RESUMENEl complejo Mycobacterium avium (MAC) es un patógeno que se encuentra en el medioambiente y causa infecciones tanto en pacientes inmunocompetentes como inmunocomprometidos. Se presenta el caso de un paciente VIH positivo varón de 38 años infectado por P. jirovecii y aparentemente infectado por Mycobacterium tuberculosis desde el año 2009, el cual fue tratado con antibioticoterapia para pneumocistosis y terapia antituberculosis (TB) logrando mejoría parcial. En el año 2012 se le realizó nuevamente examen de cultivo y un nuevo tratamiento anti TB, frente a la sospecha de estar en presencia de una cepa de TB multidrogorresistente se recomienda realizar la identificación micobacteriana. El examen de cultivo fue positivo y el resultado genotípico resultó positivo para MAC. Se reporta el primer caso de un paciente VIH/SIDA con infección pulmonar por MAC en el Perú, así como una breve revisión de los aspectos epidemiológicos, clínicos y de tratamiento. One case is presented: an HIV positive, 38 year old male patient, infected with P. jirovecii and apparently infected with Mycobacterium tuberculosis since 2009. He was treated with antibiotic therapy for pneumocystosis and antituberculosis (TB) therapy, which achieved a partial improvement. In 2012, the patient underwent a culture test and new anti TB treatment. Upon suspicion of a drug resistant TB strain, it was recommended to perform the mycobacterial identification. The culture test was positive and the genotypic result was positive for MAC. The first case of an HIV/AIDS patient with MAC lung infection in Peru is reported, as well as a brief review of the epidemiological, clinical and treatment aspects to the case.
OBJETIVO: Presentar las características clinicopatológicas de lesiones no neoplásicas de piel en mujeres con VIH/SIDA. DISEÑO: Estudio retrospectivo descriptivo. MATERIALES Y MÉTODOS: En 18 casos de mujeres con lesiones no neoplásicas de piel y VIH/SIDA se tomó biopsias de piel para estudio histopatológico e inmunohistoquímico, en búsqueda de otros agentes etiológicos patógenos. RESULTADOS: La mayoría de mujeres estaban en el grupo etáreo de 10 a 29 años (55,6%), y de 30 a 39 años (22,2%); y con estadio clínico B3 (33,3%) o C3 (27,8%). Las lesiones dermatológicas halladas más frecuentemente fueron dermatitis fibrosante (50%) y dermatitis granulomatosa (38,9%); otros agentes patógenos en la piel fueron Cándida (44,4%) y virus Epstein Barr (22,2%). CONCLUSIONES: Se debe alertar a las pacientes con infección por VIH para acudir a consulta médica cuando aparecen lesiones en su piel, con el fin de prevenir la superinfección de ellas por otros microorganismos patógenos, según su estado de inmunodeficiencia.
El presente estudio se fundamenta en la guerra de cuarta generación en la frontera norte ecuatoriana. El término “guerra de cuarta generación” se originó en 1989, cuando William Lind publicó, simultáneamente, en la edición de octubre de 1989 de Military Review y de Marine Corps Gazette, órganos oficiales del Ejército y del Cuerpo de Infantería de Marina de los Estado Unidos, quien se refiere a la guerra de los ejércitos de los Estados contra oponentes no estatales como las Fuerzas Armadas Revolucionarias de Colombia y hoy conocidos como los Grupos Irregulares Armados. La frontera norte del Estado ecuatoriano se ha visto afectada en la última década por los GIA, y un imporante hecho que ocurrió en el sector de Angostura, en enero del 2008, donde las Fuerzas Armadas de Colombia violaron territorio ecuatoriano mientras realizaban operaciones ocultas contra las FARC, en territorio ecuatoriano. El 26 de septiembre del 2016, el presidente de Colombia, Juan Manuel Santos, firmó formalmente los acuerdos de paz de Colombia con las FARC, pero no todos sus integrantes entregaron las armas. Los grupos que no se acogieron a la paz, continuaron realizando actos de terrorismo y secuestro, es decir, realizando Guerra de cuarta generación en territorio ecuatoriano.
