Seine gewOhnliehe Eintrittspforte ist die verletzte Itaut. Ihre Infel~ztion ftihrt zum l~Iilzbrandkarbunkel, der bei seiner verhaltnisma$igen Gutartigkeit wohl dem Kliniker, besonders dem Chirurgen, viel h~iufiger begegnet, als dem pathologischen Anatomen. Bei reehtzeitiger ehirurgischer Behandlung, ja. selbst ohne arztliehe Eingriffe, kommt es bier nut in wenigen Fgllen zu der tSdlichen Allgemeinerkrankung. Viel seltener, aber auch uagleich bSsartiger als diese von der gnl~eren KOrperbedeckung ausgehende Infektion sind die beidenFormen derMilzbranderkrankung, die yon den Sehleimha.uten des Respirations-und Verdaunngstraktus ihren Ausgang nehmen. So gibt Eiehhorst die Mortaliti~t beim Hautmilzbrand a.uf 5-20 %, beim Lungen-nnd Darmmiizbrand auf 50-90 % an. Der Lungenmilzbrand, die Hadernkrankheit, ist eine sehr seltene Erkrankung, und der Anthrax gastro-intestinalis gehOrt, wie besonders aus den Mitteilungen des Kaiserliehen Gesundheitsdmts hervorgehL zu den seitensten Krankheiten, die den Mensehen tiberhaupt befallen. Bei der aul~erordentlichen Widerstandsfi~higkeit des Milzbrandbazillus bzw. seiner Sporen ist es auffgllig, daf~ wit bei Menschen nieht hiiufiger eine viszerale Milzbrandinfektion beobaehten, ttier mag Wohl, wie ja bei allen Infektionskrankkrankheiten, neben erfolgreichen hygienischen MaSnahmen, vor allem mangelnde Disposition eine Rolle spielen. Sie erklart das seltene Auftreten aber nieht a]]ein und nicht fiir alle Formen des Milzbrandes. Bemerkenswert ist, daf~ naeh einer Mitteilung aus dem KaiserI. Ges.-Amt yon Burkhardt yon 276 irn Jahre 1911 Erkrankten 257 eine Berufsinfektion batten. Diese ist vor allem ftir die Milzbrandkarbunkel der Haut und fiir den Lungenmilzbrand wohl fast die alleinige Quelle der Erkrankung. Nur ausnahmsweise ~drd im au~erberufliehen Dasein jemand Gelegenheit haben, z.B. milzbrandhaltiges Tierfleisch mit wunden Stellen der eigenen Haut in Bertihrung zt~ bringen oder milzbrandhaltigen Staub einzuatmen. Auf diesen Gesiehtspunkt wird auch yon Burkhardt hingewiesen.
Von Kurt Rochs. Uber die Frage der kompensatorischen Leberhypertrophie ist, besonders seit P o n f i c k s 35 grundlegenden, experimentellen und histologischen Untersuchungen, viel gearbeitet women. Trotzdem gehen noch in verschiedenen Fragen die Ansichten der einzelnen Autoren welt auseinander. Die einschlSgige Literatur beginnt in den Jahren 1883/84 mit den Arbeiten einiger italienischer Autoren wie Coluccil), Tizoni 1) nnd Grifiinil), die im wesentlichen aber iiber Regeneration yon Wundnaxben in der Leber berichteten. Dann folgte 86 die bedeutende Arbeit P o d w y s s o z k i s 23, die naeh seinen eigenen Worten auch haupts~iehlich ,,ein Stadium der Wundheilung in der Leber" wax. Doch hat er schon vieles beschrieben, was auch fiir die Verh~iltnisse bei der kompensatorisehen Hypertrophie yon grol~er Bedeutung ist. So wies schon er dutch seine Tierversuche naeh, da] zur vollkommenen Regeneration yon Lebergewebe eine Wueherung von Galleng~ngen und Bindegewebe nieht geniige, es miisse dazu kommen die Wucherung der Leberzellen selbst. Die eigentliche Beaxbeittmg des Gebietes der kompensatorisehen