Acute cholecystitis is an inflammatory pathological process of the gallbladder resulting from acute obstruction of the cystic duct. Although it is more frequent in females, the number of male patients increases with advancing age, reaching 30% of cases over 65 years of age. It presents as a surgical emergency and usually requires hospitalization for treatment. It is associated with significant morbidity and mortality, especially in elderly patients. The most frequent cause is lithiasis, responsible for 90% of cases. In the differential diagnosis of acute cholecystitis, inflammatory or non-inflammatory diseases with expression located in the upper right hemiabdomen must be considered. They are: right-based pneumonia, hepatitis, pyelonephritis, and even myocardial ischemia or infarction. Other diseases of the digestive tract should be kept in mind, such as acute appendicitis of subhepatic location, complicated peptic ulcer and acute pancreatitis. Imaging of the gallbladder and bile ducts has changed dramatically in the last 20 years. The replacement of transparietal cholangiography and oral cholecystography by modern, non-invasive techniques has brought great advances in the diagnosis of diseases of the bile ducts. Currently, the diagnosis and imaging follow-up of biliary diseases is based on ultrasound (US), computed tomography (CT), magnetic resonance imaging (MRI) and scintigraphy. US remains the test of choice in the initial evaluation of acute biliary diseases, due to its ease of performance, wide availability and high accuracy in the diagnosis of acute cholecystitis.
Este artigo procurou destacar a relação entre endometriose e infertilidade, caracterizando aspectos a respeito da epidemiologia, sintomatologia e diagnóstico com enfoque nas possíveis causas da infertilidade nas mulheres acometidas por esta patologia. A endometriose é uma doença inflamatória crônica que causa danos à saúde da mulher, a reprodução, aumenta o risco de depressão, traz limitações nas atividades de vida diárias e consequente redução da atividade social e laboral, devido à sintomatologia que lhe é tão característica. A infertilidade causada pela endometriose pode estar associada a distúrbios imunológicos, endócrinos ou por distorções anatômicas juntamente à aderências de tecido fibroso. Atualmente existem três opções terapêuticas disponíveis como terapia para a infertilidade associada à endometriose: tratamento clínico, cirurgia e tecnologias adaptadas para reprodução assistida. Por fim, é notório que a endometriose causa infertilidade, porém requer mais pesquisas devido aos mecanismos pelos quais leva a este fim ainda não estão totalmente elucidados e não existe uma terapêutica padronizada para as pacientes portadoras da patologia.
O abscesso cerebral é uma lesão expansiva e possui a maior parte das suas manifestações derivadas desse efeito de massa sobre o parênquima encefálico. Essa apresentação geralmente é constituída pela tríade: cefaleia, febre e déficits neurológicos focais, porém, as literaturas atuais instituem que essa relação tem maior especificidade do que sensibilidade, visto que a maior parte dos pacientes não apresenta a tríade ao diagnóstico. Nesse direcionamento, é visto que a alteração do estado mental é de maior coerência ao efeito expansivo da lesão, sendo considerada uma associação de maior representatividade do abscesso cerebral. Nessa medida, são realizadas técnicas de neuroimagem em pacientes que apresentam quadros infecciosos agudos ou crônicos originados das possíveis fontes de contaminação, associados a uma sintomatologia com convulsões, déficits neurológicos focais ou sinais de aumento da pressão intracraniana. Dessa forma, o diagnóstico neurorradiológico distingue a lesão que se apresenta como uma hipodensidade na TC, hipointensidade na RMT1 e hiperintensidade na RMT2 e FLAIR, associada com uma borda de realce, característica do abscesso cerebral e importante no diagnóstico diferencial.
