ObjectiveTo evaluate the diagnostic validity of fine-needle capillary cytology (FNCC) in palpable tumours.Material and methodsA retrospective, single-tray, cross-sectional diagnostic test study was carried out. We reviewed the cytological reports of the case files of the Cytology Unit of the Northern Regional Institute of Neoplastic Diseases (IREN) from January 2012 to December 2016.ResultsA total of 332 patients were selected, with an average age of 54.77 years (range 13–90 years); 61.4% of patients were female. The most frequent anatomical sites were lymph nodes (49.7%), thyroid (13.3%), breast (12.3%) and soft tissues (11.4%). Twenty-five cytologies did not have a histological correlation and six showed an atypical result. In the lymph node study, the most frequent pathology was metastatic carcinoma (49.7%), followed by lymphoma (13.3%). The FNCC had a sensitivity of 99.55%, a specificity of 98.77%, a positive predictive value of 99.55% and a negative predictive value of 98.77%. The positive likelihood ratio was 80.63%.ConclusionsFNCC is a useful, safe, reliable and economical ambulatory technique with minimal complications and high diagnostic accuracy.

SUMMARYObjective: The main objective was to identify the clinico-pathologic-picture and the influence on prognosis of some variables. Material and methods: We analized retrospectively the files of 44 adults patients with soft -tissue sarcomas managed at Belen Hospital, Peru, between 1967 and 1994. Estimation of survival was made by the method of Kaplan -Meier and to assesss the prognostic variables we used the log-rank test. Results: The median age was 46± 19 years (range, 16 to 85 years) and there were 21 males and 23 females (M:F, 0.9:1). The most common complaints were tumor and pain. Twenty -three tumors (52%) were located in the lower and 9 (21%) in the upper extremities. Most patients were classified in UICC stage II (48%), followed by stage III (23%), stage IV (18%) and stage I (11%). The majority of tumors were fibrosarcomas (34%), liposarcomas and rhabdomyosarcomas (14%). Thirteen tumors (30%) were classified as high -grade, 24 (55%) as intermediate -grade and 7 (16%) as low -grade. Twenty -seven patients underwent surgery, 13 received adjuvant chemotherapy and/or radiotherapy, and 4 were treated with primary chemotherapy and/or radiotherapy with 5 -year survival rates of 54%, 11 %, and 0%, respectively. The 5 -year survival rate for the entire group was 38%. According to an univariate analysis of the dependence of survival on variables, the following factors were significant: Age (p < 0.01), lenght of disease (p < 0.001), site (p < 0.001), lymph node status (p< 0.001), histologic grade (p< 0.01), clinical stage (p < 0.001) and type of treatment (p <0.001). Conclusion: The major factor in survival outcome is the ability to completely resect lesion. The most common histologic type was the transitional cell carcinoma (75%), followed by the squamous cell carcinoma (18.7%), and adenocarcinoma (6.3%). The survival at 5 years in patients with the transitional cell carcinoma was 36%, and for all other histologic types was of 0%. Eleven patients underwent surgical treatment, 4 received surgical and radiation therapy, and one was treated with radiotherapy alone, with a 5-year survival of 19%, 75%, and 0% respectively. The 5-year survival for the entire group was of 32%. Conclusion: An early diagnosis and an adequate management using nephroureterectomy combined with incision of bladder cuff for renal pelvis tumors should be considered to obtain a longer survival. RESUMEN Objetivo:Identificar el cuadro clínico-patológico y determinar la influencia del estadío clínico, tipo histológico y tipo de tratamiento en la sobrevida a 5 años. Material y Métodos: Revisamos las historias clínicas de 16 pacientes con carcinoma de pelvis renal tratados en el Hospital Belén, Trujillo, Perú, entre 1966 y 1995. La edad varió de 45 a 85 años, con una media de 64.7 años. Ocho pacientes fueron varones y 8 fueron mujeres. Resultados: El tiempo medio de enfermedad fue de 30 meses. Los síntomas más comunes fueron hematuria (81.3%) y dolor en en el flanco (37.5%). La mayoría de pacientes presentó palidez (43.8%) y masa palpable (32.3%) al examen clínico. El riñón izquierdo fue el más afectado (56.3%). La citología exfoliativa (PAP) detectó células neoplásicas en el 28.8% de los casos. La etapificación final fue: estadíos I-II, 31.2% y estadíos III-IV, 68.8%, con una sobrevida quinquenial de 100% y 0%, respectivamente. El tipo histológico más frecuente fue el carcinoma de células transicionales (75%), seguido por el carcinoma epidermoide (18.7%) y adenocarcinoma (6.3%). La sobrevida a 5 años para el carcinoma de células transicionales fue de 36% y para aquellos con otro tipo histológico fue de 0%. Once pacientes recibieron tratamiento quirúrgico, 4 cirugía más radioterapia y uno sólo radioterapia, con una sobrevida a 5 años de 19%, 75% y 0%, respectivamente. En la presente serie la sobrevida a 5 años fue de 32%. Conclusión: El diagnóstico precoz y el uso de la nefroureterectomía con resección en cuña en vejiga deberían ser considerados para obtener una mejor sobrevida en estos pacientes. Objetivos: Identificar el cuadro clínico, la recurrencia y sobrevida quinquenal por tumor desmoide. Material y métodos: Analizamos 18 pacientes manejados en el Hospital Belén, Trujillo, Perú, desde 1966 a 1994. Resultados: Los antecedentes más frecuentes fueron embarazo reciente (n=8), cirugía previa (n=3) y traumatismo (n=1). La edad promedio fue de 29.5 ± 16.7 años (rango, 5 meses a 75 años). El 50% de los casos se presentó en la tercera década de la vida. Hubo 6 hombres y 12 mujeres (H:M, 1:2).El síntoma y signo más común fue tumor (100%), acompañado por dolor (66.7%), impotencia funcional (22.2%) y parestesias (16.7%). El diámetro tumoral promedio fue 10.9 ± 5.6 cm. Las neoplasias se localizaron en pared abdominal (n=12), cabeza - cuello (n=3) y miembros inferiores (n=3). El tratamiento más utilizado fue el quirúrgico (n=14), y la radioterapia se utilizó en 2 casos irresecables. En los 10 pacientes que tuvo seguimiento completo la tasa de recurrencia quinquenal fue de 33.3%, siendo ésta menor con el tratamiento quirúrgico (25%) que con el radioterápico (50%). Además, hubo menor recurrencia con la cirugía amplia y radical (16.7%) en comparación con la resección local simple (50%). La sobrevida quinquenal en la serie total fue de 87.5%, siendo ésta menor con el tratamiento quirúrgico (83.3%) que con el radioterápico (100%). Conclusiones: La localización más frecuente de estos tumores estuvo en la pared abdominal. El tratamiento quirúrgico con un margen amplio de resección ofrece un mejor control local de los tumores desmoides.
