Zusammenfassung. Fragestellung: Erhöht sich nach einer stationären Entwöhnungsbehandlung der Anteil Erwerbstätiger und welche Merkmale sind prognostisch relevant? Methode: Retrospektive Feldstudie in der mittels McNemar Test Veränderungen im Erwerbsstatus, mittels Chi Quadrat Test und dreifaktorieller Varianzanalyse Unterschiede zwischen Erwerbstätigen und Arbeitslosen und mittels binärer logistischer Regression Prädiktoren ermittelt wurden. Ergebnisse: Nach einer Entwöhnungsbehandlung der 396 alkoholabhängigen Patienten nahm der Anteil Erwerbstätiger signifikant zu. 89,2 % der zu Behandlungsbeginn Erwerbstätigen (N = 194) blieben erwerbstätig, obwohl sie in der Regel in ihrer Arbeitsfähigkeit stark gefährdet waren. Bei den Arbeitslosen (N = 150), von denen 28 % erwerbstätig wurden, waren sechsmonatige Abstinenz nach Behandlungsende (OR = 3,3) und jüngeres Alter (OR = 2,8) die stärksten Prädiktoren für Erwerbstätigkeit zur Ein-Jahres-Katamnese. Etwa ein Drittel der Patienten waren trotz Alkoholkonsum zur Ein-Jahres-Katamnese erwerbstätig. Jüngere Patienten waren ein Jahr nach Behandlungsende eher erwerbstätig, ältere Patienten eher abstinent. Schlussfolgerungen: Die Ergebnisse bestätigen die Hypothese, dass eine stationäre Entwöhnungsbehandlung sowohl zur Abstinenz als auch zu einer Zunahme Erwerbstätiger führt. Obwohl Abstinenz die wichtigste Voraussetzung für Erwerbstätigkeit ist, wäre zu prüfen, welchen Patienten unter welchen Bedingungen und mit welchen Trinkmustern es gelingt, erwerbstätig zu werden oder zu bleiben.
Zusammenfassung. Fragestellung: Gestatten die Merkmale „Anzahl vorheriger Entgiftungen“, „Depressivität“ und „Selbstwirksamkeitserwartung, SWE“ eine Prognose der Abstinenz zur Ein-Jahres Katamnese? Methode: Prospektive Replikations-Feldstudie in einer anderen Klinik, in der mittels binärer logistischer Regression und Chi Quadrat Tests Unterschiede in Patienten-Merkmalen zwischen zur Ein-Jahres-Katamnese durchgehend abstinenten (N = 285) vs. rückfällig gewordenen Alkoholabhängigen (N = 274) analysiert wurden. Ergebnisse: Ebenso wie in unserer vorherigen Studie waren Alter, Geschlecht, Schulbildung, Arbeitslosigkeit, Familienstand, Partnersituation, suchtspezifische und psychische Komorbidität prognostisch nicht relevant, Ausnahme Persönlichkeitsstörungen. Wiederum hatten Patienten mit weniger als zwei Entgiftungen und einer hohen SWE die höchste Wahrscheinlichkeit ein Jahr durchgehend abstinent zu leben (82 %). Ebenso bestätigt wurde, dass Verbesserungen in der psychischen Belastung nicht mit Abstinenz korrelieren. Depressivität und vorherige Entwöhnungsbehandlungen wurden nicht als Prädiktoren repliziert. Schlussfolgerungen: Vorherige Entgiftungen, SWE und Persönlichkeitsstörungen könnten für den hier untersuchten Kliniktyp allgemeinverbindliche Prädiktoren sein. Inwiefern der Ausschluss der beiden Prädiktoren Depressivität und vorherige Entwöhnungsbehandlungen eine Folge neu eingeführter Interventionen für Depressive und Therapieerfahrene ist, wäre zu prüfen. Wie in unserer vorherigen Studie gestattet das Regressionsmodell trotz geringer Varianzaufklärung und mittlerer Effektstärken die Ableitung kausaler Hypothesen zur klinikspezifischen Verbesserung der Behandlung. Replikationsstudien sollten ebenso wie die empirische Orientierung ein fester Bestandteil verhaltenstherapeutischer Behandlung sein.
