A cirrose hepática é um processo de cicatrização patológica, irreversível em seus estágios avançados com complicações que podem ser letais. A lesão crônica causa inflamação e fibrose hepática, com consequente formação de septos e nódulos de regeneração fibrosos, que podem levar a complicações futuras como a hipertensão portal. Dentre as suas causas, estão as hepatites virais, hepatite alcoólica, doença hepática gordurosa não alcoólica, doenças autoimunes, colestásticas e de armazenamento. Testes de fibrose não-invasivos são úteis para definir o grau de progressão da doença, com o intuito de intervir e definir a melhor conduta para cada paciente. Terapias antifibróticas, atualmente, visam à inativação da diferenciação das células hepáticas estreladas, responsáveis pela progressão fibrótica. No entanto, ainda se encontram em fase de testes, mas parecem ser promissoras. Trata-se de uma revisão bibliográfica sistemática, de natureza quantitativa, que utilizou as plataformas PubMed (Medline), Scientific Eletronic Library On-line (SciELO) e Cochrane Library como bases de dados para a seleção dos artigos, na língua inglesa. A escolha dos artigos foi realizada por meio da leitura do título, resumo e, por fim, da leitura do artigo na íntegra, sendo realizada uma análise criteriosa fundamentada nos critérios de inclusão e exclusão. Conclui-se, portanto, que as principais etiologias da cirrose hepática são as hepatites virais, hepatite alcóolica e doença hepática gordurosa não alcoólica. Observou-se que as diferentes etiologias causam diferentes formas de evolução fibrótica, que dependem da origem dos tipos celulares e mecanismos envolvidos, fundamentais para avaliar em qual estágio a terapia teria o máximo desempenho.
Acidente Vascular Encefálico (AVE) e Aneurisma Cerebral (AC) são patologias que acometem milhares de pacientes pelo mundo anualmente, são de difícil controle, possuem alta morbimortalidade e se correlacionam, já que a ruptura de um aneurisma cerebral desencadeia uma hemorragia intracraniana. Existem fatores de risco como maus hábitos de vida, tabagismo, hipertensão arterial sistêmica (HAS), idade, sexo, os quais influenciam na etiologia dessas doenças. O artigo em questão visa à revisão de literatura de aneurisma cerebral e acidente vascular encefálico (isquêmico e hemorrágico) no Brasil, ao avaliar os fatores de risco envolvidos, o sexo mais acometido, quadro clínico, diagnóstico e a abordagem inicial nesses pacientes, promovendo o melhor prognóstico e qualidade de vida. Espera-se contribuir para uma maior compreensão da fisiopatologia, fatores de risco e conduta efetiva de tais patologias, a qual permite a redução da morbimortalidade e, consequentemente, um prognóstico satisfatório nos pacientes acometidos.
Introdução: Devido à necessidade de isolamento social imposto pela Covid-19, para conter o avanço da pandemia, sucedeu-se uma mudança imprevista na rotina em todas as faixas etárias da população, o que pôde cooperar para o surgimento de alteração no sono e em hábitos alimentares, ansiedade, dentre outras, durante este período. Objetivo: Identificar na literatura dados sobre o efeito do isolamento social devido à pandemia da COVID-19 frente às crianças e adolescentes para o risco de obesidade. Métodos: Foi realizada uma revisão integrativa de literatura. A busca na literatura, ocorreu durante os meses de janeiro e fevereiro de 2023 nas bases de dados Medical Literature Analysis and Retrieval System Online (MEDLINE), Web of Science, PUBMED, Scopus, Literatura Latino Americana e do Caribe em Ciências da Saúde (SCIELO) e Scientific Eletronic Library Online (LILACS). Resultados: O isolamento social teve um efeito negativo para a saúde dessa população, considerando as associações da obesidade e suas consequências deletérias que podem aumentar durante o mesmo, devido à facilitação de um estilo de vida sedentário. Conclusão: Apesar de poucos dados na literatura, ainda assim notou-se o impacto do isolamento social por meio do aumento do sedentarismo, da obesidade e de hábitos alimentares supercalóricos. Esses fatores magnificaram a ocorrência da obesidade infantil no período da pandemia da Covid-19, o que reflete maiores riscos à saúde dessa população e evidencia a necessidade de maior atenção ao sobrepeso e obesidade infantil.
Fístulas arteriovenosas durais (FAVDs) são lesões adquiridas, que consistem em uma ou mais conexões fistulosas no interior dos folhetos da dura-máter, com envolvimento das paredes de um seio venoso dural ou então das veias leptomeníngeas adjacentes. Sua incidência é de difícil determinação, no entanto, segundo estudos, ela é estimada em 10% a 15% de todas as malformações cerebrovasculares diagnosticadas por angiografia. Os fatores predisponentes ao desenvolvimento das FAVDs são o traumatismo cranioencefálico, tromboflebite cerebral, neurocirurgia prévia e infecções. O objetivo deste estudo é apresentar um caso de Fístula Arteriovenosa Dural Intracraniana em um paciente de 58 anos, ressaltando os aspectos imagenológicos, etiológicos, fisiopatológicos e, sobretudo, os tipos de classificação da doença e a terapia utilizada. W.S.D., sexo masculino, 58 anos, natural e procedente de São Paulo – SP, deu entrada no pronto-socorro de um hospital de referência da capital paulista queixando-se de parestesia de membros superiores (MMSS) há 3 meses. Foi realizado exame físico e anamnese de forma minuciosa, na qual o paciente negou cefaleia e outros sintomas associados. Foi submetido à investigação com uma ressonância magnética (RNM) da região cerebral e medular em ponderações T1 e T2, as quais demonstraram efeito tumefativo que comprometia a transição bulbo-medular e a medula cervical de C2, as quais encontravam-se edemaciadas. Além disso, estavam proeminentes os vasos leptomeníngeos nos hemisférios cerebelares, estes patognomônicos da FAVD. Ademais, foi observado hipersinal em T2, com padrão estriado/tigroide, típico de degeneração mielínica progressiva. O paciente apresentava agressivo refluxo venoso cortical com drenagem perimedular espinhal na veia cortical e, dessa forma, enquadrou-se na Classificação de Cognard tipo V, sendo considerado um paciente portador de FAVD maligna. Foi então realizado tratamento endovascular com embolização transarterial, o qual proporcionou fechamento completo da fístula e, dessa forma, o paciente obteve um prognóstico favorável. As fístulas arteriovenosas durais, por serem uma condição rara e com variadas manifestações clínicas, podem passar despercebidas pelo profissional médico. Dessa forma, é de fundamental importância o conhecimento da doença, com o intuito de proporcionar ao paciente um diagnóstico precoce e uma terapia eficaz.
