Introdução: O câncer de mama é a neoplasia maligna mais incidente entre as mulheres em todo o mundo. A mamografia é o único exame de eficácia comprovada para o rastreio do câncer de mama, evitando que o diagnóstico seja realizado apenas em estágios mais avançados da doença. Atualmente, recomenda-se pelo Ministério da Saúde que mulheres entre 50 e 69 anos realizem a mamografia bienalmente. Com isso, a possibilidade de cura é maior, assim como a de realizar tratamentos menos agressivos. Como forma de padronizar os resultados da mamografia, o Colégio Americano de Radiologia desenvolveu o sistema Breast Image Reporting and Data System (BI-RADS). As categorias indicam a probabilidade de um resultado benigno ou maligno baseado em achados de imagem. Seus benefícios incluem orientação quanto à conduta a ser tomada de acordo com cada categoria. Os resultados são classificados de 0 a 6, representando inconclusivo, normal, achado benigno, provavelmente benigno, suspeito, altamente suspeito e malignidade comprovada por biópsia, respectivamente. Durante o período de pandemia de COVID-19, foram necessárias medidas com o fim de reduzir a disseminação do vírus. Nesse contexto, muitas pacientes adiaram a realização da mamografia, o que pode ter aumentado consideravelmente o atraso nos diagnósticos de câncer de mama. Objetivos: Analisar o número de mamografias realizadas no estado do Rio de Janeiro e seus resultados segundo a classificação BI-RADS, no ano anterior à pandemia de COVID-19 e durante o período da pandemia no país. Materiais e métodos: Trata-se de um estudo descritivo, que utilizou dados referentes à realização do exame de mamografia no Rio de Janeiro por local de residência no ano de 2019, período anterior à pandemia de COVID-19 no país, comparando-os com os dados do ano de 2020, durante o período de pandemia de COVID-19 no Brasil. Os dados foram coletados do Sistema de Informação do Câncer (SISCAN), e as variáveis selecionadas foram: número de exames realizados e categoria BI-RADS. O programa Microsoft Excel foi utilizado para a análise dos dados. Resultados e conclusão: Ao todo, foram realizadas 92.649 mamografias no Rio de Janeiro no ano de 2019, sendo 81.172 entre março e dezembro. Em 2020, do início da pandemia (março) ao fim do ano, foram efetuadas 49.502 mamografias, uma redução de 39,0% — incluindo redução de 81,9% em maio, o mês mais afetado. Levando-se em conta a categoria BI-RADS, em todo o período analisado houve redução de 28,5% em BI-RADS 0; de 42,9% em BI-RADS 1; de 39,2% em BI-RADS 2; de 42,4% em BI-RADS 3; e de 10,2% em BI-RADS 4. Nas categorias 5 e 6, houve aumento de 4,8 e 90,5%, respectivamente. Foi observada redução significativa no número de mamografias realizadas em 2020 no Brasil e aumento nos achados altamente suspeitos e de malignidade comprovada por biópsia. Problemas antigos no acesso a esse exame foram agravados pela crise gerada pela COVID-19. Isso representa uma sobrecarga de exames para os próximos anos e um número potencial de cânceres de mama diagnosticados tardiamente.
RESUMO Objetivo: analisar os dados dos pacientes internados para realização de hernioplastia inguinal unilateral no Brasil no ano anterior à pandemia de COVID-19, e durante o período da pandemia no país. Métodos: trata-se de um estudo descritivo, utilizando dados referentes às internações para realização do procedimento cirúrgico de hernioplastia inguinal unilateral no Brasil no período de março de 2019 a fevereiro de 2020, comparando com os dados de março de 2020 até fevereiro de 2021. Os dados foram coletados do Sistema de Informações Hospitalares (SIH/SUS) e as variáveis selecionadas foram: número de internações, taxa média de permanência hospitalar e taxa de mortalidade. Resultados: ao todo, foram realizadas 119.312 internações para realização de hernioplastia inguinal unilateral no Brasil no período de março de 2019 a fevereiro de 2020. Já durante o período de pandemia no país, foram registradas 53.445 internações para este procedimento. A média de permanência hospitalar aumentou em relação ao ano anterior. A taxa de mortalidade registrada no ano anterior à pandemia foi de 0,11, já no período da pandemia, foi de 0,20. Conclusão: foi observado que durante o período de pico da pandemia de COVID-19 no Brasil, o número de internações para realização de hernioplastia inguinal unilateral foi reduzido em 55,21%. Contudo, houve um aumento significativo da taxa de mortalidade desse procedimento. Esse resultado pode ser explicado pelo aumento da mortalidade em pacientes infectados pelo vírus SARS-CoV-2, e também pela restrição da realização de procedimentos cirúrgicos eletivos nesse período, priorizando quadros emergenciais, os quais são mais complicados, e consequentemente, com maior mortalidade.
