The aim is to analyze the value of endothelial dysfunction markers during pregnancy. Materials and methods: We have examined 153 pregnant women to identify endothelial dysfunction markers of endothelin-1, nitrogen oxide (NO) that have been studied using immunoenzymometric sets for an uncomplicated and complicated pregnancy. Results: We found that the concentration of endothelin-1 repeatedly exceeded the rates in pregnant women with miscarriages than during physiological pregnancy. The diametrically opposite pattern concerns the level of nitrogen oxide. These changes in the markers of the functional state of the endothelin indicate the development of the dysfunction of this system in women with the pathology of pregnancy. Conclusions: Consequently, endothelial dysfunction can be considered to be one of the reasons for miscarriage in the examined women. Therefore, the definition of markers of endothelial dysfunction has prognostic value.

У статті представлені результати імуноферментного аналізу Е-селектину в сироватці крові вагітних із фізіологічною вагітністю та невиношуванням вагітності. Невиношування вагітності, особливо на ранніх термінах, і досі залишається актуальною як медичною, так і соціально-демографічною проблемою. Зниження здорового потенціалу населення призводить до збільшення відсотку жінок, вагітність яких перебігає із ускладненнями або взагалі переривається достроково. На думку багатьох вчених, досягнення успіху у вирішенні питання ранніх перинатальних втрат полягає у глибшому вивченні етіопатогенезу даної проблеми. Тому пріоритетним напрямком є пошук маркерів, які дозволять виявляти доклінічні форми загрози переривання вагітності, що дасть змогу пролонгувати вагітність та знизити перинатальні втрати. На сучасному етапі для уточнення анатомічних і функціональних порушень репродуктивної системи велике значення приділяється теорії ендотеліальної дисфункції та її ролі в розвитку і патогенезі акушерських ускладнень. Мета дослідження. Визначити роль Е-селектину у розвитку невиношування вагітності. Матеріали і методи дослідження. Проводили визначення Е-селектину у сироватці крові 153 вагітних із фізіологічним перебігом і невиношуванням вагітності, які перебували на обліку у Жіночій консультації №1 м. Чернівці та на стаціонарному лікуванні у гінекологічному відділенні КМУ «Міський клінічний пологовий будинок №1». Усі вагітні розподілені на чотири групи: 30 жінок з неускладненою вагітністю в терміни 6-12 тиж. (I група-контрольна), 60-із загрозою самовільного аборту (II група), 34-зі спонтанним абортом (III група) і 29-з вагітністю, що не розвивається (IV група). Середній вік обстежених жінок становив 18-45 років. У більшості пацієнток мали місце ратифікаційні аборти і репродуктивні втрати в анамнезі. Для пацієнток усіх груп характерна відсутність гінекологічних і екстрагенітальних захворювань у гострій і підгострій стадіях. У дане дослідження не включали вагітних із антифосфоліпідним синдромом. Е-селектин визначали методом імуноферментного аналізу з використанням тест-системи виробництва фірми Diaclone (Франція). Людський CD62EELISA набір призначений для кількісного визначення «in vitro» розчинної адгезивної молекули ендотеліальних лейкоцитів ELAM-1 (CD62E) у плазмі, сироватці людини, буферному розчині або в культурі клітин. Він визначає як природний, так і рекомбінантний CD62E. Результати дослідження. Проведене дослідження показало, що у жінок із неускладненою вагітністю в терміни 6-12 тиж. рівень Е-селектину становив 69,04 ± 4,30 мкг/л, а при загрозі переривання вагітності падав до 52,43 ± 3,91 мкг/л. При спонтанному аборті цей показник характеризувався тенденцією до ще більшого зниження-43,06 ± 5,95 мкг/л, а при вагітності, що не розвивається,-до 47,14 ± 6,04 мкг/л. One of the most crucial topics at the moment is the viral infection caused by the SARS-CoV-2 coronavirus, which has acquired not only medical, but also, of course, social significance. The COVID-19 pandemic has challenged healthcare systems around the world.Pregnant women are the most vulnerable category of people, along with the elderly. Due to low immune reactivity, they are more susceptible to severe complications than others, which makes the problem of pregnancy in patients with COVID-19 a pressing issue.A successful pregnancy requires changes in the immune system of the pregnant woman to tolerate a genetically foreign fetus. These changes in the immune system, as well as in the cardiac, pulmonary, and other systems, can lead to increased susceptibility or increased morbidity and mortality due to infection during pregnancy. Considering the physiological adaptations associated with pregnancy, we found that the high metabolic demand to maintain normal intrauterine development increases the burden of oxidative stress during pregnancy. Intracellular redox changes associated with acute phase reactions at the maternal-fetal interface may be enhanced during pregnancy. Notably, mother-to-fetal transmission of SARS-CoV-2 has not been detected in most COVID-19 pregnancies. This relative absence of vertical transmission may be due to the presence of lactoferrin in the placenta, amniotic fluid, and breast secretions. However, the cytokine storm induced during COVID-19-associated pregnancies can cause severe inflammatory damage to the fetus and, if left uncontrolled, can subsequently lead to autism spectrum disorders and brain developmental abnormalities in newborns. Given this serious health threat to a child's growth and development, preventing COVID-19 during pregnancy should be a priority.We aimed to study risk factors and assess the course of pregnancy in women with COVID-19.The study was carried out within the framework of the research work "Clinical and pathogenetic substantiation of differentiated treatment of combined pathology of internal organs" (№0122U002209).