Zusammenfassung Fragestellung Inwiefern unterscheiden sich alkoholabhängige Frauen und Männer zu Beginn einer stationären Behandlung. Methode Naturalistische Feldstudie, in der retrospektiv mittels Chi2-Test, Mann-Whitney-U-Test, Kruskal-Wallis-Test und 3-faktorieller Varianzanalyse Unterschiede zwischen Patientenmerkmalen der Frauen (N=1040) und Männer (N=1570) analysiert wurden. Ergebnisse Die Männer waren häufiger ledig und arbeitslos, hatten eine bessere Schulbildung, waren im Interaktionsstil eher dominant und konkurrierend. Die Frauen hatten häufiger eine Posttraumatische Belastungs- und eine Essstörung, waren eher nachgiebig und fürsorglich und nur im höheren Alter häufiger depressiv als die Männer. Besonders jüngere Frauen tranken Alkohol bei unangenehmen Gefühlen und körperlichen Beschwerden. Die jüngeren Männer tranken eher Alkohol in Gesellschaft und bei angenehmen Gefühlen. Die motivationale Inkongruenz war bei den Frauen insgesamt stärker ausgeprägt. Das Leben zu genießen und Selbstvertrauen zu gewinnen waren für die Frauen wichtige motivierende Teilziele der Abstinenz. Schlussfolgerungen In ihrer Kombination erlauben die Ergebnisse die Interpretation, dass sich alkoholabhängige Frauen im Vergleich zu alkoholabhängigen Männern in einer schwierigeren sozio-ökonomischen Situation befinden, dass sie selbstunsicherer sind und wahrscheinlich eine höhere auf positive Ziele orientierte Therapiemotivation haben. Eine Beschränkung auf geschlechtsspezifische Unterschiede reicht für eine genderspezifische Forschung und Behandlung nicht aus. Lebensrelevante Merkmale wie Alter, Partner- und Arbeitssituation sollten bei der genderspezifischen Forschung und Therapieplanung berücksichtigt werden.
Zusammenfassung. Hintergrund: Feldtheoretische Überlegungen veranlassten uns, entgegen dem in Katamnesestudien üblichen Vorgehen, eine getrennte Auswertung für Frauen und Männer verschiedener Altersgruppen vorzunehmen. Unsere Fragestellung lautete, inwiefern sich je nach Geschlecht und Alter unterschiedliche Prädiktoren für Abstinenz ergeben? Methode: Katamnesestudie alkoholabhängiger Personen nach stationärer verhaltenstherapeutischer Behandlung, in der retrospektiv mittels binärer logistischer Regression und Mann-Whitney-U Tests Unterschiede zwischen zur Ein-Jahres-Katamnese rückfälligen und abstinenten Patientinnen (N=712) und Patienten (N=1019) getrennt für verschiedene Altersgruppen analysiert wurden. Zur Erreichung ausreichender Stichprobengrößen wurden die Altersgruppen mittels Quartilen gebildet: 19-41, 42-48, 49-54, 55-79 Jahre. Ergebnisse: Frauen und Männer unterschieden sich nicht in der Abstinenzquote, mit Ausnahme der über 54-jährigen. Mit zunehmendem Alter steigt bei beiden Geschlechtern die Wahrscheinlichkeit für Abstinenz. Signifikante Prädiktoren (p<.01) mit mittlerer Effektstärke für Rückfall waren: vorherige stationäre Entwöhnungsbehandlungen bei den 42- bis 48-jährigen Frauen (OR: 3.6) und den über 54-jährigen Männern (OR: 4.5) und psychische Belastung bei den 42- bis 54-jährigen Männern (r=0.30). Rückfallkritische motivationale Schemata waren hohe Werte der Unzufriedenheit im Bereich das „Leben genießen“ (r=31) und „Leistung“ (r=36) bei den über 54-jährigen Frauen, „Kontrolle haben“ (r=30) bei den 42- bis 48-jährigen Männern und „Alleinsein“ bei den 49- bis 54-jährigen Männern (r=30). Schlussfolgerungen: Unsere Ergebnisse sprechen für eine geschlechter- und alterssensible Forschung und Behandlung unter Einbeziehung sozioökonomischer Merkmale und motivationaler Schemata. Insbesondere bei den unter 42-jährigen Frauen und Männern besteht wegen der hohen Rückfallquote und fehlender Prädiktoren erhöhter Forschungsbedarf.
