This paper deals with a carotid-body-like tumor found in the jugular foramen, similar to the tumors recently described by Lattes 12 as nouchromaffin paragangliomas. Kohn, 1~ in 1900, classified the carotid body as a paraganglion, in the belief that it was of sympathetic origin and part of the chromaffin system. Small masses of similar paraganglionic tissue have since been described in the following locations: 1) At the level of the middle ear. As early as 1840, Valentin 26 described a gangliolum tympanicum, associated with the tympanic branch of the 9th cranial nerve. Later, in 1879, Krause H showed that this structure resembles the carotid body and this has been confirmed recently by the studies of Guild s and Lattes and WaltnerY In addition to the paraganglion tympanicum, Guild s described what he called the glomus jugularis "in the adventitia of the dome of the jugular bulb, immediately below the bony floor of the middle ear." ~) At the aortic-pulmonary region. Paraganglia related to the major vessels at the base of the heart were mentioned in 190~ by Biedl and Wiesel. 2 In the middle thirties, Palme, 2~ Seto, 24 Muratori ~9 and Nonidez 2~ described these structures in detail. From their contributions, well summarized by Boyd, 4 we learn that these "aortic bodies" are similar to the carotid bodies and that they can be found in four different locations: a) above the ductus arteriosus; b) on the trunk of the pulmonary artery, near the origin of the left coronary artery; c) near the root of the innominate artery; d) on the left part of the aortic arch. 3) Within the ganglion nodosum. Muratori is described in 193~ a paraganglionic formation inside the vagus nerve, at the level of the ganglion nodosum, and called it paragangli intravagali. He found it in different species of the bird, "Ucelli." His findings were confirmed by the works of White 2s and Lattes, ~2 who described the paraganglion intravagale in man.
As chamadas espículas positivas a 14 e 6 ciclos por segundo, descritas e correlacionadas por Gibbs e Gibbs em 1951, não têm sido referidas em vários trabalhos electrencefalográficos. Essa omissão talvez decorra do fato de que os métodos de exame usados por muitos electrencefalografistas não oferecem condições favoráveis para o reconhecimento desses complexos de espículas positivas a 14 e 6 ciclos por segundo. O autor descreve os métodos que vem empregando, tecendo considerações sôbre os vários fatôres que lhe permitiram reconhecer nítidamente êsses complexos em 12% dos traçados feitos em seu Serviço. Salienta que essa anormalidade se apresenta com mais evidência quando se usam orelhas opostas como referência, na chamada técnica monopolar, em uma ou outra fase de sonolência ou sono. Mostra a baixíssima incidência nos traçados obtidos em vigília e as razões de seu difícil reconhecimento, mesmo no traçado de sono, quando a técnica chamada bipolar é a única utilizada. O trabalho é documentado com 14 ilustrações.
Cinco casos de coréia de Sydenham foram submetidos à terapêutica hormonal com Cortisona, Prednisona e Prednisolona; o fenobarbital e a clorpromazina foram utilizados como medicação sintomática e a Penicilina-Benzatina como profilática. Depois de analisadas as referências bibliográficas, comparados os resultados de ν;rios autores, e ajuizados os resultados obtidos em nossos casos, acreditamos poder concluir que: 1) a terapêutica hormonal da coréia de Sydenham, com o uso de ACTH, Cortisona, Prednisona ou Pred-nisclona apresenta resultados satisfatórios; 2) a Prednisona e a Prednisolona são os hormônios mais úteis no tratamento da coréia de Sydenham, sendo possível que sua associação aos salicilatos permita obter resultados ainda melhores; 3) o tratamento deve prolongar-se até o completo desaparecimento dos sintomas, as doses variando de acôrdo com a gravidade e a evolução da moléstia; 4) a terapêutica hormonal deve ser usada sem prejuízo de medicação sintomática (sedativa) e profilática (antibiótica).
