IntroductionHuman T-cell lymphotropic virus type 1 (HTLV-1) infection is intractable and endemic in many countries. Although a few individuals have severe symptoms, most patients remain asymptomatic throughout their lives and their infections may be unknown to many health professionals. HTLV-1 can be considered a neglected public health problem and there are not many studies specifically on patients' needs and emotional experiences.ObjectiveTo better understand how women and men living with HTLV-1 experience the disease and what issues exist in their healthcare processes.MethodsA qualitative study using participant observation and life story interview methods was conducted with 13 symptomatic and asymptomatic patients, at the outpatient clinic of the Emilio Ribas Infectious Diseases Institute, in Sao Paulo, Brazil.Results and DiscussionThe interviewees stated that HTLV-1 is a largely unknown infection to society and health professionals. Counseling is rare, but when it occurs, focuses on the low probability of developing HTLV-1 related diseases without adequately addressing the risk of infection transmission or reproductive decisions. The diagnosis of HTLV-1 can remain a stigmatized secret as patients deny their situations. As a consequence, the disease remains invisible and there are potentially negative implications for patient self-care and the identification of infected relatives. This perception seems to be shared by some health professionals who do not appear to understand the importance of preventing new infections.ConclusionsPatients and medical staff referred that the main focus was the illness risk, but not the identification of infected relatives to prevent new infections. This biomedical model of care makes prevention difficult, contributes to the lack of care in public health for HTLV-1, and further perpetuates the infection among populations. Thus, HTLV-1 patients experience an “invisibility” of their complex demands and feel that their rights as citizens are ignored.
Brazilian AIDS and HIV-1-seropositive patients have had free access to highly active antiretroviral therapy (HAART) since November 1996. Although secondary data based on official mortality statistics indicate a sharp decrease in AIDS mortality, few if any studies tried to estimate the prognosis for patients with HIV who have been followed from the beginning of the HAART era. An observational study, with retrospective and prospective components, was done in 233 adult HIV-1-infected subjects who were recruited in the last 10 years at the outpatient sector of the Secondary Immunodeficiencies Clinic of the Department of Dermatology, Hospital das Clinicas da FMUSP, Sao Paulo, Brazil. The definition of AIDS followed the guidelines issued by the Centers for Disease Control (CDC) in 1987. One hundred sixty patients were asymptomatic, 46 had AIDS, 24 had AIDS-related complex, and 3 presented with acute infection at study entry. Twenty-nine (18%) of the asymptomatic subjects developed AIDS during follow-up, with 5 (3%) deaths. Among the 46 AIDS cases at entry, 7 (17%) died during follow-up. Thus, a total of 12 people (5.2%) died of AIDS in this cohort over a mean follow-up of 5.2 years and 24 people were lost to follow-up (10.3%). Ninety percent of the survivors were on combined therapy (82% with 3 or more drugs, and 8% with 2 drugs), while 10% were not taking antiretrovirals. People with AIDS at entry were 5 times more likely to die during this period compared to patients who were asymptomatic at entry (p = 0.006). Women showed better outcomes than men, reflecting differences in CD4+ T-cell counts at study entry. All but 1 patient progressed to AIDS during the pre-HAART era (before 1996). In spite of its recent decline, mortality from AIDS-related conditions remains an important public health issue.
Realizou-se um estudo qualitativo para conhecer como mulheres vivendo com HIV/Aids atribuem sentido às suas decisões reprodutivas, bem como caracterizar seu desejo inconsciente. Foram realizadas entrevistas em profundidade com 15 mulheres adultas, atendidas em um hospital especializado em doenças infecciosas e em uma organização não-governamental. Foi construído um relato sobre a trajetória de vida das entrevistadas com base na técnica da história oral do tipo temática e na perspectiva teórica psicanalítica. Referiu-se aos conceitos de identificação imaginária e identificação simbólica para explicitar aspectos do desejo inconsciente. As falas das entrevistadas mostraram contradições e suas decisões reprodutivas são pautadas por uma lógica inconsciente, sendo que a gravidez, nesse contexto, tem a função de restauração narcísica cujo objetivo é gerar um rebento soronegativo. Outra contradição é que as entrevistadas avaliam as decisões reprodutivas de outras mulheres soropositivas como uma "loucura" ou "irresponsabilidade", enquanto justificam suas próprias decisões procurando se desresponsabilizar por seu desejo inconsciente. Assim, uma contribuição da Psicanálise para o campo da saúde pública é a inclusão das idiossincrasias na relação do sujeito com o outro e, deste modo, viabilizar o resgate da singularidade do desejo inconsciente dos sujeitos, além de permitir uma reflexão sobre a interferência dessas questões no cuidado integral, o que, afinal, pode interferir no acolhimento das necessidades complexas de mulheres vivendo com HIV/Aids.
