RESUMOA família tem papel fundamental junto ao paciente no enfrentamento do câncer, sendo uma estrutura para as perdas e limitações que a doença determina. Objetivou-se compreender a percepção de familiares cuidadores de pessoas com câncer em fase terminal sobre cuidados paliativos. Trata-se de um estudo com abordagem qualitativa que foi realizado em uma clínica de cuidados paliativos de um hospital referência para o tratamento do câncer com oito familiares de pacientes que estavam internados. Foi utilizado um roteiro de entrevista semiestruturado. As entrevistas foram gravadas e posteriormente transcritas na íntegra. Os dados foram analisados com base na técnica de Análise de Conteúdo, modalidade temática segundo Bardin. O Projeto foi aprovado pelo Comitê de Ética em Pesquisa. Os resultados obtidos a partir da análise do conteúdo apontaram para as categorias: Conceito de cuidados paliativos; Significações dos cuidados paliativos; Desafios diante da terminalidade. A percepção dos familiares cuidadores entrevistados possibilitou a compreensão sobre o binômio família-paciente e a influência que o familiar cuidador exerce no processo de adoecimento e acompanhamento da pessoa em tratamento paliativo. INTRODUÇÃONo meio social, o câncer é visto como uma doença incurável e o seu diagnóstico abala a vida das famílias das pessoas com problemas oncológicos, provocando uma série de mudanças que geram conflitos emocionais pelo medo da experiência que terão que viver (1) . Os recursos terapêuticos para combater a doença são ?, entretanto, o câncer é considerado uma doença invasiva e complexa cujo curso em pacientes caminha para o prognóstico de terminalidade (1) . A terminalidade da vida é definida quando todos os recursos terapêuticos curativos encontram-se limitados e quando não há mais tempo e oportunidade de fazer algo para o indivíduo que se torna irrecuperável e evolui para a morte (2) . Como isso, é necessária uma assistência humanizada ao paciente e sua família, fundamentada na filosofia dos cuidados paliativos que consiste em melhorar a qualidade de vida destes que enfrentam problemas relacionados à doença fora da possibilidade de cura (3) . A Organização Mundial da Saúde define a atenção paliativa como um cuidado ativo e total de pacientes cuja doença não responde mais ao tratamento terapêutico. Trata-se de uma abordagem de cuidado diferenciada que proporciona uma melhor qualidade de vida ao paciente e seus familiares por meio de uma adequada avaliação para o controle da dor e outros sintomas, além de um suporte psicossocial e espiritual (2) . No tratamento paliativo, a família é uma unidade de cuidados por apresentar demandas sociais, espirituais, físicas e psicológicas durante o processo de cuidado de seu familiar em adoecimento (4) . Investigar como a comunicação acontece em UTI e as estratégias utilizadas para minimizar as assimetrias na terapêutica. Método: Revisão integrativa da literatura realizada em julho de 2017 nas bases de dados da LILACS, MEDLINE e BDEnf por meio dos cruzamentos dos descritores "Comunicação", "Relações Enfermeiro-Paciente", "Unidade de Terapia Intensiva" e "Enfermagem" para responder a questão norteadora: "Quais as estratégias utilizadas na unidade de terapia intensiva para minimizar as assimetrias na comunicação terapêutica?". Definiram-se os critérios de inclusão: ser artigo original, nos idiomas inglês, português e espanhol e estar disponível na íntegra. Foram utilizados os critérios de exclusão: teses, dissertações, monografias, editoriais e artigos de revisão. Resultados: A amostra final foi constituída de cinco artigos. Emergiu da análise de conteúdo a modalidade temática "Comunicação Terapêutica em UTI ", e dois tópicos articulados com o tema central: I-Assimetrias na Comunicação Terapêutica em UTI; e II-Estratégias para minimizar as assimetrias no processo de comunicação; Conclusão: Os enfermeiros muitas vezes se sentiram limitados em sua capacidade de se comunicar com pacientes e famílias.
