SummaryObjectiveThe aim of this study was to evaluate the determinants of chronic kidney disease (CKD) with special emphasis on sickle cell trait (SCT).MethodsThree hundred and fifty-nine patients (171 men and 188 women), aged 18 years or older, with reduced kidney function (eGFR < 90 ml/min/1.73 m2) and seen at secondary and tertiary healthcare in Kinshasa were consecutively recruited in this cross-sectional study. Serum creatinine and haemoglobin electrophoresis were performed in each patient. CKD was defined as < 60 ml/min/1.73 m2. Logistic regression analysis was used to assess determinants of CKD with a special emphasis on SCT. A p-value < 0.05 defined the level of statistical significance.ResultsSCT was present in 19% of the study population; its frequency was 21 and 18% (p > 0.05) in patients with and without CKD, respectively. In multivariate analysis, sickle cell trait was not significantly (OR: 0.38; 95% CI: 0.559–1.839; p = 0.235) associated with CKD; the main determinants were dipstick proteinuria (OR: 1.86; 95% CI: 1.094–3.168; p = 0.02), the metabolic syndrome (OR: 1.69; 95% CI: 1.033–2.965; p = 0.03), haemoblobin ≥ 12 g/dl (OR: 0.36; 95% CI: 0.210–0.625; p = 0.001), and personal history of hypertension (OR: 2.16; 95% CI: 1.202–3.892; p = 0.01) and of diabetes mellitus (OR: 2.35; 95% CI: 1.150–4.454; p = 0.001).ConclusionSCT was not an independent determinant of CKD in the present case series. Traditional risk factors emerged as the main determinants of CKD.
IntroductionEn vue d'améliorer la prise en charge des patients souffrant de neuropathie (NP) associées à l'infection HIV, nous avons essayé de déterminer le profil clinique des personnes souffrant de NP au cours du suivi thérapeutique de leur infection HIV.MéthodesIl s'agit d'une étude transversale (n= 101) menée au centre d'excellence depuis 1 an. Notre analyse est essentiellement clinique. Par un examen clinique minutieux, nous avons recherché tous les symptômes et signes cliniques des NP. Subjectivement, les douleurs dominent le tableau. Pour affiner leur diagnostic, nous avons utilisé l’échelle DN4 (Diagnostic des douleurs neuropathiques) et l’échelle EVA (Evaluation de la gravité des douleurs). Nous avons ensuite analysé nos données en fonction de certains autres facteurs épidémiologiques tels que le taux des CD4, le traitement anti-HIV etc.RésultatsLes 101 patients représentent 3,12% de la cohorte générale; 53,3% des patients présentent une abolition des réflexes ostéotendineux des membres inférieurs; 77,89% présentent une hypoesthésie thermo algique en chaussette et en gants; 25% ont présenté une amyotrophie des membres inférieurs; 76,5% ont été soumis à un traitement antirétroviral contenant la stavudine; 11,7% ont pris la didanosine (DDI) et Abacavir (ABC). 84% ont une moyenne de CD4 de 292 cel/mm3.ConclusionLa NP altère la qualité de vie de nos patients et diminue l'adhérence au traitement antirétroviral. Plusieurs facteurs sont incriminés dans la survenue de la NP, l'effet direct des antirétroviraux, l'effet inflammatoire dysimmunitaire, l'effet infectieux lié aux infections opportunistes. D'autres facteurs seront recherchés et analysés ultérieurement.
L'aplasie cutanée circonscrite du cuir chevelu ou aplasia cutis congenita, en nomenclature latine, est une malformation rare du cuir chevelu caractérisée par une absence de ce dernier. Elle peut intéresser l’épiderme ou toute la peau. La prise en charge d'une telle malformation est assez délicate. Elle est généralement chirurgicale et le pronostic dépend en grande partie de malformations vasculaires associées. Le risque infectieux et/ou hémorragique est élevé. Cette malformation peut être isolée, ne se limitant qu’à la peau du cuir chevelu; tout comme elle peut faire partie d'un syndrome polymalformatif associant l'atteinte d'autres tissus ou organes tel qu'on peut l'observer dans le syndrome d'Adams-Oliver. Nous rapportons le cas d'un nouveau-né rwandais chez qui on a constaté, à la naissance, une absence du cuir chevelu, parcellaire, intéressant une grande partie du vertex fronto-pariétal et la région pariétale gauche. Une IRM avec séquences angiograpiques réalisée plusieurs jours plus tard, montre une agénésie de la partie frontale du sinus longitudinal supérieur. A notre connaissance, aucune observation faisant état de cette malformation n'a jamais été publiée en Afrique Centrale.
