SummaryThis is a review of studies concerning the use of meditation techniques in psychiatry and psychotherapy. A brief history and characteristics of meditation are presented, with an emphasis on mindfulness meditation, which is a type most widely used as a health intervention. Potential adverse effects of meditational practices are also brought to attention. We focus mostly on the links of meditation to psychosis, describing both conditions in which meditation may cause decompensation, as well as presenting research on the effects of the implementation of modified meditation techniques in psychotic patients. In order to better understand the phenomena described we interpret the effects of meditation using psychodynamic and cognitive theories. The studies presented here suggest that in certain circumstances meditation may trigger psychosis, but they also show that interventions based on mindfulness may play an important clinical role in the alleviation of symptoms during psychotic episodes. meditation / mindfulness / psychosis / schizophrenia / psychotherapy
The article reconstructs postpsychiatry's core propositions and briefly describes its theoretical background and assumptions. It also presents chosen aspects of postmodern psychotherapy, which seem to be in many ways similar to postpsychiatry's ideas. Although they are drawn from different inspiration, postpsychiatry and postmodern psychotherapy seem to come to similar conclusions, especially regarding the role of the patient in the therapeutic process, the meaning of psychiatric diagnosis, and the importance of the institutional, cultural, and social contexts in mental health practice and research. The paper also aims to place postpsychiatry and postmodern psychotherapy in a Polish context, focusing on the ethical challenges faced by psychiatry and showing that some of postpsychiatry's ideas and solutions to contemporary problems were already present in the Polish psychiatric literature of the 20th century. It also contains a brief description of the Polish social and historical context of psychiatry, as well as key aspects of the Polish legal system that relate to mental health and seem to reflect the nature of biomedical explanations of mental distress. It concludes that the model of psychiatric care postulated by "postmodern" approaches seems more ethical and scientifically and philosophically grounded and promises better treatment results than the "traditional" biomedical model.
Identification with Christ among psychiatric patients is an example of a complex and multifaceted phenomenon. As a delusion it includes a misidentification (change of identity) in the layer of content and, usually, grandiosity and/or paranoid traits in the formal aspect. What is more, it fits in the category of religious delusions, which are perhaps the most controversial type of delusions and as such require special sensitivity as well as knowledge beyond psychology or psychiatry. The aim of the articleis to show the phenomenon of identification with Christ among psychiatric patients, taking into account different ways of its explaining and understanding. Papers relating to the topic, both theoretical considerations and case studies, found in the EBSCO database were analyzed. Searching for the articles the following key words were used: identity, identification, delusion, Jesus/Christ/Messiah, psychosis, schizophrenia. The analysis included all (actually not numerous) articles except for the one linked to cognitive approach which did not significantly contribute to the issue. Given the multiplicity of ways of explaining and understanding the experience of identification with the figure of the Messiah, it seems to be a mistake to hold both objectivist and one-sided, based on one theory, attitude towards it. Such an experience should be recognized in the context of the history of patient's life and the all possible mechanisms leading to its occurrence, as well as the meanings hidden beneath the symptom, should be take into account. It is also important to be well-oriented in the system of religious beliefs and spiritual needs of the patient.
Artykuł powstał z zamiarem wypełnienie luki w polskim piśmiennictwie psychiatrycznym, w którym nie znalazło się dotąd miejsce na analizę fenomenu choroby afektywnej dwubiegunowej (ChAD) pod kątem konsekwencji, jakie może ona mieć dla tożsamości osób nią dotkniątych. W tym celu, i w związku z nieobecnością badań w warunkach polskich, podjęto próbę odnalezienia wszystkich dotychczasowych doniesień na ten temat w anglojęzycznych czasopismach fachowych i monografiach. W rezultacie dokonano szczegółowego opisu i opatrzono komentarzem pięć gruntownych badań o zasadniczo jakościowym charakterze. Jakkolwiek „tożsamość” operacjonalizowana była w każdym z nich nieco odmiennie, to każdorazowo odpowiadała pytaniu „kim jestem?”, zarówno pod względem fenomenologicznym, przejawiającym się najczęściej w utracie poczucia ciągłości, jak i poznawczym, związanym z kluczowymi elementami obrazu siebie. Przedstawione badania ujawniły szereg zagrożeń dla tożsamości, mniej lub bardziej specyficznych dla ChAD. Charakter tych badań nie pozwala na generalizację ich wyników, ani na odpowiedź na pytanie o skalę zjawiska. Z drugiej strony, względne podobieństwo tych wyników ujawnia niedostrzegane dotąd trudności, potencjalnie częste i istotne dla bardziej całościowego rozumienia i terapii osób z diagnozą ChAD, dla której implikacje zestawiono w końcowej części tekstu.
Wyniki: Wypowiedzi uczestników badania ukazały złożony obraz ich zmagań z tożsamością w przebiegu zaburzenia afektywnego dwubiegunowego. Na znaczącą część tego obrazu składały się postawy wobec diagnozy i leczenia. Połowa badanych sygnalizowała sprzeciw wobec patologizacji dwubiegunowości (stanowiącej część ich tożsamości) oraz agresywnego leczenia skutkującego m.in. postrzeganym obniżeniem zdolności intelektualnych-jednego z fundamentów ich tożsamości. Pozostali przyjmowali obowiązującą narrację psychiatryczną, czując się najbardziej sobą w czasie remisji i podejmując zgodne z zaleceniami lekarskimi starania o utrzymywanie się w tym stanie. Wnioski: Niezależnie od rozumienia swojego zaburzenia i stosunku do leczenia żaden z badanych nie ugłaśniał kryzysu tożsamości. Uczestników badania cechowała wysoka tolerancja niepewności, przypuszczalnie będąca funkcją takich zmiennych, jak wiek (w tym zachorowania) i czas życia z zaburzeniem, rozwinięte myślenie postformalne i motywacja do poznawania siebie. Pozytywne przeformułowania oraz klarowna narracja na temat życia z zaburzeniem afektywnym dwubiegunowym umożliwiły jej akceptację. U jednych oznaczało ona zgodę na przypisaną diagnozę, u innych afirmację dwubiegunowości. Żaden z badanych nie definiował siebie poprzez rolę pacjenta. Tożsamościowym poczuciom ciągłości i spójności sprzyjało nieoddzielanie zdrowych i chorych aspektów siebie. Wyniki, poza wartością poznawczą, mogą dostarczyć klinicystom wzorców stanowiących możliwe punkty odniesienia do pomocy w procesie integracji tożsamości w przebiegu zaburzenia afektywnego dwubiegunowego tym osobom, które nie zdołały wykształcić podobnych strategii. Summary Objectives: The objective of this research was to explore the contents of experiences associated with the identity of people diagnosed with bipolar disorder (BD). Methods: Six semi-structured interviews were conducted with people over 30. Their transcripts were subject to interpretative phenomenological analysis (IPA).
