Verrucous carcinoma (VC) is an uncommon type of low-grade, well differentiated and non-invasive squamous cell carcinoma (SCC). The development of invasive SCC within a VC is rare and distinction from the classical oral SCC is a frequent dilemma for pathologists. We report a case of a 78 years old male former smoker, alcoholic, presenting a painless leukoplastic verrucomatoous lesion in mandibular alveolar ridge measuring approximately 4cm diameter, with 2 months evolution. Histological examination of the incisional biopsy revealed proliferation of well-differentiated squamous epithelium forming high exophytic hyperkeratinized papillary projections and deep blunt buds. In focal areas, invasive tumor epithelial nests with more apparent cytologic atypia were observed. Immunohistochemical analysis showed intense expression of p16 but weak positivity for ki-67. The diagnosis was VC with microinvasive SCC areas. The patient underwent surgery with a safety mar- gin and is under follow-up. “Hybrid” lesions composed of typical VC and invasive SCC accounts for about 20% of oral VC. A discussion on the criteria of differential diagnosis and biological behavior of these lesions is also provided.
Introdução: Estima-se que a incidência da síndrome de Down seja de um a cada 660 nascimentos. Sendo as anomalias dentárias frequentes nessa parcela da população, é importante seu correto diagnóstico para o planejamento do tratamento. Objetivo: Realizar o levantamento das alterações dentárias, como as anomalias dentárias de desenvolvimento, por meio de radiografi as panorâmicas. Métodos: Foram avaliadas radiografi as panorâmicas realizadas em 27 pacientes com síndrome de Down atendidos na Unidade de Diagnóstico Oral e Odontologia para Pessoas Especiais (UDOPE), entre agosto de 2011 e julho de 2012, em comparação com 27 pacientes não sindrômicos atendidos no Departamento de Odontologia da UFS, no Hospital Universitário do município de Aracaju, SE. Os dados foram tabulados e analisados estatisticamente através do teste Qui-quadrado com nível de signifi cância de 5% e o odds ratio, comparando os grupos e com a literatura. Resultados: Dos pacientes com síndrome de Down, dezoito apresentaram-se com giroversão (67%), dez apresentaram um ou mais casos de agenesia (37%), cinco apresentaram um ou mais dentes conoides (19%), quatro com um ou mais dentes não irrompidos (15%), três com um ou mais microdentes (11%), dois pacientes com no mínimo um caso de dilaceração apical (7%), dois apresentaram um ou mais dentes com hipercementose (7%), um paciente com retenção prolongada no arco (4%) e um com taurodontismo (4%). Conclusões: As anomalias dentárias foram encontradas em alta prevalência no grupo com síndrome de Down, dentre elas a giroversão foi a mais prevalente.
