Roel Verheul Wim van den Brink en Kees van der VeldenSamenvatting Dit artikel geeft een overzicht van alle relevante literatuur over de samenhang van persoonlijkheidsstoornissen volgens DSM-III(R) en verslaving. De paragraaf 'Korte historische schets' beschrijft de ontwikkeling van het denken over de rol van persoonlijkheid en persoonlijkheidspathologie bij verslaving. In de paragraaf 'Prevalentie van comorbiditeit' worden dı´e steekproefkenmerken, diagnostische criteria en meetprocedure-factoren behandeld, die een belangrijke invloed op de geobserveerde prevalentie van As-II-stoornissen hebben. In 'Causale relaties' worden mogelijke oorzakelijke verklaringen voor het verband tussen As-II en verslaving en de beschikbare empirische steun voor deze verklaringen aan de orde gesteld. Vervolgens worden de beschikbare studies over het natuurlijk beloop en de behandelresultaten besproken. Ten slotte worden in de paragrafen 'Diagnostiek' en 'Behandelimplicaties' richtlijnen voor de meting van As-II-stoornissen en voor de indicatiestelling en de behandeling van verslaafden met persoonlijkheidsstoornissen gegeven. IntroductieIn navolging van de ontwikkelingen in de psychiatrische hulpverlening wordt sinds enkele jaren ook in de verslavingszorg in toenemende mate onderschreven dat de diagnostiek van persoonlijkheidsstoornissen van belang kan zijn voor de behandeling. De vragen naar het waarom van dit belang en naar de mogelijke implicaties ervan zijn nog lang niet beantwoord. Wie van plan is om zich grondig in persoonlijkheidsstoornissen bij verslaafden te verdiepen, dient zich te realiseren dat we aan het begin van een vrijwel zeker lange ontwikkeling staan en dat we vooralsnog te maken hebben met een grotendeels braakliggend terrein. Persoonlijkheidsdiagnostiek en de meting van persoonlijkheidsstoornissen behoren tot de meer complexe onderdelen van de psychodiagnostiek. Voorts bestaan er talrijke relaties tussen persoonlijkheid en verslaving, die niet tot uitdrukking komen in een descriptief-diagnostische benadering, maar die wel degelijk van belang kunnen zijn voor de behandeling. Ten slotte staan we nog aan het begin van de ontwikkeling van praktische richtlijnen voor behandelinterventies en zijn we nog lang niet toe aan goed gefundeerde richtlijnen voor indicatiestelling.Wanneer we het bovenstaande goed tot ons hebben laten doordringen, zullen we in staat zijn de nieuwsgierigheid en openheid ten opzichte van ons onderwerp te ontwikkelen, die nodig zijn om te begrijpen wat we op dit moment wel en wat we nog niet weten. Alleen dan zullen we in staat zijn de vragen te stellen die nog niet beantwoord zijn, en het onderzoek uit te voeren dat nodig is om verder te komen. De langdurige behandeling van een jonge vrouw met paniekaanvallen en obsessies, omringd door botte mensen Kees van der Velden Samenvatting In dit artikel worden de wederwaardigheden beschreven van de nu 31-jarige Cindy de Vriend, die dertien jaar geleden door haar huisarts voor psychiatrische behandeling werd verwezen.In de behandeling kunnen twee fasen onderscheiden worden. In de eerste fase had Cindy allerhande moeilijkheden, maar geen paniekaanvallen. In de tweede fase was paniek de belangrijkste klacht. Om de moeilijkheden van de tweede fase te begrijpen is het nodig iets van Cindy's toestand in de eerste fase te weten; vandaar dat die vrij uitvoerig wordt weergegeven. Na een behandeling waarin enkele bekende directe interventies -ademhalingsoefeningen, schrijfopdrachten, exposure -zo goed en zo kwaad als het ging werden toegepast kreeg Cindy medicamenteuze therapie. Uit de gevalsbeschrijving kan de lezer leren dat farmacotherapie bij paniek niet vanzelf moeiteloos verloopt. In de discussie wordt ingegaan op vragen over diagnostiek en de gevolgde behandelingsstrategie. Eerste fase: gezinstegenspoedAanmelding en taxatie Cindy, achttien jaar, 'administratief medewerkster' met havo-opleiding, zou volgens haar huisarts geen 'nee' durven zeggen. Haar vader en haar oudere broer Tom zouden haar vernederen. De vader werd door de huisarts als een impulsief reagerende man getypeerd, die in de praktijk van de huisarts regelmatig voor moeilijkheden had gezorgd, vooral wanneer hij niet op zijn wenken bediend werd. Haar moeder leed sinds twee jaar aan multiple sclerose, kon niet meer spreken en was 'volledig verpleegbehoeftig'; zij werd thuis verzorgd. Er was een geestelijk gehandicapt broertje dat in een instituut voor diep zwakzinnigen verbleef. De huisarts had ongeveer een jaar lang wekelijks gesprekken met Cindy, maar haar zelfverzekerdheid was hierdoor niet merkbaar toegenomen.Cindy was een zwaar opgemaakt meisje, dat bij het eerste gesprek vergezeld werd door een ongeveer 25-jarige, op dezelfde wijze opgemaakte vrouw, Belinda geheten, die niet alleen de verzorgster van Cindy's moeder was, maar, zoals in latere gesprekken met Cindy bleek, ook de minnares van de heer De Vriend, Cindy's vader. Om hun plezier in elkaars gezelschap te onderstrepen dronk en lachte het jonge paar heel wat af. Belinda bleek te willen dat Cindy vrienden en vriendinnen buitenshuis zou zoeken, speciaal om het geluk van de heer De Vriend en zijn geliefde niet te verstoren, behalve op de avonden waarop het paar buitenshuis pret maakte, en Cindy nodig was om op haar moeder te passen. Cindy zelf had sinds enkele jaren een seksuele verhouding met haar broer Tom, waaraan zij zich niet wist te onttrekken. Tom was de trots van zijn vader, zijn grootmoeder en ook van Belinda. (Waar deze trots op gebaseerd was werd niet duidelijk.) Cindy was vooral bij haar moeder geliefd.Zij verzorgde haar moeder trouw.
