The establishment of the bond between mother and baby in the postpartum period is important for ensuring the physical and psychological health of both. This short communication reports the first phase of the cross-cultural translation and adaptation to the Brazilian context of the Postpartum Bonding Questionnaire (PBQ). Four aspects of equivalence between the original scale and the Portuguese version were evaluated: the conceptual, semantic, operational and item equivalences. Literature review, the study of PBQ history, translation, expert evaluation, back-translation and pretests involving 30 mothers with children aging up to 7 months using a primary healthcare unit were conducted. Each step demonstrated the need for adjustments, which were made during the adaptation process. At the end of the study, a version of PBQ in Brazilian Portuguese equivalent to the original one was obtained, offering promise for national studies on the mother-baby bond, and its influence on health, and for use in health services.
ResumoSabe-se que o consumo de substâncias psicoativas entre as mulheres apresenta fatores predisponentes, mantenedores e consequências diferentes do que entre os homens. A violência doméstica entre parceiros íntimos, além de ser um problema grave e com alta incidência entre a população feminina, representa um dos principais fatores associados à dependência química na mulher. A partir de revisão de literatura, foi verificado o cenário nacional e internacional atual dos impactos da violência doméstica entre mulheres usuárias de drogas. Verificouse a urgência de programas de prevenção à violência doméstica e de atenção psicossocial e especializada às crianças e mulheres que sofreram esse tipo de violência, tendo em vista prevenir o uso abusivo de substâncias como forma desadaptativa de manejar o sofrimento decorrente da violência. Assim, concluiu-se que o histórico de violência doméstica deve ser investigado em toda avaliação e tratamento de mulheres dependentes de substâncias, e a interrupção da violência e a proteção da mulher devem ser prioridades no projeto terapêutico.Palavras-chave: Transtornos relacionados ao uso de substâncias, gênero, violência doméstica, gênero e saúde. AbstractPsychoactive substance use among women is known to have predisposing factors, maintenance factors, and consequences that are different from those applicable to men. Domestic violence and intimate partner violence, in addition to being a serious problem, with a high incidence in the female population, represents one of the main factors associated with chemical dependence in women. A literature review was conducted to assess the current national and international scenarios of the impacts of domestic violence among female drug users. The study revealed an urgent need for programs designed to prevent domestic violence, as well as for psychosocial and specialized care for children and women who have suffered this type of violence, with a view to preventing substance abuse as a maladaptive way of managing the suffering caused by violence. In conclusion, history of domestic violence should be investigated in the assessment and treatment of substance-dependent women; violence interruption and protection of women should be treatment plan priorities.Keywords: Substance-related disorders, gender, domestic violence, gender and health. IntroduçãoDe acordo com o art. 5º da Lei nº 11.340, de 7 de agosto de 2006, conhecida como Lei Maria da Penha, a violência doméstica (VD) é caracterizada por qualquer tipo de violência contra a mulher que leve ao sofrimento físico (inclusive lesões ou morte), sexual ou mental, dano moral ou patrimonial 1 . A lei é aplicada ao contexto doméstico e familiar, compreendendo as pessoas que convivem no mesmo espaço, que se considerem ou não parentes. Também é aplicada em casos de relação íntima de afeto (violência por parceiro íntimo -VPI), em que o agente da violência convive ou convivia com a mulher agredida ainda que não morassem no mesmo lugar 1 . A depressão perinatal (DP) é a complicação obstétrica com maiores índices de subdiagnóstico e um importante fator de risco para o suicídio, considerado uma das causas mais comuns de mortalidade materna. Consequentemente, a atenção à saúde mental deve ser considerada uma das prioridades médicas durante a gestação e o puerpério, pois a identificação precoce da DP pode produzir benefícios substanciais para a saúde materna, infantil e familiar. O principal instrumento psicométrico para a prevenção secundária da DP é a Escala de Depressão Pós-Parto de Edimburgo, validada para brasileiros e para aplicação por telemedicina (TM). A utilização da TM é considerada um grande avanço também na obstetrícia. A aplicação de instrumentos psicométricos por TM produz informações com acurácia semelhante ou maior do que o modo de registro local com papel e caneta, além de permitir maior honestidade devido à maior percepção de privacidade, reduzindo, assim, a influência da psicofobia. Portanto, a utilização da TM e da telepsiquiatria na saúde mental da mulher apresenta-se como um recurso estratégico para ampliar o acesso e melhorar os resultados do rastreamento da DP.
O uso de escalas na prática clínica é bastante conhecido e pode ser útil quando os instrumentos são utilizados como parte do processo de avaliação diagnóstica, na identificação da presença ou ausência de um determinado transtorno, no monitoramento do progresso terapêutico e na quantificação e documentação da gravidade de determinados sintomas. O período perinatal pode estar associado a desfechos adversos e impactar de maneira negativa a saúde mental das mulheres. Embora existam poucos instrumentos especificamente desenvolvidos para essa subpopulação, há uma uma crescente tendência em se afirmar que o monitoramento cuidadoso e contínuo de sintomas e condições psiquiátricas prevalentes nesse período através de questionários de autoavaliação pode trazer benefícios na tomada de decisão ou busca de acompanhamento especializado e precoce, quando necessário.
A cannabis é a substância psicoativa ilícita mais consumida pelas gestantes no Brasil e no mundo. Muitos estudos foram publicados tendo em vista a saúde do feto e do bebê em aleitamento materno, porém poucos focaram na saúde da gestante e da puérpera usuária de cannabis. O novo papel e os desafios de ser mãe podem ser protetivos para a cessação do uso de drogas, porém para mulheres em situação de vulnerabilidade podem representar risco para a manutenção da dependência. O presente artigo de revisão de literatura apresenta uma visão global sobre o uso de cannabis pelo gênero feminino, destacando-se as particularidades dos impactos desse consumo na gravidez e no pós-parto e as intervenções necessárias para a atenção e cuidado à mãe.