Resumo: A insuficiência renal crônica é um problema da saúde pública e a hemodiálise é um tratamento invasivo e desgastante, do qual os pacientes relatam sentimentos de impotência e dependência. A pesquisa teve como objetivos propor atividades lúdicas e relaxamento durante sessões de hemodiálise e investigar opinião dos pacientes sobre o trabalho, bem como conhecer vivências e percepções quanto à doença e tratamento. Método: Trata-se de uma pesquisa qualitativa realizada em um hospital geral da região da Baixada Santista -litoral sul de São Paulo -com grupos de pacientes durante as sessões de hemodiálise, na qual se realizaram atividades lúdicas, relaxamento e visualização, entrevista em profundidade e aplicação de escala Likert. As entrevistas foram categorizadas pelo método de análise de conteúdo do tipo temática. Resultados: Foram realizados quatro grupos envolvendo um total de 13 participantes, com idades entre 20 e 70 anos, de ambos os sexos, com tempo de hemodiálise que variou de um mês a dois anos. Conclusão: Relato dos participantes sobre as atividades propostas indicam que houve melhora relativa do humor e que elas contribuíram para melhor enfrentamento da situação de imobilidade e ociosidade durante as sessões de hemodiálise, além de permitirem a expressão de necessidades subjetivas singulares. Ludic Activities to Improve Psychological Well-Being with Patients in HemodialysisAbstract: Chronic renal failure is a public health problem and hemodialysis is considered an invasive and exhausting long-term process. Patients usually report feelings of helplessness and dependence, and ludic activities might provide involvement, joy and pleasure. Objectives: To propose ludic activities, to investigate patients' opinions about these activities, and to understand patients' experience regarding disease and treatment. Method: This was a qualitative study conducted in a general hospital in the metropolitan area of Santos, Brazil, with groups of patients during hemodialysis sessions, where ludic activities, relaxation and visualization, in-depth interviews and application of a Likert scale were performed. The speeches were categorized by the method of content thematic analysis. Results: We had four groups involving 13 participants, aged between 20 and 70 years, of both sexes. Conclusion: The proposed activity is a positive factor for mood improvement and contributed to better cope with the situation of immobility and idleness, mobilizing objectively and subjectively, and in addition, allowing the expression of emotions in a constructive way, revealing patients' unique needs and idiosyncrasies.
Objective: Understand the meanings of breastfeeding inhibition to prevent vertical transmission among women living with HTLV-1 (WLHTLV). Methods: A qualitative research with participant observation and in-depth interviews was undertaken, using a pretested thematic script, analyzed by means of Bardin's thematic content analysis. The participants were 13 people -11 women and two men -over 18 years of age, diagnosed with HTLV-1 and without co-infections. The study was undertaken at a private room in a specialized center in São Paulo between June/2006 and April/2008, where the researcher worked, so that she was familiar with and had access to the users. The subjects were selected by convenience, during the participant observation. The subjects' reports were recorded, transcribed and analyzed in search of senses and meanings to elaborate the categories. Excerpts were presented, identified by fictitious names. Results: Breastfeeding inhibition is a complex decision that is even more difficult in a context in which the health team does not know this infection. Conclusion: The lack of knowledge on HTLV-1 in the hospital context is a risk for the vertical transmission of this virus and entails significant emotional consequences. The health team needs information and education for comprehensive care and welcoming of WLHTLV's specific needs. ResumoObjetivo: Compreender os sentidos da inibição da amamentação como prevenção da transmissão vertical entre mulheres vivendo com HTLV-1 (MVHTLV). Métodos: Trata-se de pesquisa qualitativa com observação participante e entrevistas em profundidade, por meio de roteiro temático, pela análise de conteúdo temática de Bardin, pré-testado e realizados com 13 pessoas -11 mulheres e dois homens -maiores de 18 anos, diagnosticados com HTLV-1 e sem coinfecções, entre Junho/2006 a Abril/2008, em sala reservada de centro especializado em São Paulo, onde atuava a pesquisadora, psicóloga, com familiaridade e acesso aos usuários. A seleção dos sujeitos ocorreu por conveniência durante a observação participante. Os relatos dos sujeitos foram gravados, transcritos e analisados na busca dos sentidos e significados para elaboração das categorias e, foram apresentados trechos destes, identificados por nomes fictícios. Resultados: A inibição da amamentação é uma decisão complexa dificultada em um contexto de desconhecimento dessa infecção pela equipe de saúde. Conclusão: O desconhecimento do HTLV-1 no contexto hospitalar se torna um risco para a transmissão vertical desse vírus, além de consequências emocionais significativas. Indica-se a necessidade de informação e formação da equipe de saúde para um cuidado integral e o acolhimento das necessidades específicas de MVHTLV.
Pessoas que vivem com HIV/Aids (PVHA) possuem saúde bucal vulnerável. A formação do dentista – tradicionalmente tecnicista e procedimento-centrada – não é sensível a demandas subjetivas dessa população. O objetivo foi compreender como PVHA lidam com a saúde bucal, identificando subsídios para cuidado humanizado e integral. Realizou-se uma pesquisa qualitativa com 12 PVHA adultos em um serviço de Infecções Sexualmente Transmissíveis - IST/Aids sobre saúde bucal e expectativas quanto à assistência. Foram realizados grupos focais e entrevistas em profundidade. Os discursos foram categorizados pelo método de análise de conteúdo temática. Observou-se necessidade de formação dos dentistas para além das técnicas e guidelines , destacando importância da escuta e relações dialógicas para ampliar e complexificar a relação de cuidado. Espera-se que este trabalho com base na Educação Permanente (EP) em Saúde possa subsidiar ações educativas em saúde bucal que contemplem vivências e pontos de vista singulares de PVHA.
