Det utvikles få nye antibiotika, og resistensproblemene øker. Rasjonell bruk av eksisterende antibiotika blir derfor stadig viktigere. Antibiotikabruk som er basert på kunnskap om preparatenes farmakokinetiske og farmakodynamiske egenskaper, vil oftest gi bedre behandlingseffekt og kan også bidra til å redusere resistens og gi mindre bivirkninger.Optimal dosering av antibiotika er viktig for å oppnå best mulig effekt og minske risikoen for bivirkninger. Valg av doseregime kan også påvirke resistensutvikling hos bakterier (1). For å ta gode og informerte valg ved antibiotikabehandling er det ny ig å ha innsikt i noen grunnleggende prinsipper og begreper kny et til farmakokinetikk og farmakodynamikk. I denne artikkelen vil vi gå inn på hvordan ulike grupper av antibiotika har ulike dose-respons-profiler for antimikrobiell aktivitet og vise hvordan fagfeltene farmakologi og mikrobiologi kombineres for å gi kliniske anbefalinger. Vi vil også trekke frem noen utvalgte strategier for praktisk optimalisering av antibiotikabehandling. Kunnskapsgrunnlaget er utvalgte artikler og publikasjoner, kombinert med forfa ernes fagkompetanse.
