Current classification of primary progressive aphasia (PPA) encompasses three variants: non-fluent (nfvPPA), semantic (svPPA) and logopenic (lvPPA). Previously lvPPA was regarded as aphasic form of Alzheimer's disease (AD). However, not all patients with lvPPA phenotype present with AD pathology. Despite abundant literature on differentiation of lvPPA from svPPA and nfvPPA, studies comparing lvPPA with AD and mild cognitive impairment (MCI) are scarce. This study aimed at analyzing written descriptive output in lvPPA, AD and MCI. Thirty-five patients participated in the study: 9 with lvPPA, 13 with AD and 13 with MCI. Most aspects of writing performance were comparable in three groups. However, letter insertion errors appeared in 44% patients with lvPPA, while they were absent in AD and MCI. Patients with lvPPA used more verbs than patients with AD. Writing profile may complement other neuropsychological assessment results in the differential diagnosis of lvPPA. Letter insertion errors and frequent verb use may raise a query of lvPPA.
As omission errors predominate in writing of patients with PSP-RS, writing seems to be compromised mainly because of oculomotor deficits, that may alter visual feedback while writing.
Zmiany w zakresie mowy i komunikacji obserwuje się zarówno w otępieniu z ciałami Lewy’ego (DLB), jak i w chorobie Alzheimera (AD). Badanie miało na celu wyodrębnienie różnic międzygrupowych i ich aspektów leksykalnych i syntaktycznych w mowie wiązanej. W badaniu uczestniczyło 12 pacjentów z DLB oraz 13 osób z AD. Analizie poddano przetranskrybowane wypowiedzi będące opisem obrazka sytuacyjnego, ze szczególnym uwzględnieniem pierwszej minuty narracji. W wypowiedziach pacjentów z DLB odnotowano mniejszą liczbę słów i zaimków określonych niż w wypowiedziach osób z AD. Chorzy z AD tworzyli natomiast dłuższe wypowiedzi i w ciągu minuty wypowiadali więcej słów. Wyniki zinterpretowano w odniesieniu do profilu poznawczego typowego dla DLB oraz AD. Zaburzenia dyskursu w AD są związane z deficytami leksykalno-semantycznymi, które zubożają zawartość informacyjną tworzonej wypowiedzi.
Aim: The paper presents longitudinal observation of four patients diagnosed with the logopenic variant of primary progressive aphasia (lvPPA). Materials and methods: The results of 3–4 language and cognitive assessments were available for four individuals (three women, one man) with lvPPA. The length of the observation period was 2–4 years. Language evaluation was comprehensive and addressed narrative speech, naming, word and sentence comprehension, repetition, reading and writing. Neuropsychological examination, whenever feasible, assessed visuospatial function, praxis, memory and executive functions. Results: Anomia was not an isolated symptom at the time of lvPPA diagnosis. Rapid deterioration of both spoken and written communication was observed. Reading aloud single words and high-frequency word comprehension were preserved longer than other linguistic competences. The narrative speech was progressively impoverished in terms of idea density, manifested by a reduced use of nouns and verbs in particular and an increased use of pronouns. At the advanced stage of the disease, the idea density was very low. There was marked deterioration of visuospatial functions, praxis and episodic memory in all patients. Progression to full-blown dementia was observed in all patients. Conclusions: The progression of linguistic and cognitive symptoms in lvPPA, albeit slightly heterogeneous, is relatively fast. The patients usually reach dementia stage within 1–2 years of the diagnosis and sometimes even earlier than 2 years of the declared symptom onset. When planning long-term management of lvPPA cases, rapid deterioration and progression to full-blown dementia within a relatively short time should be considered.
W obrazie klinicznym otępienia z ciałami Lewy’ego (DLB) zaburzenia mowy i komunikacji współwystępują z deficytami funkcji poznawczych. W zakresie dyskursu odnotowuje się zaburzenia w aspekcie syntaktycznym i leksykalnym. Badanie miało na celu wykazanie korelacji scyntygraficznych z nieprawidłowościami leksykalnymi i syntaktycznymi w mowie opowieściowej chorych. W badaniu uczestniczyło 12 pacjentów z DLB. Wyniki przeprowadzonej analizy ukazują zależności pomiędzy wybranymi aspektami dyskursu narracyjnego w DLB a zaburzeniami perfuzji mózgowej. Stwierdzono, że istnieje zależność pomiędzy zaburzeniami w aspekcie leksykalnym i syntaktycznym wypowiedzi a hipoperfuzją okolic skroniowych, ciemieniowych i potylicznych w półkuli lewej i prawej. Zdolność do tworzenia wypowiedzi mówionej na podstawie złożonego materiału ilustracyjnego wymaga zaangażowania percepcji wzrokowej, co może wyjaśniać związek zaburzeń dyskursu z hipoperfuzją tylną, szczególnie po stronie prawej.
