Objective The objective of this study is to evaluate leg length discrepancy in adolescent idiopathic scoliosis. Methods A retrospective study of 80 subjects with adolescent idiopathic scoliosis (AIS) was conducted. The inclusion criteria were patients aged 10 to 18 years old with posteroanterior (PA) and lateral full-length radiographs. The exclusion criteria were patients subjected to surgery or orthotic treatment, those with other spinal disease, and those with poor quality x-rays. The parameters evaluated were: age, sex, Risser stage (RS), triradiate cartilage (TC), scoliotic curvatures, differentiated according to Lenke classification, sagittal (SB) and coronal balance (CB), and leg length discrepancy, which was assessed through the difference between the femoral heads (LLD) and through the assessment of pelvic obliquity (PO). Results The majority of patients with AIS demonstrated a mild LLD (<1 cm). The mean LLD was significantly different (p<0.01) between the scoliotic population with a main thoracolumbar curvature and those with a main lumbar curvature. When there was an LLD, it was the left limb that was shortened in most cases. The side of the longer lower limb had a direct influence on the CB (p=0.052). Conclusions This study demonstrates that in an AIS population with small LLD values, the extent of the shortening has a stronger impact on coronal balance and location than on the dimension of the main scoliotic curvature. These results demonstrate the importance of a more in-depth study on the effects of LLD <1 cm in the development of AIS and coronal imbalance. Level of evidence IV; Case Series.
ResumoA fratura isolada de Tillaux representa uma rara e frequentemente mal diagnosticada fratura de tíbia distal anterolateral em adultos. Normalmente ocorre em adolescentes que se aproximam da maturidade esquelética por avulsão do ligamento tibiofibular anteroinferior. Este estudo de revisão de literatura baseada em caso tem como objetivo lembrar a existência dessa identidade em adultos, resumir seu mecanismo de lesão, diagnóstico e procedimentos de tratamento. De acordo com a literatura, este é apenas o oitavo caso descrito: uma mulher de 46 anos sofreu uma fratura isolada de Tillaux com 4 mm de deslocamento e realizaram-se redução e fixação aberta com parafuso canulado duplo. Após a reabilitação adequada, alcançou-se um excelente desfecho funcional e radiológico. É importante reconhecer e tratar adequadamente essas lesões distintas para evitar maiores instabilidades, alterações degenerativas e limitação da função articular do tornozelo. Um diagnóstico precoce e a osteossíntese apropriada desempenham um papel significativo em um prognóstico de recuperação bem-sucedido.
Complete closed talus dislocation is a rare and serious injury, with a bad prognosis due to the risk of avascular necrosis. This article describes the clinical case of a young soccer player, who during sports competition suffered an injury in his right ankle, after bad positioning of his foot on the ground. On examination, an irreducible deformity in supination of the foot was evident, and the X-ray and the CT-scan showed a complete anterolateral talus dislocation associated with a peroneal malleolus fracture. An emergent open reduction was performed with osteosynthesis of the peroneal malleolus fracture. The teenager had a satisfactory functional recovery, with no evidence of progression to avascular necrosis, having resumed his sports practice after 6 months of follow-up.
