Introducción : El objetivo de este estudio fue evaluar clínica y radiográficamente después de 36 meses de función, 54 implantes colocados en el maxilar inferior y rehabilitados con sobredentaduras bajo el protocolo de carga inmediata, utilizando sistema de retención tipo bola. Métodos: 27 pacientes totalmente edéntulos quienes atendieron el llamado de control después de 36 meses de haber recibido cada uno dos implantes marca BioHorizons de conexión interna, instalados en la región inter foraminal del maxilar inferior. Los 54 implantes fueron evaluados en términos de supervivencia, cantidad de pérdida ósea peri-implantar, estado de los tejidos peri-implantares, el comportamiento de la prótesis y el grado de satisfacción de los pacientes. Resultados: La tasa de supervivencia a los 36 meses fue de 98.14%, el promedio de perdida ósea radiográfica fue de 0,74 ±0,5, el 60,1% de los implantes presentaron nivel de placa grado 0, el 33,2% evidenció placa únicamente cuando se instrumentó con la sonda alrededor del implante, el promedio de profundidad al sondaje fue de 1,75 ±0,75 mm. Se encontró que el 57% de las prótesis tenían retención activa , El 43% no presentó retención o su retención estaba dada por uno solo de sus elementos, el 53,8% de las sobredentaduras se encontraron estables en el momento de la evaluación , el 100% de los pacientes manifestaron satisfacción total. Conclusión: a 36 meses de observación una sobre dentadura mandibular retenida por pilares en bola colocada sobre dos implantes no ferulizados y cargados inmediatamente parece ser un tratamiento predecible para pacientes con desdentación total inferior con una alta tasa de supervivencia de los implantes, con unos niveles de pérdida ósea peri implantar dentro de los estándares reportados por la literatura y con un alto grado de satisfacción de los pacientes.

ABSTRACT:This research aimed at determining sleep bruxism events recorded by polysomnography and masseter and temporalis muscle activity recorded by surface electromyography before and after performing a selective grinding. This study comprised three stages: 1. Pre-test (A Clinical Evaluation, an assessment through semi-adjustable articulator, a polysomnography and an electromyography); 2. Selective grinding; 3. Post-test (polysomnography and an Electromyography). A convenience sample of 10 subjects, with premature contact points, was assessed in a dental clinic. Subjects between 21 and 23 years were assessed. All of them had premature contacts. Eight subjects presented subsequent interferences during lateral movements. Data did not meet the criteria of normality or homoscedasticity thus non-parametric Wilcoxon test was used to compare the means of variables before and after. An average of bruxism episodes increased in stages 1 and REM but there was not a significant statistical difference in the polisomnographic recording after selective grinding. In stages 2 and 3 of sleep 26 % and 54 % reductions were found respectively. In stage 3 the difference between events was statistically significant. The recording of electromyographic activity before and after selective grinding showed a decrease in amplitude, length and area of the masseter and temporalis muscles. The difference was statistically significant only for temporary muscles in the length values and the right temporal area. After selective grinding the number of bruxism episodes decreased during the sleep stages 2 and 3. The selective grinding showed a statistically significant reduction of action potentials recorded by the surface electromyography of temporal muscle.

RESUMEN:La investigación de los músculos de la masticación, ayuda a un mejor entendimiento de la función oral al permitir un análisis cuantitativo de su actividad y fuerza. Comparar la actividad y fuerza de los músculos maseteros (M) y temporales (T) derechos (D) e izquierdos (I) de pacientes rehabilitados con sobre-dentaduras inferiores (RSI), con la de pacientes dentados con oclusión funcional (PD). Estudio de corte transversal , con muestreo intencional de 28 adultos RSI y 28 adultos PD .Se analizó la actividad muscular de M y T durante la masticación y contracción voluntaria máxima (CMV) mediante electromiografía de superficie; y la fuerza, mediante mioescanografía. La comparación de promedios se realizó mediante la prueba U de Mann Whitney. Los promedios de la CVM en el grupo ISD en micro-voltios (mV) ; MI:0,53 .La diferencia fue estadísticamente significativa en la fuerza, la CVM de M y T ; y en la masticación en el T y M derechos (p<0.05). Hay diferencias en la fuerza y la actividad durante contracción voluntaria máxima y la masticación en maseteros y temporales de pacientes PRSI comparados con pacientes PD.PALABRAS CLAVE: Músculo temporal; músculo masetero, actividad motora, fuerza de mordida (MeSH) INTRODUCCIÓNLa masticación es una función compleja que incluye reflejos voluntarios y automáticos controlados por patrones generados por el sistema nervioso central y regulados por varios receptores (extero-propioreceptores y viscero-receptores) (Rilo et al., 2007). La pérdida de un diente natural puede rehabilitarse con prótesis soportadas por los dientes remanentes, por la mucosa alveolar o por medio de implantes oseo-integrados. Se ha sugerido que la evaluación funcional de una rehabilitación prostodóntica debe incluir la medida de la fuerza al morder y la valoración de los movimientos masticatorios y los registros electromiográficos de superficie de los músculos que intervienen en la masticación (Gartner et al., 2000;Ferrario et al., 2004;Feine & Lund, 2006; Van Steenberghe & Jacobs, 2006). Las personas edéntulas, aún con prótesis totales bien adaptadas, tienen reducidas varias funciones orales, como la fuerza de mordida, la eficiencia masticatoria, los umbrales táctiles y la percepción oral de la dureza y el espesor de los objetos (Manly et al., 1952;Karkazis, 2002).
