Background and objectives: Grain food consumption is a trigger of gluten related disorders: celiac disease, non-celiac gluten sensitivity (NCGS) and wheat allergy. They demonstrate with non-specific symptoms: bloating, abdominal discomfort, diarrhea and flatulence. Aim: The aim of the review is to summarize data about pathogenesis, symptoms and criteria of NCGS, which can be helpful for physicians. Materials and Methods: The PubMed and Google Scholar databases were searched in January 2019 with phrases: ’non-celiac gluten sensitivity’, non-celiac gluten sensitivity’, non-celiac wheat gluten sensitivity’, non-celiac wheat gluten sensitivity’, and gluten sensitivity’. More than 1000 results were found. A total of 67 clinical trials published between 1989 and 2019 was scanned. After skimming abstracts, 66 articles were chosen for this review; including 26 clinical trials. Results: In 2015, Salerno Experts’ Criteria of NCGS were published. The Salerno first step is assessing the clinical response to gluten free diet (GFD) and second is measuring the effect of reintroducing gluten after a period of treatment with GFD. Several clinical trials were based on the criteria. Conclusions: Symptoms of NCGS are similar to other gluten-related diseases, irritable bowel syndrome and Crohn’s disease. With Salerno Experts’ Criteria of NCGS, it is possible to diagnose patients properly and give them advice about nutritional treatment.
Pelvic venous disorders (PeVD) also known as Pelvic Congestion Syndrome (PCS) affect a great number of women worldwide and often remain undiagnosed. Gynecological symptoms caused by vascular background demand a holistic approach for appropriate diagnosis. This is a relevant cause of chronic pelvic pain and atypical varicose veins. The diagnosis is based on imaging studies and their correlation with clinical presentation. Although the aetiology of PCS still remains unclear, it may result from a combination of factors including genetic predisposition, anatomical abnormalities, hormonal factors, damage to the vein wall, valve dysfunction, reverse blood flow, hypertension and dilatation. The following paper describes an in-depth overview of anatomy, pathophysiology, symptoms, diagnosis and treatment of PCS. In recent years, minimally invasive interventions have become the method of first choice for the treatment of this condition. The efficacy of a percutaneous approach is high and it is rarely associated with serious complications. Key Messages Pelvic venous disorders demand a holistic approach for appropriate diagnosis. This article takes an in-depth look at existing therapies of Pelvic Congestion Syndrome and pathophysiology of this condition. Embolisation is an effective and safe treatment option.
Ocena i leczenie bólu przebijającego u chorych na nowotwory Evaluation and treatment of breakthrough pain in cancer patients Streszczenie Wstęp: U chorych na nowotwory mogą występować epizody bólu o znacznym natężeniu. Przebijający ból nowotworowy (BTCP, breakthrough cancer pain) to najczęstsze określenie przemijającego zwiększenia natężenia bólu, jednakże pomimo przeprowadzenia wielu badań klinicznych, nadal istnieją kontrowersje dotyczące definicji, epidemiologii i leczenia farmakologicznego BTCP. Cel pracy: W pracy dokonano przeglądu badań klinicznych dotyczących rozpoznania, oceny klinicznej i postępowania terapeutycznego u chorych z rozpoznaniem BTCP. Metody: Przeprowadzono przegląd literatury naukowej z baz Pubmed, MedLine i Embase ze szczególnym uwzględnieniem definicji, podziału, oceny i leczenia BTCP, zgodnie z wytycznymi Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Wyniki: Najlepszym wyborem w terapii BTCP wydaje się być opioid o szybkim początku działania (ROO, rapid-onset opioid). Produkty fentanylu przewyższają morfinę podawaną drogą doustną zarówno pod względem skuteczności analgetycznej, jak i szybkości działania. Dostępnych jest niewiele badań klinicznych porównujących skuteczność różnych produktów fentanylu w leczeniu BTCP. Wybór analgetyku powinien być dokonywany indywidualnie, w zależności od aktualnego stanu klinicznego pacjenta. Dawka leku stosowanego w leczeniu BTCP powinna być miareczkowana, jednakże przy początkowej dawce należy również uwzględnić tolerancję na opioidy podawane długotrwale w leczeniu bólu podstawowego. Algorytmy rozpoznania BTCP powinny być wykorzystywane wraz z dokładną i kompleksową oceną stanu klinicznego pacjenta.
