ResumenIntroducción: El riesgo suicida es cada vez más común entre los niños y adolescentes. No hay estudios actuales y locales de esta realidad en el norte peruano. El objetivo fue determinar si el riesgo suicida estuvo asociado al bullying y depresión en escolares de colegios secundarios. Pacientes y Método: Se realizó un estudio transversal analítico a través de encuestas validadas en que se midió el riesgo suicida (cuestionario de salud mental del Instituto Nacional de Salud Mental del Perú y Escala de Plutchik), el bullying (European Bullying Intervention Project Questionnaire, EBIPQ), el ciberbullying (European Cyberbullying Intervention Project Questionnaire, ECIPQ), y la depresión (Escala de Depresión de Beck) en una ciudad en el norte peruano. Se obtuvo estadísticos de asociación mediante los modelos lineales generalizados. Resultados: De 289 escolares, el 20% tenía riesgo suicida y algún tipo de depresión. De los que estaban en el tercil superior de la escala de bullying, el 38% tuvieron ideación suicida, en cambio, de los que estuvieron deprimidos el 63% tuvieron ideación suicida. Incrementaron la frecuencia de riesgo suicida el tener un puntaje del tercio superior para bullying (RPa: 1,83; IC 95%: 1,13-2,99; valor p = 0,015), el estar deprimido (RPa: 3,32; IC 95%: 1,69-6,51; valor p < 0,001), el antecedente familiar de suicidio (RPa: 1,99; IC 95%: 1,55-2,56; valor p < 0,001), el deseo pasivo de morir (RPa: 2,20; IC 95%: 1,86-2,61; valor p < 0,001), el haber planificado en algún momento el suicidio (RPa: 2,05; IC 95%: 1,60-2,64; valor p < 0,001) y el estudiar en un colegio privado (RPa: 1,16; IC 95%: 1,14-1,18; valor p < 0,001); ajustado por el sexo y por la sede educativa. Conclusión: La salud mental de los escolares estuvo comprometida en múltiples aspectos, estando varios de ellos asociados a una mayor frecuencia de riesgo suicida. 209ARTÍCULO ORIGINAL Suicidio y Bullying -R. Sandoval-Ato et al IntroducciónEl suicidio a tempranas edades se ha convertido en un importante problema de salud pública, que repercute principalmente en países en desarrollo, así como, en la disgregación y disfuncionalidad familiar 1,2 . To support the global restart of elective surgery, data from an international prospective cohort study of 8492 patients (69 countries) was analysed using artificial intelligence (machine learning techniques) to develop a predictive score for mortality in surgical patients with SARS-CoV-2. We found that patient rather than operation factors were the best predictors and used these to create the COVIDsurg Mortality Score (https://covidsurgrisk.app). Our data demonstrates that it is safe to restart a wide range of surgical services for selected patients.
Background COVID-19 can be asymptomatic in a substantial proportion of patients. The assessment and management of these patients constitute a key element to stop dissemination. Aim To describe the assessment and treatment of asymptomatic infection in patients with a confirmed diagnosis of COVID-19. Methods We searched five databases and search engines for preprints/preproofs, up to August 22, 2020. We included cohort, cross-sectional, and case series studies, reporting the assessment and management of asymptomatic individuals. We extracted data on total discharges with negative PCR, length of hospitalization, treatment, and number of patients who remained asymptomatic. A random-effects model with inverse variance method was used to calculate the pooled prevalence. Results 41 studies (nine cross-sectional studies, five retrospective studies and 27 reports/case series; 647 asymptomatic individuals), were included, of which 47% were male (233/501). The age of patients was between 1month and 73 years. In patients who became symptomatic, length of hospitalization mean was 13.6 days (SD 6.4). Studies used lopinavir/ritonavir, hydroxychloroquine plus ritonavir/lopinavir, hydroxychloroquine with and without azithromycin, ribavirin plus interferon and interferon alfa. The proportion of individuals who remained asymptomatic was 91% (463/588 patients; 95%CI: 78.3%–98.7%); and asymptomatic individuals discharged with negative PCR was 86% (102/124 individuals; 95%CI: 58.4%–100%). Conclusions There is no standard treatment for asymptomatic COVID-19 individuals. There are no studies of adequate design to make this decision. It has been shown that most asymptomatic individuals who were followed have recovered, but this cannot be attributed to standard treatment.
Objective The objective of this study was to investigate whether glycated hemoglobin (HbA1c) is a valid surrogate for evaluating the effectiveness of antihyperglycemic drugs in diabetes mellitus (DM) trials. Methods We conducted a systematic review of placebo-controlled randomized clinical trials (RCTs) evaluating the effect of a treatment on HbA1c (mean difference between groups) and clinical outcomes (relative risk of mortality, myocardial infarction, stroke, heart failure, and/or kidney injury) in patients with DM. Then, we investigated the association between treatment effects on HbA1c and clinical outcomes using regression analysis at the trial level. Lastly, we interpreted the correlation coefficients (R) using the cut-off points suggested by the Institute for Quality and Efficiency in Healthcare (IQWiG). HbA1c was considered a valid surrogate if it demonstrated a strong association: lower limit of the 95 percent confidence interval (95 percent CI) of R greater than or equal to .85. Results Nineteen RCTs were identified. All studies included adults with type 2 DM. None of the associations evaluated was strong enough to validate HbA1c as a surrogate for any clinical outcome: mortality (R = .34; 95 percent CI −.14 to .69), myocardial infarction (R = .20; −.30 to .61), heart failure (R = .08; −.40 to .53), kidney injury (R = −.04; −.52 to .47), and stroke (R = .81; .54 to .93). Conclusions The evidence from multiple placebo-controlled RCTs does not support the use of HbA1c as a surrogate to measure the effectiveness of antihyperglycemic drugs in DM studies.
Introducción: El riesgo de prematuridad abarca una amplia variedad de complicaciones al nacer, como enfermedades infecciosas, hemorragia intraventricular, deficiencias neurosensoriales, insuficiencia respiratoria y afectación de otros sistemas, así como problemas de crecimiento y desarrollo. Por tanto, la consecución del Objetivo de Desarrollo del Milenio de reducir la mortalidad infantil, depende en gran medida de la reducción de la mortalidad relacionada con el nacimiento prematuro, lo que lo convierte en uno de los desafíos más importantes para la salud pública moderna. Objetivo: Identificar los factores maternos asociados a prematuridad. Material y Métodos: Se realizó un estudio de tipo caso-control retrospectivo en 2000 recién nacidos vivos de un hospital público de Trujillo, entre el 2015-2019. Se dividieron los grupos según el criterio de edad gestacional. Se analizaron las variables clínicas maternas pareadas según prematuridad o a término, y se realizó un análisis bivariado y multivariado por regresión logística ajustada por confusores. Resultados: Se demostró que el número de controles prenatales menor a cuatro durante la gestación, está asociado a prematuridad (OR 2,65; IC95%: 2,21-3,18). Por otro lado, la ausencia de una infección del tracto urinario (OR 0,73; IC95%: 0,56-0,95), está asociado a menor riesgo de prematuridad. Conclusiones: El número de controles prenatales es un factor asociado importante para parto prematuro, mientras que la ausencia de una ITU puede estar asociada a menor tasa de partos prematuros en gestantes de riesgo.
