Obesity is a prevalent health problem all over the world. It is associated with several diseases including infections. It impairs the immune system function by plenty of mechanisms. For instance, leptin and adiponectin are cytokines produced by the adipose tissue, both participating in immunity, but their effects are impaired in obese patients. Moreover, immune cells also show defects in their functions. They produce a pro-inflammatory state and contribute to obesity-related diseases. Innate immune system and adaptive immunity are both impaired in obese patients which causes a poor response to infections. In addition, in surgical site infections (SSI), there are local factors that must be considered. The large adipose panicle and visceral adipose tissue increase the surgical technique difficulty and extend the operative time. Besides, the adipose tissue has poor oxygenation and reduces operative field. It has been proven that obesity is associated to surgical site infection irrespective of type of surgery. However, minimal invasive surgery has demonstrated that reducing surgical trauma can diminish the risk for surgical site infection.
ResumenEl linfoma primario del sistema nervioso central (LPSNC) constituye el 2% de los linfomas extranodales y entre 0,3% y 1,5% de todas las neoplasias intracraneales en pacientes inmunocompetentes, siendo más frecuente a partir de los 60 años. Reportamos el caso de un paciente varón de 76 años, sin antecedentes médicos de importancia, que inició su enfermedad con inestabilidad en la marcha, dificultad para mover el hemicuerpo izquierdo, a predominio braquial, cefalea holocraneal y mareos. Ingresó a emergencia despierto, parcialmente desorientado, Glasgow 14 y ptosis palpebral derecha; además, hemiparesia e hipoestesia izquierda, a predominio crural. Por tomografía computarizada cerebral se evidenció imagen captadora de contraste en región frontoparietal derecha, intra y extra craneal, edema cerebral y desplazamiento de la línea media. Se realizó craneotomía y biopsia del tumor, diagnosticándose linfoma difuso de células grandes B del sistema nervioso central. Fue VIH negativo. Se descartó otro tumor primario y metástasis. Recibió esquema CHOD/BVAM y radioterapia. Evolucionó favorablemente. Según resonancia magnética cerebral postratamiento, desapareció el tumor. Abstract Primary central nervous system lymphoma (PCNSL) constitutes 2% of extranodal lymphomas and 0,3%-1,5% of all intracranial neoplasms in immunocompetent patients, being more frequent after the sixth decade of life. We report a case of a 76 year-old man with no antecedents who started his disease with march instability, difficulty to move left side of his body with brachial predominance, holocraneal headache and dizziness. He arrived at emergency with Glasgow 14 and right eyelid ptosis. He had left sided hemiparesis and hypoesthesia with crural predominance and occasional disorientation. Brain computed tomography (CT) showed a contrastenhanced image in the right frontoparietal region with both extra-and intra-cranial components, cerebral edema and middle line deviation. Craniotomy and biopsy of the tumor resulted in diffuse large B cell lymphoma of the central nervous system. The patient was HIV negative. Other primary tumor and metastasis were discarded. He was treated with 'CHOD/BVAM' regimen and radiotherapy. He evolved favorably. No tumor was found in the post treatment brain MRI. Las lesiones por accidentes de tránsito son la octava causa de muerte en el mundo y primera causa de muerte en personas de 15 a 29 años. Por este motivo, la Asamblea General de las Naciones Unidas ha declarado como la Década de Acción por la Seguridad Vial al periodo [2011][2012][2013][2014][2015][2016][2017][2018][2019][2020] (1) . REFERENCIAS BIBLIOGRÁFICASSe conocen cinco factores de riesgo importantes para accidentes de tránsito: velocidad, cascos, cinturones de seguridad, sistemas de retención infantil y alcohol al conducir, siendo este último uno de los más importantes. Ello ha ocasionado que la mayoría de países tengan niveles de alcoholemia máximos permitidos (1) . No obstante, son pocos los estudios que evalúan la efectividad de esta medida. Por ello, realizamos un estudio ecológico para determinar cómo varía la tasa de muertes por accidentes de tránsito por cada 100 000 habitantes de acuerdo al nivel de alcoholemia permitido. La muestra fue de 105 países, incluyéndose a aquellos que figuran en el Reporte del Estado Global en Seguridad de Caminos 2013 de la Organización Mundial de la Salud (Global Status Report on Road Safety) y que tengan niveles de alcoholemia permitidos iguales para conductores noveles y profesionales. Se excluyeron países sin data disponible en dicho reporte.Evaluamos el nivel de alcoholemia permitido (variable independiente) y la tasa de muertes por accidentes de tránsito por cada 100 000 habitantes (variable dependiente), definiéndose muerte por accidente de tránsito a aquella ocurrida dentro de los 30 días posteriores al suceso. Cartas al editorRev Peru Med Exp Salud Publica. 2015; 32(2):395-406. 399Se encontró una media de niveles de alcoholemia permitidos de 0,053 ± 0,027 g/dL (rango 0,0-0,1 g/dL). La tasa de muertes por accidentes de tránsito tuvo una media de 15,812 ± 8,948 muertes por 100 000 habitantes (rango 0-68,3 muertes por 100 000 habitantes). La regresión lineal simple reveló una asociación significativa entre ambas variables (p=0,009).Observamos que a menor nivel de alcoholemia permitido menor es la tasa de muertes por accidentes de tránsito. Sabemos que un conductor con una alcoholemia entre 0,02-0,05 g/dL tiene 2,6 a 4,6 veces más riesgo de sufrir un accidente fatal que un conductor sobrio (2) . No existe nivel de alcoholemia seguro para conducir (3) , por ello, la reducción del nivel de alcoholemia permitido junto con la aplicación de sanciones drásticas ha tenido efecto (4) .
