The aim of this study was to compare the clinical, biochemical, and pathologic findings of normocalcemic patients with macroscopically enlarged parathyroid tissue identified at thyroid surgery with those of patients treated surgically for preoperatively proved primary hyperparathyroidism (PHPT). The records of 28 patients with incidental parathyroid enlargement and 533 patients with PHPT were reviewed to compare age, sex, serum calcium and phosphate, intact parathyroid hormone (iPTH), parathyroid weight, number of diseased glands, cell and histologic types, PTH content, and cure rate. Incidentally found lesions were lighter and developed in younger patients. Biochemistry and pathology found them to be less hyperfunctioning. Sex, number of diseased glands per patient, and cell type were not different. PTH content was low in the incidental lesions. Incidentally discovered enlarged parathyroid glands are mildly hyperfunctioning at the time of discovery. They may represent an early stage of lesion responsible for overt PHPT. In the absence of knowledge concerning their significance and evolution, we recommend that enlarged parathyroids found during the course of a thyroid operation be removed.
Objetivo: diagnosticar o crescimento intra-uterino restrito (CIUR) por meio do índice ponderal de Rohrer e sua associação com morbidade e mortalidade neonatal precoce. Métodos: estudo retrospectivo, descritivo e de coorte transversal, no qual foram incluídos 2741 recém-nascidos (RN), sendo 2053 casos de grávidas hígidas, 228 de pré-eclâmpsia leve (PE), 52 com PE grave, 25 com PE que evoluiu para eclâmpsia, 136 de ruptura prematura das membranas (RPM) e 247 de tabagistas. O índice ponderal (IP) de Rohrer foi calculado segundo a equação: IP = peso/estatura 3 x 100 e se utilizaram os valores 2,25 e 3,10 dos percentis 10 e 90 de Lubchenco. Classificou-se como CIUR assimétrico aquele RN com IP < 2,25 e peso inferior ao percentil 10, como simétrico, IP entre 2,25 e 3,10 e peso inferior ao percentil 10, ao passo que adequado para idade gestacional (AIG) aquele com IP entre 2,25 e 3,10 e peso entre os percentis 10 e 90. O estudo estatístico foi realizado pelo teste t não pareado, teste não paramétrico do χ 2 e teste exato de Fisher, considerado significante valor de p < 0,05. Resultados: o baixo peso (<2500 g) ao nascer incidiu em 3,6% (100/2741) dos casos, ao passo que a taxa de CIUR diagnosticado pelo IP foi de 15,7% (430/2741), sendo 14,0% assimétricos e 1,7% simétricos. A taquipnéia transitória foi a complicação mais freqüente (8,3%) entre os assimétricos, seguido da asfixia (5,7%) e infecção (2,6%). A taquipnéia transitória incidiu em 6,5% dos simétricos, seguido da asfixia (4,3%), síndrome de aspiração de mecônio (2,2%), hipoglicemia (2,2%) e infecção (2,2%). O óbito neonatal precoce foi semelhante entre os RN com CIUR e AIG, ambos alcançando cifra de 0,3%. Conclusões: o índice ponderal de Rohrer diagnosticou os diferentes padrões de crescimento intra-uterino, os quais não seriam reconhecidos utilizando-se o peso em função da idade gestacional. Os RN assimétricos apresentaram maior taxa de taquipnéia transitória e de asfixia, entretanto sem significância estatística em relação às demais modalidades de crescimento intrauterino. A taxa de óbito neonatal precoce foi semelhante entre os RN assimétricos e os AIG.

Objective: To evaluate the placental alterations in smoking mothers. Method: A case‐control stereologic study of 10 placentas from smokers and 10 from non‐smokers. Result: The authors found a significant increase in stromal volume density, villus size and trophoblast thickness, and a significant decrease in villus and vessel surface densities, total surfaces, and number/mm2 histological field in the smokers' placenta. Conclusions: The alterations found in the smokers' placenta greatly contribute to impairing the exchanges between maternal and fetal blood.

Introduction:Schwannomas—Schwann cells–originating tumors—may develop in many locations. However, primary schwannomas arising within lymph nodes are extremely rare, with only a few cases described to this date in the English literature. For the intranodal location, most of the cases are described in the abdominal cavity. In these cases, clinicians may consider and check for familial disorders, such as neurofibromatosis type 2 (NF2) and schwannomatosis also called neurofibromatosis type 3. Schwannomas are benign neoplasms. Histologically, differential diagnosis for spindle-cell lesions in lymph nodes is important and must be done carefully, mainly because they may be attributable to metastatic disease. We report a case of a primary schwannoma arising in a cervical lymph node.Background:Primary schwannomas arising within lymph nodes are extremely rare, with only a few cases reported. Since they are benign neoplasms, the differential diagnosis with other intranodal spindle cell lesions, mostly malignant, is important.Methods:An asymptomatic 69-year-old woman, previously submitted to left hemithyroidectomy for a benign folicular nodule, underwent thyroidectomy totalization following the identification of a large thyroid nodule in routine evaluation.Results:Gross and microscopic examination and ancillary studies were consistent with the diagnosis of intranodal schwannoma. The patient had acquired bilateral hypoacusia. Therefore, type 2 neurofibromatosis was considered and vestibular schwannomas ruled out.Conclusion:Herein, we present the second case of a primary schwannoma in a cervical lymph node reported so far. The relevance of the differential diagnosis is highlighted.
OBJETIVO: Observar a sucção digital ou da mão em fetos, identificar os fatores relacionados à gravidez de importância para sua manifestação e identificar, após o nascimento do bebê, a permanência da sucção digital. MATERIAIS E MÉTODOS: Dois exames de ultra-sonografias fetais foram realizados em 55 gestantes, nos períodos de 20-24 e 25-32 semanas. Para avaliar os fatores relacionados à gravidez, foram utilizados questionários. Para identificar a permanência da sucção digital, visitas foram realizadas uma semana após o nascimento. RESULTADOS: A sucção digital foi detectada na maioria dos fetos, exceto em 20. Estatisticamente não houve diferenças significativas entre estado de saúde da gestante e o grau de aceitação da gravidez com a manifestação da sucção digital em fetos. O teste Q de Cochran não evidenciou diferenças significativas entre os sinais de sucção digital em fetos e a instalação do hábito em recém-nascidos. CONCLUSÃO: A ultra-sonografia é um método importante para a observação de sinais de sucção digital em fetos. Esses sinais não implicam hábito bucal após o nascimento.
