Изучить факторы, ассоциированные с неблагоприятным годовым прогнозом у больных с острым нарушением мозгового кровообра-щения, и прогностическую роль патологического лодыжечно-плечевого индекса (ЛПИ). Материал и методы. В исследование включено 345 пациентов (возраст 63,6±7,8 лет; 181 мужчин и 164 женщин) с ишемическим инсультом, которые наблюдались в течение 1 года. Пациенты были разделенные на 2 группы. Группа 1 включала пациентов с благоприятным исходом инсульта, а группа 2 -пациентов, которые в течение года имели какие-либо клинические события, в том числе летальный исход. Всем па-циентам проводилось стандартное неврологическое и инструментальное обследование, а также оценка состояния периферических артерий с помощью сфигмоманометрии. Результаты. Группы не различались по возрасту и полу. Частота неблагоприятных исходов (летальный исход, повторное нарушение мозго-вого кровообращения, сердечно-сосудистые события) через 1 год после перенесенного ишемического инсульта составила 29,5%. Наибо-лее часто в группе 2 выявлялись хроническая сердечная недостаточность, фибрилляция предсердий, перенесенные ранее сосудистые события, наличие периферического атеросклероза, избыточная масса тела. Пациенты группы 2 исходно имели более грубый неврологический дефицит. Патологический ЛПИ выявлен у 70,7% пациентов группы 2, в то время как у пациентов группы 1 он выявлен у 33,8% (р=0,000001). При проведении регрессионного анализа выявлена сильная взаимосвязь патологического ЛПИ с неблагоприятным исходом инсульта. Заключение. При ишемическом инсульте патологический ЛПИ позволяет выявить пациентов с периферическим атеросклерозом и прово-дить у них целенаправленные профилактические мероприятия. Стратификация риска может способствовать более эффективной вторичной профилактике у пациентов с цереброваскулярной патологией.Ключевые слова: лодыжечно-плечевой индекс, инсульт, факторы риска, исходы инсульта. Aim. To study the factors associated with a poor annual prognosis in patients with acute stroke and prognostic role of pathological ankle-brachial index (ABI). Material and methods. The study included 345 patients (age 63.6±7.8 years, 181 males and 164 females) with ischemic stroke that were observed for 1 year. Introduction. Ischemic stroke is the most common disease of the brain vessels worldwide. In recent years, we began to pay attention to the fact that the evaluation of the ABI may have prognostic value in different cohorts studied, not only in epidemiological studies, but also in patients with the presence of cardiovascular disease. The aim of the study was to investigate the prognostic role of abnormal ABI in patients with ischemic stroke. Material and methods. The study included 345 patients with a diagnosis of ischemic stroke. All patients underwent the study status of peripheral arteries using VaSera VS-1000 device. Groups of normal (n = 193) and abnormal ABI (n = 152). Patients were followed up for 1 year, assessed cardiovascular events were compared by the presence of risk factors, severity of neurological deficit, and according to laboratory tests and ultrasound data of the peripheral arteries. Results. The frequency of adverse outcomes (death, recurrent stroke, cardiovascular events) in 1 year after ischemic stroke were 15.6% of patients with a normal ABI and 46.7% of patients with abnormal ABI (p = 0.0001). In patients with abnormal ABI often preserved neurological deficit and the degree of its severity was higher compared to patients with normal ABI. When carrying out logistic regression analysis revealed strong correlation with pathological ABI adverse stroke outcome (OR 4.7; 95% CI 2,8-7,9, p = 0.00008). Conclusions. Evaluation of the ABI can help to identify a group of patients with ischemic stroke with increased risk of subsequent cardiovascular events requiring additional effort on their secondary prevention.
