Doença de Cushing é causada pelo adenoma de hipófise corticotrófico que aumenta o nível de cortisol e consequentemente os níveis de glicose no sangue e o depósito de gordura nos tecidos e órgãos. Esse estudo propõe relatar o caso de uma paciente portadora dessa patologia rara visando entender a síndrome que o hipercortisolismo/adenoma resulta e discorrer sobre o desafio clínico. Trata-se de um estudo de caso cujos dados foram obtidos por meio de avaliação de prontuário cedido pela paciente, análise dos exames diagnósticos realizados e revisão da literatura. A paciente de 21 anos queixava de obesidade de difícil controle. Foram realizados exame físico, exames laboratoriais e de imagem os quais levaram a hipótese diagnóstica de adenoma de hipófise. Após o procedimento de retirada cirúrgica, o material foi encaminhado ao anatomopatológico para biópsia, que confirmou o diagnóstico de adenoma de hipófise corticotrófico. Há que se atentar para as anormalidades clínicas causadas pelo excesso de cortisol relacionados ao diagnóstico, contemplando a necessidade de um tratamento multidisciplinar.
A mama é um tecido sujeito a frequentes alterações decorrentes de diversos fatores, como desequilíbrios hormonais. Essas mudanças podem ocasionar a doença fibrocística da mama (DFM) cujo termo abrange várias patologias comuns e benignas que acometem as mulheres de forma cada vez mais recorrente. No que concerne a essa doença, o diagnóstico, manejo dos sintomas e efetividade no tratamento são de suma importância para aumentar a qualidade de vida das pacientes, bem como minimizar os riscos dessas lesões agravarem-se para câncer de mama. Nesse aspecto, o presente trabalho realizou uma pesquisa minuciosa através das plataformas virtuais Medical Literature Analysis and Retrieval System Online (MEDLINE) via Pubmed, Biblioteca Virtual em Saúde (BVS) e Google Acadêmico para compor essa Revisão Integrativa da Literatura (RIL). Após a seleção dos artigos, que foram separados devido a aplicação dos critérios de inclusão e exclusão, constatou-se que a DFM afeta várias mulheres com sintomas como mastalgia (dor), secreção mamária, edema e diversos outros sintomas que impactam negativamente na sua qualidade de vida física, psicológica e sexual, sendo necessário o diagnóstico e tratamento precoce.
Síndrome de Münchausen por Procuração ocorre quando um cuidador mente sobre a saúde da pessoa sob seus cuidados. Esse estudo objetivou sintetizar os resultados obtidos em publicações sobre essa síndrome visando sua maior compreensão ao elencar as características das vítimas, a motivação do agressor, os sinais e sintomas que possibilitam seu diagnóstico, bem como as recomendações aos profissionais da saúde e especialistas acerca do diagnóstico precoce. Trata-se de uma revisão integrativa da literatura utilizando um formulário como instrumento para a coleta de dados em 15 publicações de 2019 a 2022. Verificou-se que a maioria das publicações (81,25%) ocorreu na área médica; as características metodológicas foram revisão de literatura, seguida de relato de caso associado à experiência clínica; as vítimas foram crianças de até 12 anos, independente do sexo; os agressores, na grande maioria, são as mães com histórico de abusos e transtornos emocionais, sem motivação aparente ou relevante e muitas são profissionais da área da saúde. Os sinais e sintomas apresentados pelas vítimas são inespecíficos e intermitentes o que dificulta o diagnóstico precoce e exige dos profissionais da saúde atenção constante aos sinais de alerta demonstrados pela vítima e presentes no comportamento do agressor. O diagnóstico precoce dessa síndrome é um desafio porque seus sintomas dependem da história factícia contada pelo agressor. No entanto, a dificuldade de reconhecimento clínico de portadores dessa síndrome é minimizada quando os profissionais da saúde estão atentos a essa possibilidade e trabalham de forma multidisciplinar.
