In many countries, stroke mortality has decreased, between 1950 and 2005, often very considerably. The different dates of mortality downturn likely are consistent with the implementation of various prevention strategies. This could be translated to policy interventions for stroke control in countries with a rising trend of the disease.

A review of the most current substance use disorder (SUD) treatment/care literature best practices, suggests implementation of user-involvement/oriented models of care results in successful care outcomes. User-involvement models of care are (1) person-centred, (2) demonstrate a patient/person participation approach, (3) apply shared decision-making, and (4) include a recovery model approach to care and client-provider interaction. Current studies that have examined the implementation of user-involvement models of care in SUD treatment are limited, and furthermore, represent a great degree of overlap without identifying care outcomes that are specific to each model. There appears to be no empirical study that examines SUD care outcomes based on a synthesis of the four models and this may be an indication of further research.

Objective: The objective of the qualitative study reported in this paper was to expand knowledge of substance use disorder (SUD) care best practices, by examining the experiences of residential SUD care participants with user-involvement/oriented care models. These included person-centered, shared decision-making, recovery model and patient/person participation. The goal of the study was to provide a better understanding of the following: each user-involvement model as it relates to residential SUD care from the perspective of residential SUD care participants, the importance of concretizing the concepts for future empirical studies and the development of a nomenclature for the synthesis of the models to inform future empirical studies and assist practitioners with applying the core concepts of the four models in a way that is congruent with the outcomes of empirical studies. Methods:The author collected data by conducting semi-structured, open-ended, one-on-one interviews with a convenience sample of 12 study subjects between the ages of 24 and 65 years (11 males and 1 female) who selfidentified as having successfully completed at least one residential care program for substance use disorder(s). All interviews were recorded and transcribed, and grounded theory methodology was used to analyze the results. The study design was approved by the University of Pennsylvania Institutional Review Board, February 2016.Results: Data collected from the study subjects represented some degree of experience with all four userinvolvement models in residential substance use disorder care. Data also revealed experiences that represented the opposite of the models, and was indicative of care affected by organizational stress and trauma (e.g. burnout, vicarious trauma) and a lack of trauma-informed care. Conclusion:Subjects perceived the care to be most beneficial in the presence of the user-involvement models; however, quality care was compromised and negated by the presence of organizational stress and trauma and a lack of trauma-informed care. The current study finds organizational stress and trauma and a lack of traumainformed care not only serves as an impediment to the delivery of user-involvement/oriented care models, but becomes the overarching phenomena that undermines and negates the delivery of user-involvement models of care in residential substance use disorder treatment.

Assessment consists on multi-dimensional aspects of health, collection of saliva samples, and permission for review of general practice medical records. Background As the world population is ageing, and ageing is often stereotyped as a time of mental restriction and inflexibility, individuals make flexible use of available resources, including recruiting regions and other cognitive processes. Our aim is to identify what determines successful ageing across the adult lifespan into old age of cognitive abilities such as memory, attention, emotion, language and action. Methods A population-based cohort of 3000 adults, aged 18+, will be recruited with demographic and basic cognitive assessments. Of these, 700, aged 18e87 with 100 per decile, will be selected for comprising structural and functional neuroimaging [MRI and magnetoencephalography (MEG)] and neuropsychological tests. We will measure neural integrity and integration across cortical regions. On a subset of 280 adults further investigations will use functional MRI, MEG and electroencephalogram, and further behavioural testing. Formal statistical models will be used to examine the changes that occur with healthy ageing, and the reorganisation in terms of strategies and structures invoked to compensate for them. This approach offers hypothesis-driven insights into healthy ageing that are relevant to the general population. Results Collection of data started in Jan-11, with the initial cohort taking 2 years to recruit and a further 3 years for all detailed investigations. Conclusions Our research will generate a unique resource of neuroimaging and cognitive measures about change across the adult lifespan. Our analysis will help us to identify what characterises older adults with preserved performance and how normal ageing differs from pathological ageing in conditions such as Alzheimer's disease.
