A cirurgia metabólica apresenta-se como estratégia promissora no tratamento do diabetes mellitus tipo 2, com vários estudos demonstrando a eficácia sobre o controle glicêmico e outros desfechos metabólicos. No entanto, os mecanismos subjacentes ainda estão sob investigação e estudos complementares são necessários para determinar seus efeitos a longo prazo e eficácia frente às terapias medicamentosas convencionais. OBJETIVOS: Analisar as repercussões das diferentes modalidades de cirurgia metabólica sobre o diabetes mellitus tipo 2. MÉTODOS: Trata-se de revisão integrativa realizada por meio das bases de dados PUBMED, SCIELO E COCHRANE entre os meses de setembro e outubro de 2022 utilizando os descritores MESH “Metabolic surgery” e “Type 2 Diabetes mellitus”, articulados com o operador booleano “AND”. Foram aplicados filtros para obtenção de artigos publicados entre 2017 e 2022; em línguas portuguesa, inglesa e espanhola; e disponíveis na íntegra. RESULTADOS: Foram encontrados 424 artigos, dos quais 32 foram selecionados por melhor se adequarem ao tema proposto. DISCUSSÃO: Diversas modalidades cirúrgicas estão disponíveis, sendo o Bypass Gástrico em Y de Roux, a abordagem padrão-ouro. A melhoria do perfil glicêmico entre pacientes com DM2 se dá de maneira dependente e independente da perda de peso. Numerosos estudos confrontam os procedimentos entre si, bem como as terapias cirúrgicas e terapias médicas convencionais. Porém, uma minoria explora a comparação às terapias medicamentosas específicas. CONCLUSÕES: Os mecanismos pressupostos aos resultados da cirurgia estão sendo desvendados, contudo deve-se reconhecer a incompletude dos conhecimentos adquiridos até então. Apesar da totalidade dos estudos analisados demonstrarem superioridade da cirurgia, mais evidências são necessárias.
O diabetes mellitus (DM) é uma doença crônica que, quando não controlada, implica diversas complicações agudas e crônicas ao indivíduo, e por isso, o controle da doença deve ser realizado de maneira a evitar hiper/hipoglicemias. Ao longo dos últimos anos, bastantes ferramentas surgiram com a proposta de melhorar a monitorização e a regulação da glicose, facilitando decisões clínicas e melhorando o tratamento. Visto isso, esse trabalho objetivou realizar uma revisão integrativa afim de avaliar o impacto das novas tecnologias na melhora do controle da glicemia e qualidade de vida do paciente com DM. Com esse intuito, foram analisados 14 trabalhos acerca do tema e após observados os resultados, concluiu-se que esses recursos permitem ao paciente menos hipoglicemias e maior controle glicêmico, além de confiança no tratamento e sensação de bem-estar. Desse modo, conhecer sobre essas tecnologias é importante para facilitar a difusão e manejo destas pelos pacientes e profissionais de saúde.
RESUMOOs corticoesteroides são uma classe de hormônios esteroides produzidos no córtex da suprarrenal e estão relacionados com diversas funções orgânicas, como a resposta ao stress, resposta imunitária, regulação da inflamação, metabolismo dos carboidratos, catabolismo das proteínas, regulação hidroeletrolítica e comportamento. Estes hormônios podem ser sintetizados em laboratórios dando origem a uma classe de medicamentos muito utilizados atualmente em certos tipos de doenças ou ainda como tratamento profilático no caso de transplantes. Apesar da sua importância, o tratamento com esse tipo de medicamento, deve ser acompanhado pelo médico com parcimônia, pois podem causar vários efeitos colaterais que comprometem a qualidade de vida do paciente. Dessa forma, o presente estudo buscou identificar e discorrer sobre as principais manifestações adversas no organismo que resultam do uso dessa classe de medicamentos. Para esse fim, foi feita uma revisão de literatura, a partir da busca nas bases de dados do "Pubmed" e do "UptoDate". Com isso, identificamos um amplo espectro de manifestações metabólicas e neuropsíquiátricas que dependem da dose e da duração do tratamento com glicocorticoides e selecionamos as que são dignas de maior detalhamento. Conclui-se que antes de se iniciar a administração de glicocorticoides, é necessária uma avaliação detalhada do indivíduo, levando em consideração os possíveis riscos e os benefícios ao paciente como um todo.