Leberhypertrophie selbst begann aber erst mit den Arbeiten P o n f i e k s ~*. Ihm folgten dann auf experimen~ellen Gebiet v. MeisterlG Zadoe=Cohn~O und neuerdings Carraro4. Darin stimmen sie alle fiberein, dal~ bei derRegeneration vonLebergewebe das wesentliehe Moment die Wucherung der Leberzenen selbst ist, wie schon P o d w y s s o z k i ausgesproehen hatte, den Ga]lengangswueherungen sehreiben sie nut eine untergeordnete Rolle zu 2). Da es sich bei diesen Versuehen um plStzliehe Aussehaltung verh~ltnism~Big groBer Absehnitte yon Lebergewebe handelte, traten stets, entspreehend der stiirmisehen Ersatzwucherung (P o n f i c k), zahlreiehe ~VIitosen in Leberzellen und GaIleng~ngen auf. Einer wie groflen Regeneration die Leber f~hig ist, bewiesen besonders die Kaninchenversuehe P o n f i e k s and v. M e i s ~ e r s. In einem gewissen Gegensatz zu diesen experimentellen Befunden stehen die Verhi~ltnisse bei der kompensatorischen Hypertrophie der menschlichen Lebern. z) Zitier~ nach K r e t z ~. ~) C a r r a r o kommt zu dem Resultat, daI~ sieh die neugebildeten Gallengiinge hie in Leberzetlen umwandeln. Die Literatur tiber die degenerativen, entziindlichen und arteriosklerotischen Erkrankungen der 5Iieren hat im letzten Jahrzehnt eine grol]e Bereicherun.g erfahren und zu einer wesentliehen FSrderung unserer Kenntnisse auf diesem Gebiete gefiihrt. Da~ wir uns bier trotzdem auch heute noch auf ,,einem der strittigsten Kapitel der gesamten Pathologie" (Orth) befinden, lehren gerade die Arbeiten bernfener Autoren der letzten Jahre. Es herrseht tiber die grundsiitzliehe Beurteilung anatomiseher und ldinischer Bilder im Bereieh der hierher gehSrigen Krankheiten in mancher Hinsicht noeh keineswegs Einigkeit. Weder naeh iir noch naeh rein anatomisehen Gesiehtspunkten, ebensowenig unter Zugrundelegung der funktionel]en StOrungen hat sieh eine Einteilang der in das grol~e Gebiet des Morbus Brightii (ira Sinne yon Volhard nnd Fahr) gehSrenden 5Iierenleiden durchftihren lassen. Es haben daher neuerdings Asehoff und Fr. )/iiiller ge[ordert, bei der Benennung dieser Krankheiten ihrePathogenese als ausschlaggebend zu betraehten.Immer mehr ist dutch die Arbeiten der letzten Jahre die grol~e Bedeutung der entziindlichen Veriinderungen tier lgalpighischen K0rper im Gesamtgebiete der Brightsehen Kl"ankheit hervorgetreten, naehdem vor einigen Jahren LShlein, im Anschlu~ an i~ltere Untersuehnngen, besonders v(m Langhans, auf ihre Wiehtigkeit ftir die Entstehung der Schrumpfniere hingewiesen haste.Von neuem geweckt wnrde das Interesse fiir die ~=ephritis dutch da, s gehiiufte Aufr yon 5Tierenentz':Jndungen bei Kriegsteilnehmern im Felde. Die Kriegsnephritis hat eine bei Beginn des Feldzuges ungeahnte Wiehtigkeit als I-teereskrankheit erlangt. Fr. Mtilier bezeiehnet sic als ein eigenartiges und ganz neues IO'ankheitsbild. Es hat aueh auf dem Gebiete der Nephritis, wie auf manchem andern, der Krieg befruehr auf die Wissenschaft gewh'kt; er hat uns die M(iglichkeit gegeben, an der Hand eines gro~en, yon vornherein gut beobaehteten Materials unsere bisherigen Kenntnisse kritiseh zu prLifen und zn erweitern.
