La intususcepción intestinal en pacientes adultos es una entidad infrecuente, la cual corresponde alrededor del 5% de todas las causas de obstrucción intestinal. Su diagnóstico no es fácil, dado lo poco especifico de los síntomas de los pacientes que la presentan; este se sustenta principalmente en los hallazgos de los estudios de imagen. El manejo quirúrgico es la piedra angular del tratamiento de esta patología, y su éxito va a estar determinado por el diagnóstico oportuno y la experticia del cirujano tratante. En este artículo se presenta el caso de un paciente masculino de 62 años, que consulta por cuadro de dolor abdominal inespecífico y síntomas urinarios irritativos, el cual por persistencia del dolor abdominal pese al manejo medico es llevado a cirugía, donde se diagnostica de manera intraoperatoria una invaginación intestinal a nivel de íleon distal.
Introducción: los linfomas son neoplasias malignas del sistema linfoide y se clasifican en dos tipos: linfoma de Hodgkin y linfoma no Hodgkin. Los linfomas primarios de nasofaringe son infrecuentes y constituyen aproximadamente el 15% de todos los linfomas de cabeza y cuello.
Reporte de caso: se presenta el caso de una paciente de 57 años de edad con antecedente de hipertensión arterial, quien consultó por signos y síntomas asociados a obstrucción nasal. Al examen físico y en las imágenes diagnósticas se evidenció una masa en la nasofaringe, el reporte histopatológico e inmunohistoquímico concluyó un linfoma difuso de células grandes B.
Conclusiones: de los linfomas que se presentan en cabeza y cuello, los de origen nasofaríngeo son infrecuentes. El linfoma difuso de células grandes B es el subtipo más común en esta localización y su diagnóstico y tratamiento debe ser oportuno debido a que suelen ser diagnosticados en estadios avanzados y su comportamiento clínico es agresivo.
Introducción. El carcinoma escamocelular es una neoplasia maligna derivada de los queratinocitos. Usualmente se presenta en áreas expuestas al sol con daño por radiación ultravioleta. El origen primario umbilical es raro, siendo más frecuente las metástasis. Se expone el caso del carcinoma escamocelular primario umbilical más grande y de comportamiento agresivo reportado en la literatura.
Presentación de caso. Paciente femenina de 45 años, con antecedentes de diabetes mellitus. No presentó antecedes de otras neoplasias. Consulta por síndrome constitucional y masa tumoral en mesogastrio. Se llevó a resección quirúrgica. Los hallazgos operatorios revelaron un tumor que comprometía los tejidos blandos de la pared abdominal y la pared del intestino delgado, con implantes tumorales en el hígado. El examen patológico macroscópico y microscópico, correlacionado con las imágenes diagnósticas y los hallazgos quirúrgicos, fue consistente con un carcinoma escamocelular primario del ombligo, que se extendía desde la región umbilical hasta la submucosa del intestino delgado, con metástasis en hígado y ganglios linfáticos. La paciente falleció posteriormente.
Conclusiones. Las neoplasias umbilicales son poco comunes; si bien son más frecuentes las metástasis, aunque los tumores primarios son excepcionales, deben considerarse como posibilidad diagnóstica. Es necesaria una valoración completa de los pacientes debido a que estos tumores pueden tener un comportamiento clínico agresivo y un pronóstico desfavorable.
