В процессе эмбриогенеза нарушение формирования органов мочеполовой системы встречается достаточно ча-сто -пороки развития мочевого тракта, приводящие к урологическим заболеваниям в детском возрасте, состав-ляют 10-35%, при этом 35-40% приходится на долю врожденного гидронефроза (ВГ) с гипоплазией почечной ткани [1].Васкулогенез является одним из ключевых процес-сов, ответственных за формирование органов половой си-стемы, становится необходимой составляющей эмбриоге-неза. Материал и методы. В исследование включены 34 ребенка с врожденным гидронефрозом в возрасте от 3 дней до 7 лет, которые разделены на три группы в зависимости от степени нарушения гемодинамических показателей почки, критерием чего выступил индекс резистентности (ИР). Результаты. Проведенное электронно-микроскопическое исследование выявило у детей всех возрастов во всех группах наличие признаков сосуди-стой мальформации артерий всех калибров, а также гипопластические изменения почечной паренхимы. Наиболее значи-мыми ультраструктурными признаками, демонстрирующими тесную связь между процессами дизангио-и дизнефрогенеза является однотипность нарушения строения базальной мембраны гломерул и артериол, что проявляется неравномерно-стью ее толщины и стертостью слоев, а также незрелость эндотелиальных клеток как капилляров гломерул, так и артериол (крупный размер и округлая форма). Важным фактором, подтверждающим их взаимосвязь, является прямая корреляция между повышением значений ИР на всех ветвях почечной артерии по мере нарастания тяжести гипопластических изме-нений в паренхиме гидронефротически измененных почек. Заключение. Продемонстрирована взаимообусловленность процессов дизангио-и дизнефрогненеза у детей с врожденным гидронефрозом. BACKGROUND: Ongoing efforts aim at overcoming the challenges of conventional transurethral resection of bladder tumor (TURBT) such as the high recurrence rate, difficulty of pathologic interpretation and complications including wall injury. OBJECTIVE: To prospectively assess the safety and efficacy of Thulium fiber en bloc resection of bladder tumor (Tm-fiber ERBT) compared to TURBT. MATERIALS AND METHODS: The prospective non-randomized study included 129 patients with non-muscle-invasive bladder cancer (NMIBC) divided into two groups: 58 patients underwent conventional TURBT and 71 –Tm-fiber ERBT with FiberLase U1 (NTO IRE-Polus, Russia). Relapse-free survival (RFS), detrusor presence and complication rates were assessed. For multivariable analysis we used the Pearson chi-squared Hosmer-Lemeshow goodness of fit test; to compare survival –Cox regression analysis; for operative data comparison –chi-square test with Fisher’s correction; for survival analysis –the Kaplan–Meier method and logrank test. RESULTS: RFS rates at 3 and 6 months were 84.5% and 67.2% for conventional TURBT versus 97.2% and 91.5% for Tm-fiber ERBT (p = 0.011 and p < 0.001, respectively). Detrusor muscle was present in 58.6% of cases treated with conventional TURBT vs 91.6% for the Tm-fiber ERBT group (p < 0.001). The obturator nerve reflex and bleeding were noted in 17.2% and 10.3% of TURBT cases, respectively; and in none of cases treated with Tm-fiber ERBT. Limitations included the non–randomized nature and the small sample size. CONCLUSIONS: Tm-fiber ERBT seems to be a safe and efficacious treatment option for NMIBC. Tm-fiber ERBT had fewer adverse events, was more likely to secure detrusor muscle in the specimen and resulted in better RFS rates than conventional TURBT. Based on these promising data, we have started a prospective randomized clinical trial comparing en bloc TURBT with conventional TURBT (ClinicalTrials.gov NCT03718754).

In Russia more than 125,000 patients with various venous diseases, lower extremity varicose veins (LEVV) being predominant, were annually operated on. In recent years, there has been a trend toward younger patients with signs of LEVV. Screening studies have revealed the signs of the disease in 10-15% of high-school children. The high prevalence of LEVV as a whole and its younger onset in recent decades cause more attention to an investigation of the relationship between the development of varicose veins, in childhood and adolescence in particular, and genomic changes. Patients with varicose veins have been noted to have a genetically reduced capacity for contraction of the smooth muscle cells of the vein walls, their remodeling due to the increased synthesis of matrix Gla protein, overproduction of TGF-β1, a matrix metalloproteinase inhibitor, hyperhomocysteinemia, and mutations in the genes encoding the synthesis of thrombomodulin. Varicose vein transformation is considered to be a minor phenomenon of undifferentiated connective tissue dysplasia (UCTD) leading to failure of their walls due to abnormalities in the fibrous structures and extracellular matrix. Confirmation of the role of UCTD in the development of varicose veins will be able to provide an individual approach to treating patients and to choosing adequate postoperative therapy aimed at preventing a disease recurrence.
