A história do controle da infecção no homem se confunde com a história da humanidade. Na atualidade, o controle da dor, em qualquer área da medicina, inclusive a odontologia teve um grande avanço e constitui um aspecto praticamente esclarecido pela ciência. Entretanto, a infecção ainda constitui um problema a ser dominado por todas as áreas médicas e uma real preocupação para todos os profissionais envolvidos no seu controle. As infecções agudas da região orofacial são causadas pelas atividades patogênicas de microorganismos como vírus, fungos e bactérias. A infecção cirúrgica é causada principalmente por infecção bacteriana, a progressão de qualquer infecção é determinada pela resposta do hospedeiro aos organismos invasores. As infecções odontogênicas constituem um dos problemas mais dificieis de tratar em odontologia. Elas podem variar desde simples infecções bem localizadas de baixa intensidade que exigem apenas tratamento mínimo, a infecções graves nos espaços fasciais que causam risco de vida. Conquanto a maioria das infecções odontogênicas seja tratada facilmente, com técnicas cirúrgicas simples e terapia médica de suporte, que inclui a administração de antibióticos, o clínico deve sempre ter em mente que ocasionalmente, tais infecções podem tornar-se graves em breve espaço de tempo. O presente trabalho tem como objetivo ressaltar a importância do conhecimento sobre infecção bucal e maxilo-facial para um correto diagnóstico e tratamento. Referências: 1.CORTESI, W. A reabilitação oral de pacientes com maxilas atróficas tem sido bastante discutida na literatura. Esse trabalho apresenta a técnica de reconstrução de maxila atrófica através de enxertia óssea utilizando osso removido da crista ilíaca. De acordo com LIM et al em 2004, uma quantidade mínima de tecido ósseo é necessária para a inserção e manutenção da estabilidade do implante, após a perda dos dentes o processo alveolar maxilar pode sofrer reabsorção tanto vertical quanto horizontal, podendo atingir um estágio de pneumatização do seio maxilar que em alguns casos contra-indica a instalação de implantes. A literatura sugere que quando há a pneumatização acentuada do seio maxilar e reabsorção de rebordo alveolar, a técnica de sinus Lift, introduzida primeiramente por TANTUM em 1970, e revisada por MISCH na década de 80, é indicada. Dentre as áreas doadoras extra-orais a crista óssea do ilíaco tem sido uma das opções de escolha, segundo HARBON et al, por apresentar baixo índice de complicações pós-operatórias, baixa morbidade e menor reabsorção pós-cirúrgica Esta técnica permite, após seis meses da cirurgia, a instalação de implantes osseointegrados e posterior reabilitação protética. O presente trabalho apresenta um caso clínico de enxertia ósseo de crista ilíaca em maxila atrófica e reabilitação com implantes.
O cisto odontogênico calcificante é uma lesão incomum com comportamento clínico e histopatológico variável. Em alguns casos parecem ser cistos não-neoplásicos, enquanto em outros casos não apresentam características císticas, sendo considerados como neoplasias. A forma cística é a mais freqüente e a característica histológica mais comum é a presença de um número variável de células fantasmas no componente epitelial. A região de caninos e incisivos é a de maior incidência da lesão e a média de idade dos pacientes é de 33 anos. Geralmente se apresenta como uma lesão unilocular radiolúcida bem definida, podendo apresentar estruturas radiopacas no interior. O objetivo deste trabalho é apresentar o caso clínico da paciente L.G., 39 anos, gênero feminino, leucoderma, que foi encaminhada ao serviço de Cirurgia Buco-Maxilo-Facial da UFPR. A lesão apresentava-se assintomática e foi um achado durante o exame radiográfico, no qual observou-se a presença de uma lesão radiolúcida, estendendo-se da região do dente 12 ao 23. Os dentes 12, 11 e 21 apresentavam tratamento endodôntico e o dente 22 não respondeu ao teste de vitalidade. A hipótese diagnóstica foi de cisto periapical e o tratamento proposto foi a enucleação da lesão. O diagnóstico histopatológico foi de cisto odontogênico epitelial calcificante.
Os traumas na região do zigoma não são necessariamente de grande intensidade, porém muitas vezes se apresentam sob a forma de disjunções, isto é, separação do zigoma dos ossos a ele unidos pelas suturas, que são os pontos frágeis não só do zigoma como também dos outros ossos da face. As fraturas orbitais são as que mais tem chances de se tornar complexas, devido a anatomia da região. O processo orbital do zigoma forma parte do assoalho da órbita, separando a cavidade orbital do seio maxilar. (BARROS,2000) Paciente J.T., do sexo masculino, 59 anos, leucoderma, foi encaminhado ao Serviço de Cirurgia e Traumatologia Buco-Maxilo-Faciais do Hospital XV, relatando queda de nível, apresentando edema e equimose periorbital, hemorragia subconjuntival esquerda e escoriações faciais . Este informou episódios de vômitos e desmaios, minutos após o ocorrido. Ao exame extra-oral verificou-se afundamento da região malar esquerda , ausência de qualquer alteração visual e boa abertura bucal. Aos exames de imagens, observou-se fratura do osso zigomático, rebordo infra-orbitário e em região fronto-zigomática do lado esquerdo . Para redução das fraturas, foram realizados acessos extra-oral e intra-bucal, sendo utilizado 03 miniplacas do sistema 2.0 mm e 12 parafusos de titânio para a fixação interna rígida. Após seis meses, o paciente encontra-se em proservação, tendo sido alcancado o restabelecimento funcional.
