INTRODUÇÃO: Dado o envelhecimento populacional, torna-se relevante avaliar a sobrecarga e a qualidade de vida de quem cuida dos idosos. Essa prática costuma ser árdua. O cuidador precisa acrescentar, às atividades diárias, tarefas referentes ao idoso, expondo-se a um elevado padrão de burden (sobrecarga), ou aspectos negativos associados ao cuidado de dependentes. Isso pode levar a uma piora da qualidade de vida e consequente adoecimento. OBJETIVOS: Avaliar o perfil sóciodemográfico e a relação entre a sobrecarga e a percepção de qualidade de vida de cuidadores de idosos do Núcleo de Atendimento à Terceira Idade do Exército (Natiex). MÉTODOS: 61 participantes foram investigados a partir dos instrumentos WHOQOL-BREF e Zarit Burden Interview. RESULTADOS: Dos 61 cuidadores avaliados, 78,6% eram do sexo feminino, 75,4% filhas, 50,8% casados, 62,3% com mais de 50 anos e 95,1% exerciam a atividade sem remuneração. Foi identificada uma relação estatisticamente significativa entre o aumento da sobrecarga e a piora da percepção de qualidade de vida (p<0,01). CONCLUSÕES: À medida que aumenta a sobrecarga, ocorre uma piora da percepção de qualidade de vida em todos os domínios: relações, psicológico, físico e ambiental (domínio ambiente teve o maior impacto). Os resultados encontrados foram superiores a outras amostras brasileiras, possivelmente pela maior facilidade do militar a recursos de saúde. Grupos de apoio a cuidadores devem ser estimulados como prevenção primária das morbidades relacionadas ao cuidar, tendo em vista a demanda crescente de idosos dependentes em nosso meio.
Background:The treatment of older adults with pain is complex and affected by age-related changes in pharmacokinetics and pharmacodynamics. Chronic pain encompasses a complex array of sensory-discriminatory, motivational-affective, and cognitive-evaluative components. Because of this complexity, both pharmacologic and nonpharmacologic approaches should be considered to treat pain.Aims:Given the large number of older persons with pain at the end of life and the few data about this issue, the objective of this article is to review the treatment of pain in this population.Patients and Methods:We searched The Cochrane Library, MEDLINE and LILACS from 1990 to 2011 and the references in retrieved manuscripts. The search terms were pain AND elderly AND end of life.Results:There are evidences of undertreatment among elderly people. The association of nonpharmacologic resources with the pharmacological treatment can help reduce the use of analgesics minimizing the side effects of long term medication. Pharmacological treatment is escalated in an orderly manner from non-opioid to weak opioid to strong opioid. Adjuvant drugs like anticonvulsants and antidepressants may be necessary.Conclusions:The sequential use of analgesics drugs and opioids are considered effective and relatively inexpensive for relieving pain, but no well designed specific studies in the elderly patient are available. There are not specific recommendations about the long-term use of complementary and alternative therapies and although their effectiveness remains unproven they should not be discouraged. Palliative sedation may be a valid palliative care option to relieve suffering in the imminently dying patient.
ResumoAs drogas anticolinérgicas podem causar efeitos adversos, mais freqüentemente nos pacientes idosos. Relatamos um caso de demência reversível e quedas, associadas ao uso de biperideno. A paciente, com 82 anos, foi admitida em lar geriátrico devido a quedas freqüentes no domicílio, acentuado défi cit cognitivo, tremores de extremidades ao movimento e perda de autonomia. Na verdade, era portadora de tremor essencial, que foi confundido com doença de Parkinson e tratado com biperideno; posteriormente, desenvolveu défi cit cognitivo que foi erroneamente diagnosticado como demência do tipo Alzheimer. Após submeter-se à avaliação especializada, foi suspensa a droga anticolinérgica e houve reversão completa do quadro demencial; o tremor essencial foi controlado com uso de propranolol. Ao avaliar um paciente com défi cit cognitivo, o clínico deve descartar possíveis causas de demência reversível, em especial, o grupo das iatrogênicas.Palavras-chaves: Biperideno, demência reversível, drogas anticolinérgicas, idoso, quedas. AbstractAnticholinergic drugs may cause adverse effects, more often in the aged patients. We describe a case of reversible dementia and falls, associated with the use of biperiden. The patient was an 82-year old woman, admitted in a geriatric home because of frequent falls at home, progressive cognitive defi cit and tremor of extremities with movement, in addition to remarkable loss of autonomy. As a matter of fact, she presented an essential tremor, which was mistakenly diagnosed as Parkinson disease and treated with biperiden; furthermore, she developed a drug-induced cognitive defi cit, erroneously interpreted as Alzheimer disease. After submitted to the specialized evaluation, the anticholinergic drug was discontinued and there was a complete improvement of cognition changes; the essential tremor is under control with use of propranolol. When evaluating a patient with cognitive defi cit, the clinician must discard possible causes of reversible dementia, especially the iatrogenic group.
