Antecedentes: La atresia de esófago (AE) es la anomalía congénita del esófago más importante, tiene una incidencia mundial de 1:3500 nacidos vivos. A nivel mundial la mortalidad es de 10% pero en países cercanos como Costa Rica es del 16%.Objetivo: Identificar las características de los pacientes con AE atendidos en el Hospital Mario Catarino Rivas.Pacientes y métodos: Se realizó un estudio de tipo descriptivo, transversal en el Hospital Nacional Dr. Mario Catarino Rivas (HNMCR). Se obtuvo la información del expediente clínico. Se incluyeron en el estudio los 23 recién nacidos con diagnóstico de AE que se atendieron en el área de Emergencia de Pediatría y sala de Cuidados Intensivos Neonatales del período de enero del 2013 a agosto del 2014.Resultados: El 78% de los niños con AE fueron varones (n=18). El 70% (n=16) de las madres realizó adecuado control prenatal y el 35% (n=8) se realizó ultrasonido (USG). Se realizó el diagnóstico entre las 24-48 horas en el 57% de los pacientes (n=13). 74% (n=17) nacieron a término. El tipo de atresia más frecuente fue el tipo III en 15 niños (65%). Fallecieron 19 de los niños (82%).Conclusiones: En nuestro sistema de salud la tasa de supervivencia a una AE es baja, y el control prenatal no aporta relevancia para el diagnóstico de esta patología.Acta Pediátrica Hondureña, Vol. 5, No. 1 y 2 / Abril 2014 a Marzo 2015: 349-354
Antecedentes: Las malformaciones anorrectales (MAR) se componen de una amplia gama de anormalidades que se asocian con varias patologías. Afectan tanto al sexo masculino como femenino, en algunos casos incluyen al aparato urinario y genital. Objetivo: Describir las características clínicas y el enfoque terapéutico de pacientes con Malformaciones Anorrectales en dos Hospitales de San Pedro Sula, Honduras, durante los años 2012 al 2016. Pacientes y Métodos: Estudio cuantitativo, descriptivo, transversal, realizado en pacientes con malformación anorrectal del Hospital Nacional Dr. Mario Catarino Rivas y el Hospital de Niños Quemados y Cirugía Pediátrica Ruth Paz. La muestra fueron 94 pacientes, la información se obtuvo del expediente clínico y se procesó en el software estadístico IBM SPSS 22. Resultados: En cuanto al sexo, 53.2% (n=50) eran mujeres y 46.8% (n=44) hombres, 30.9% (n=29) tenían un año de edad, 51.1% (n=48) provenían del departamento de Cortés. En los hombres, se encontró fístula perineal en 30.2% (n=14) pacientes y en las mujeres se encontró fístula rectovestibular en 37.3% (n=19), se encontró asociación a otro tipo de malformaciones congénitas en 29% (n=27) de los pacientes. El tratamiento consistió en colostomía protectora seguido de Anorrectoplastía sagital posterior tipo Peña y por último el programa de dilataciones anorrectalesa 59.6% (n=56) y 85.3% no presentó complicaciones quirúrgicas. Conclusiones: Las malformaciones anorrectales en este estudio afectaron más a las mujeres que a los hombres, la edad que predominó fueron los pacientes de 1 año, procedentes en su mayoría del departamento de Cortés, específicamente de San Pedro Sula.Palabras clave Ano imperforado, Cloaca, Fístula, Colostomía
Un bezoar es la acumulación de cuerpos extraños o nutrientes sin digerir en el tracto gastrointestinal. El litobezoar, la acumulación de piedras en el tracto digestivo, es frecuentemente vista en estómago. Un bezoar colónico primario es una situación excepcionalmente rara.
Antecedentes: La Invaginación Intestinal (II) es una alteración caracterizada por introducción de una porción de intestino en sí misma y en sentido distal. Está asociado a ciertas infecciones entéricas y de vía aérea superior, como adenovirus, enterovirus y rotavirus. Objetivo: caracterizar el perfil epidemiológico y clínico de la II en niños menores de 2 años. Pacientes y métodos: Estudio descriptivo, transversal se incluyeron niños menores de 2 años con sospecha clínica de II ingresados en el servicio de pediatría. Se obtuvo una muestra de 20 pacientes. Datos fueron obtenidos de expedientes clínicos, interrogatorio dirigido a padres o tutores y seguimiento hasta su egreso. Resultados: El 60% eran menores de 6 meses, relación hombre- mujer 1.5-1,35% presentó algún grado de desnutrición, 81% recibió dos dosis de vacuna rotavirus. El primer síntoma presentado fueron 40% vómitos, 25% diarrea, 15% heces con sangre y 5% distensión abdominal; la sintomatología previa a la cirugía fue: distensión abdominal (30%) dolor o llanto inconsolable (25%), paro de evacuaciones (15%), fiebre y diarrea (10%), vómitos y heces con sangre (5%). Al 70% se realizó tacto rectal, de estos 5% se palpo masa en recto y 25% presentó salida de heces con sangre. Al 100% se realizó laparotomía, la localización principal fue ileo-colica en el 50% de los casos. Conclusión: La presentación clínica de II en lactantes es atípica, lo que hace necesario realizar un examen físico exhaustivo para detectar signos clínicos característicos de la enfermedad.
