi discutir casos de mucosite oral grave quimioinduzida, tratados com solução oral para bochecho associado a laseterapia de baixa potência em pacientes pediátricos oncológicos. As patologias apresentadas pelos pacientes que apresentaram a mucosite foram: Leucemia Linfoblástica Aguda (LLA), Osteossarcoma e Tumor de Wilms. As lesões se apresentaram nos seguintes sítios anatômicos: vermelhão do lábio, mucosa labial, e língua. O tempo médio para regressão das lesões foi de 6,8 ± 3,9 dias. O protocolo de tratamento consistiu na utilização da solução para mucosite oral utilizada no Hospital Napoleão Laureano, João Pessoa/PB. Concomitante ao uso da solução oral, iniciou-se a terapia fotodinâmica com laser de baixa potência (ECCO Fibras e Dispositivos; n/s–040401; modelo–BM0004A), calibrado para um comprimento de onda de 670 nm, potência de 40mW e densidade de energia de 4 J/cm2. Todos os pacientes tiveram remissão das lesões de mucosite oral em até 14 dias após o início da intervenção terapêutica. Concluiu-se que o protocolo utilizado, com o uso da laserterapia de baixa potência aliado à solução de mucosite oral mostrou-se eficaz no tratamento da mucosite oral em pacientes pediátricos em tratamento antineoplásico.
Objetivou comparar o perfil de conhecimento sobre HMI entre odontopediatras e clínicos gerais. Este estudo foi do tipo transversal, com aplicação de questionários a 40 cirurgiões- dentistas devidamente registrados no Conselho Regional de Odontologia da Paraíba (CRO-PB), sendo 20 clínicos gerais e 20 especialistas em odontopediatria. A maioria dos participantes dessa pesquisa foi representado por mulheres (n= 36; 90,0%), com idade média de 32,45 anos (+7,62) e tempo de atuação profissional superior a 11 anos. Os dois grupos avaliados obtiveram conhecimento teórico satisfatório em relação à fatores etiológicos, destacando-se o fator genético. Sobre as características clínicas, a maioria respondeu que afeta esmalte e dentina, com opacidades demarcadas, difusa, com localização assimétrica e que afeta alguns dentes. Os dois grupos apresentaram conhecimento teórico, no entanto, a avaliação de imagens mostrou diferença entre os dois grupos. Em relação a conduta clínica, ambos grupos indicam uso de selantes, fluorterapia e restauração em cimento de ionômero de vidro, não havendo difença significativa (p>0,05). Odontopediatras identificaram corretamente imagens com HMI (n=13; 65,0%), enquanto os clínicos gerais consideram a mesma imagem como hipoplasia de esmalte (n= 11; 55,0%). Os odontopediatras apresentam maior domínio sobre a HMI em relação ao diagnóstico, porém o conhecimento sobre etiologia, tratamento e escolha de material não apresentou diferença estatisticamente significativa entre os grupos (p>0,05). Conclui-se que o perfil de conhecimento sobre etiologia, tratamento e características clínicas entre ambos grupos apresentam níveis de conhecimento semelhante. Ao avaliar imagens clínicas, odontopediatras apresentaram padrão de resposta correto, diferentemente de clínicos gerais.
