The paper reviews the methods for improving verbal communication in selected variants of Alzheimer’s disease, both in the typical and language variant. It emphasises the need to optimise the management within the framework of patientcentred care, allowing to address individual needs. As far as indirect interventions are concerned, the article presents techniques aimed at improving communication with the affected person, and more broadly – aimed at preserving the quality of life in the course of the illness. Among direct interventions, those aimed at selected system abilities and supporting language function in the context of cognitive and social function are presented. It is shown how speech and language therapy can integrate other therapeutic paradigms, developed in psychology, such as patientcentred therapy and cognitive neuropsychological rehabilitation.
W artykule podjęto – w kontekście możliwości komunikowania się pacjenta z otoczeniem w życiu codziennym – problematykę anomii w chorobie Alzheimera, wiodącego objawu patologii mowy na etapie otępienia. Na przykładzie 73‑letniego mężczyzny z otępieniem umiarkowanym omówiono deficyty zarejestrowane w badaniu logopedycznym – w próbach nazywania konfrontacyjnego oraz próbach konstruowania wypowiedzi monologowych i dialogowych, z jednoczesnym zwróceniem uwagi na charakterystyczne zachowania w sytuacji doświadczania przez osobę chorą trudności językowych. Prezentacja zjawisk uwzględnia bogaty materiał ilustracyjny w postaci pozyskanych próbek mowy. Koncentrując się na problemach w zakresie słownictwa, na bazie wskazań zawartych w programach FOCUSED i WSPIERAM, określono możliwości poprawy komunikowania się pacjenta z anomią z otoczeniem.
The paper discusses the lexical skills of adults with Alzheimer's dementia, assessed on the material of dialogue utterances. The paper presents a proposal for a way of describing lexical disorders in dementia and illustrates the problems commonly occurring in this group of patients. The linguistic analysis covered 1000 pages of transcripts of conversations with patients with moderate and severe intensity of dementia. In the discussion of the results, the most common symptoms of lexical disorders were presented that characterize functional behaviors in dementia: quasi-nomination with the use of the pronoun, transfer of a name, and delexicalization (dephraseologization). At the same time, the behaviors and reactions of patients were shown that appear in the situation of experiencing communication failures due to a decreased lexical skills.
Artykuł prezentuje najpowszechniej wykorzystywane w Polsce metody diagnostyki i rehabilitacji trudności w czytaniu i pisaniu. Powstał na podstawie przeglądu testów psychologicznych, pedagogicznych i logopedycznych (oraz innych narzędzi diagnostycznych), oferowanych przez wiodące polskie pracownie, literatury przedmiotu poświęconej zagadnieniom diagnostyki i rehabilitacji osób z trudnościami w czytaniu i pisaniu, a także na bazie doświadczeń własnych autorek, pracy diagnostyczno-terapeutycznej oraz dydaktycznej.Zgodnie z ustaleniami szerokie ujęcie problemu nakazywałoby uwzględnić w diagnostyce, już na poziomie podstawowym: 1) ocenę czytania (próby literowe, sylabowe, listy wyrazowe /dodatkowo: pseudowyrazy/, teksty), a mianowicie: technikę czytania; tempo; poprawność w aspekcie segmentalnym i suprasegmentalnym; rozumienie (także w aspekcie czytania krytycznego i twórczego); 2) ocenę pisania (próby przepisywania, pisania ze słuchu, pisania z pamięci wzrokowej i słuchowej /z wykorzystaniem materiału językowego o różnym stopniu złożoności, dodatkowo także pseudowyrazy/, teksty twórcze), a mianowicie: poprawność pisowni i interpunkcji we wszystkich typach tekstów; zgodność ze wzorcem językowym w przypadku tekstów przepisywanych, pisanych ze słuchu oraz z pamięci słuchowej i wzrokowej; poziom graficzny pisma; poprawność językową tekstu twórczego i jego spójność; 3) uzupełniająco -w zakresie odpowiednim do obserwowanych w danym przypadku -symptomów zaburzeń mowy bądź jej opóźnień. Zaniedbanym diagnostycznie obszarem pozostają zwłaszcza niespecyficzne trudności w czytaniu i pisaniu. W tej sytuacji trudno o dostosowanie oddziaływań terapeutycznych do aktualnych potrzeb rozwojowych ucznia.