Introdução: Atualmente, o termo mortalidade materna é definido como a morte da mulher grávida ou em até 42 dias após o parto, independentemente da duração e da localização da gestação. A definição leva em consideração fatores responsáveis por tornar a gestação de alto risco e por dificultar seu manejo, no entanto desconsidera causas acidentais. É sabido que as principais causas de morte materna no mundo são pré-eclâmpsia/eclâmpsia, hemorragia pós-parto, doenças cardiovasculares, eventos trombóticos e infecções, como a endometrite. Contudo, no período da pandemia do vírus SARS-CoV-2 no Brasil, foi observado um importante aumento no número de mortes maternas. O motivo ainda não é claro, porém alguns estudos mostraram que quando a hipertensão, obesidade, e/ou pré-eclâmpsia ocorrem na gravidez, associadas à infecção por COVID-19, a resposta inflamatória pode contribuir para um pior prognóstico. Ademais, a dificuldade de acesso a serviços com atendimento especializado e o acompanhamento inadequado de complicações obstétricas ainda persistem e foram potencialmente agravados pela pandemia. Objetivos: Analisar o número de mortes maternas e suas principais causas no estado do Rio de Janeiro no período anterior à pandemia de COVID-19 e no período de pico desta no país. Materiais e métodos: Trata-se de um estudo descritivo, que utilizou dados referentes à mortalidade materna registrada no estado do Rio de Janeiro no ano de 2019, anterior à pandemia de COVID-19 no Brasil, bem como nos anos de 2020 e 2021, período de pico da pandemia no país. Os dados foram coletados do banco de dados da Secretaria Estadual de Saúde do Rio de Janeiro (SES/RJ). As variáveis selecionadas foram: óbitos maternos por residência e causas maternas. O programa Microsoft Excel foi utilizado para a tabulação e análise dos dados. Resultados e conclusão: Durante o ano anterior à pandemia, foram registradas 153 mortes maternas no estado do Rio de Janeiro e, no período de 2020 a 2021, foram registradas, respectivamente, 187 e 291 mortes, resultando em aumento de 22,22% em 2020 e 90% em 2021 em relação ao ano de 2019. Em 2019, a maior causa de morte foi a pré-eclâmpsia (PE) (7,18%), em seguida a hemorragia pós-parto (HPP) e a embolia de origem obstétrica (5,88%). Em 2020, a principal causa de morte foram as doenças virais que complicaram a gravidez, parto e puerpério (34,75%), em segundo lugar a PE (10,16%) e, em seguida, a HPP e a gravidez tubária (2,87%). Já em 2021, foi observado que a maior causa de morte materna foram novamente as doenças virais, porém agora responsáveis por 52,57% dos óbitos, com a PE em segundo lugar (4,81%) e em seguida a eclâmpsia (4,47%). Qual o impacto da COVID-19 na gravidez, parto e puerpério e se o estado gravídico-puerperal altera a história natural da COVID-19 são questões que ainda precisam ser mais bem esclarecidas. Também é fundamental formular estratégias que garantam o acompanhamento de pré-natal de qualidade, mesmo diante do cenário de pandemia.
Background: Vitamin B12 is a vitamin essential for enzymatic reactions and, as it is not synthesized by the human body, and there may be a deficiency in cases of food restriction or reduced absorption. It is estimated that the prevalence of its deficiency is 20% in the population over 60 years and the clinical manifestations may be due to hematological and neurological or neuropsychiatric disorders. Methods: This is a descriptive study, using data from the medical records of patients with vitamin B12 deficiency followed at a University Hospital in Rio de Janeiro, from 2014 to 2016. The following parameters were evaluated: hemoglobin concentration (Hb), mean corpuscular volume (MCV), erythrocyte anisocytosis index (RDW), hematoscopy, platelet count, lactate dehydrogenase (LDH) enzyme dosage, mini mental status examination (MMSE), vibratory sensitivity and strength in physical examination. Main Results: Data from the medical records of 20 patients with vitamin B12 levels <200 pg/dL were analyzed. The clinical and laboratory alterations found were: abnormal hematoscopy (90%), LDH > 200 IU/L (85%), RDW > 15% (80%), Hb < 12 g/dL (70%), MCV > 100 fL (65%), altered vibratory sensitivity (55%), platelet count < 150,000/uL (25%), altered MMSE (20%), altered strength (10%). Regarding the etiologies found, the most frequent were pernicious anemia (40%) and atrophic gastritis (20%). Conclusion: The most frequent alteration in this study was abnormal hematoscopy, and the most frequent etiology was pernicious anemia. Is important knowledge of the subject among health professionals, to identify and diagnose this deficiency at an early stage.
