INTRODUÇÃOO conceito de medicamento como agente de cura já era associado ao seu potencial de dano pelas civilizações arcaicas da Mesopotâmia e Egito. O termo shêrtu, que aparece nos manuscritos da época, tem significado simultâneo de doença, pecado ou castigo divino. Somente através de uma operação de catarse, em que era atribuído um conteúdo mágico ao medicamento, o indivíduo alcançaria a purificação de seus pecados e o restabelecimento da saúde por intervenção dos deuses (Weatherall, 1990).Estes conceitos influenciaram o entendimento sobre a patologia e a terapêutica durante muitos séculos e persistem, em alguns aspectos, até nossos dias. A própria palavra fármaco teve origem a partir do termo grego pharmak, que significa "aquilo que tem o poder de transladar as impurezas". Entre os gregos, vítimas dos sacrifícios oferecidos aos deuses eram chamadas de pharmakó, e o alimento utilizado durante as cerimônias de comunhão, phármakon. Objetivo: Analisar as Relações Municipais de Medicamentos Essenciais (Remumes) do estado de São Paulo e estimar a prevalência dos grupos de medicamentos. Métodos: Trata-se de um estudo transversal realizado com 17 Remumes dos municípios sede dos Departamentos Regionais de Saúde (DRS) e 24 Remumes de municípios do DRS XIII do estado de São Paulo. Verificaram-se dados sobre os medicamentos de acordo com o primeiro nível da classificação Anatomical Therapeutic Chemical. Resultados: Os municípios sedes apresentaram entre 128 (Piracicaba) e 396 (Santos) medicamentos listados, com média de 272,3 (DP=71,1) medicamentos. Os municípios da DRS XIII apresentaram entre 32 (Luiz Antônio) e 403 (Cássia dos Coqueiros) medicamentos, com média de 198 (DP=111,4). Predominaram os medicamentos que atuavam no sistema nervoso seguidos pelos cardiovasculares e anti-infecciosos sistêmicos. Conclusões: As Remumes mostraram elevada variação no número de medicamentos e grupos farmacológicos e a necessidade de qualificação da assistência farmacêutica municipal.
The use of biopolymers in sustained release systems has been studied by many research groups because of the bioavailability and biodegradability of these compounds. Casein is a natural biopolymer whose degradation results in biologically utilisable compounds. The objective of the present study was to assess the potential of casein microcapsules (CAS/MC) as sustained release systems using acetaminophen as a model drug. CAS/MC were prepared by aqueous coacervation in lactate buffer containing gelatin, hydroxypropyl cellulose (HPC) and lecithin. After preparation, the microcapsules were treated, or not, with glutaraldehyde as a cross-linking agent. CAS/MC were loaded using two distinct procedures, either by dissolving 50% of the drug (w/w), relative to casein, in the polymer dispersion or by dissolving the drug in the coacervating solution. The drug present in CAS/MC was quantified by HPLC after an enzymatic degradation assay, and the CAS/MC were analysed by scanning electron microscopy and thermal analysis (differential scanning calorimetry and thermogravimetrical analysis). Loading of the drug was approximately 8% (w/w), with high resistance to enzymatic attack. The absence of an acetaminophen melting peak indicated that there was no drug present on the surface of the cross-linked systems. In addition, loading was accompanied by a reduction of the specific heat capacity of the systems, which suggests a decrease in stability. The outer morphology of the encapsulating polymer was affected by the process of microencapsulation. The data suggest that the microencapsulation process of aqueous coacervation and cross-linking is appropriate for the preparation of microencapsulated systems for sustained drug delivery.

The aim of the present work was to assess the adherence to medication from polymedicated patients before and after the use of a Drug-dispensing System with Usage Control (DDSUC) and compare the levels of the clinical parameters -blood pressure, postprandial glycemia, glycated hemoglobin, triglycerides and cholesterol. DDSUC consisted of a monthly drug-dispensing package, in the shape of a blister with a calendar. This quasi-experimental study was performed in a Basic Health Unit. Twenty four patients were selected to use DDSUC for 4 months. Medication adherence was assessed through Morisky-Green test. Among the participants of the study, 62.5% were women and the average age was 67 years old. Before the use of DDSUC, 83.3% of the patients were considered as "less adherent". After the use of the system, 100% were considered as "more adherent" (p < 0.01), the means of the systolic blood pressure decreased 23.7 mmHg (p=0.000), the diastolic blood pressure decreased 12.1 mmHg (p=0.004) and glycemia diminished 79.3 mg/dl (p=0.000). The use of DDSUC improved the adherence to medication and decreased the values of the clinical parameters, making patients safer when it comes to respecting the correct use of their medication.Uniterms: Medication/treatment adherence. Polymedication.O objetivo deste estudo foi avaliar a adesão ao tratamento medicamentoso de pacientes polimedicados antes e após o uso de um Sistema de Dispensação de Medicamentos e Controle de Uso (SDMCU) e comparar os níveis dos parâmetros clínicos -pressão arterial, glicemia pós-prandial, hemoglobina glicada, triglicérides e colesterol. O SDMCU foi constituído por uma embalagem mensal de dispensação de medicamentos, em forma de blister com calendário. Este estudo, do tipo quase-experimental, foi realizado em uma Unidade Básica de Saúde. Selecionaram-se 24 pacientes para utilizar o SDMCU por 4 meses. A adesão medicamentosa foi avaliada através do Teste de Morisky e Green. Entre os participantes do estudo, 62,5% eram mulheres e a idade média foi de 67 anos. Antes do uso do SDMCU, 83,3% dos pacientes foram considerados "menos aderentes". Após o uso do sistema, 100% foram considerados "mais aderentes" (p<0,01), a média da pressão arterial sistólica diminuiu 23,7 mmHg (p=0,000), a diastólica diminuiu 12,1 mmHg (p=0,004) e a glicemia diminuiu 79,3 mg/dL (p=0,000). O uso do SDMCU melhorou a adesão medicamentosa e diminuiu os valores dos parâmetros clínicos, proporcionando aos pacientes uma segurança no que diz respeito à utilização correta de seus medicamentos.Unitermos: Medicação/adesão ao tratamento.
