Introdução: Atualmente, o termo mortalidade materna é definido como a morte da mulher grávida ou em até 42 dias após o parto, independentemente da duração e da localização da gestação. A definição leva em consideração fatores responsáveis por tornar a gestação de alto risco e por dificultar seu manejo, no entanto desconsidera causas acidentais. É sabido que as principais causas de morte materna no mundo são pré-eclâmpsia/eclâmpsia, hemorragia pós-parto, doenças cardiovasculares, eventos trombóticos e infecções, como a endometrite. Contudo, no período da pandemia do vírus SARS-CoV-2 no Brasil, foi observado um importante aumento no número de mortes maternas. O motivo ainda não é claro, porém alguns estudos mostraram que quando a hipertensão, obesidade, e/ou pré-eclâmpsia ocorrem na gravidez, associadas à infecção por COVID-19, a resposta inflamatória pode contribuir para um pior prognóstico. Ademais, a dificuldade de acesso a serviços com atendimento especializado e o acompanhamento inadequado de complicações obstétricas ainda persistem e foram potencialmente agravados pela pandemia. Objetivos: Analisar o número de mortes maternas e suas principais causas no estado do Rio de Janeiro no período anterior à pandemia de COVID-19 e no período de pico desta no país. Materiais e métodos: Trata-se de um estudo descritivo, que utilizou dados referentes à mortalidade materna registrada no estado do Rio de Janeiro no ano de 2019, anterior à pandemia de COVID-19 no Brasil, bem como nos anos de 2020 e 2021, período de pico da pandemia no país. Os dados foram coletados do banco de dados da Secretaria Estadual de Saúde do Rio de Janeiro (SES/RJ). As variáveis selecionadas foram: óbitos maternos por residência e causas maternas. O programa Microsoft Excel foi utilizado para a tabulação e análise dos dados. Resultados e conclusão: Durante o ano anterior à pandemia, foram registradas 153 mortes maternas no estado do Rio de Janeiro e, no período de 2020 a 2021, foram registradas, respectivamente, 187 e 291 mortes, resultando em aumento de 22,22% em 2020 e 90% em 2021 em relação ao ano de 2019. Em 2019, a maior causa de morte foi a pré-eclâmpsia (PE) (7,18%), em seguida a hemorragia pós-parto (HPP) e a embolia de origem obstétrica (5,88%). Em 2020, a principal causa de morte foram as doenças virais que complicaram a gravidez, parto e puerpério (34,75%), em segundo lugar a PE (10,16%) e, em seguida, a HPP e a gravidez tubária (2,87%). Já em 2021, foi observado que a maior causa de morte materna foram novamente as doenças virais, porém agora responsáveis por 52,57% dos óbitos, com a PE em segundo lugar (4,81%) e em seguida a eclâmpsia (4,47%). Qual o impacto da COVID-19 na gravidez, parto e puerpério e se o estado gravídico-puerperal altera a história natural da COVID-19 são questões que ainda precisam ser mais bem esclarecidas. Também é fundamental formular estratégias que garantam o acompanhamento de pré-natal de qualidade, mesmo diante do cenário de pandemia.
