En la patología abdominal vascular, los aneurismas abdominales localizados en la arteria esplénica son de infrecuente aparición. Siendo mayoritariamente asintomáticos, cuando presentan síntomas pueden ser inespecíficos o presentarse como abdomen agudo hemorrágico por rotura del aneurisma a retroperitoneo u órganos vecinos. Se presentará a continuación un caso de un paciente con aneurisma de arteria esplénica y se revisara la literatura con el objetivo de mostrar las alternativas diagnósticas y terapéuticas.
El 15% de los individuos con DM2 presenta una úlcera en el pie y una fracción importante de ellos sufrirá en algún momento una amputación. Se realizó un estudio analítico, de corte trasversal retrospectivo de muestreo no probabilístico de casos consecutivos en pacientes con diagnóstico de pie diabético que acudieron al servicio de Urgencias del Hospital de Clínicas de San Lorenzo, en el periodo Enero -Diciembre del año 2019. Sobre un número total de 138 pacientes, se observa que el 51% recibió un manejo quirúrgico, con amputaciones de varios tipos. Se comparó el manejo instaurado en los pacientes, sea este quirúrgico o conservador según la presencia o no de ciertas comorbilidades, como edad mayor a 65 años, lugar de procedencia, glicemia capilar promedio del paciente e hipertensión arterial al ingreso; de estos existió una relación estadísticamente significativa (p<0,05) entre la necesidad de amputación y la glicemia capilar y la hipertensión arterial.El éxito de la intervención requiere un completo entendimiento de la patogénesis de las úlceras del pie diabético y una implementación rápida y estandarizada de un tratamiento efectivo. El manejo multidisciplinario puede lograr el salvataje de la extremidad, que conducirá a una mejor calidad de vida y sobrevida
OBJETIVO: Determinar las características de la atención del Consultorio de Ginecología Pediátrica. DISEÑO: Estudio observacional, retrospectivo. MATERIAL Y METODOS: Revisión de casos sobre la base de las hojas de atención HIS del Consultorio de Ginecología Pediátrica del Hospital Nacional Cayetano Heredia (HNCH) de los años 1998 y 1999. RESULTADOS: Se encontró 355 pacientes: 15 lactantes, 127 preescolares y 213 escolares. Tres médicos realizaron el 95% de las atenciones y 80% de los pacientes procedía del área de influencia del HNCH La vulvovaginitis se diagnosticó en 75.5% de los pacientes y el sangrado genital en 18,3%. CONCLUSIÓN: La vulvovaginitis y el sangrado genital son las principales causa de consulta ginecológica a cualquier edad pediátrica.
Objetivos: Determinar el riesgo de preeclampsia en gestantes nulíparas con presencia de la muesca protodiastólica (MPD) o índice de resistencia (IR) > 0,58 en la evaluación Doppler de las arterias uterinas. Diseño: Estudio de cohortes. Institución: Departamento de Ginecoobstetricia, Hospital Nacional Cayetano Heredia. Participantes: Gestantes nulíparas. Intervenciones: Entre marzo 2002 y julio del 2003, 126 gestantes nulíparas fueron estudiadas mediante velocimetría Doppler color pulsado de las arterias uterinas, entre las 24 y 26 semanas de gestación. Se consideró como exposición la presencia de la MPD o IR >0,58. Fueron covariables la edad materna, estado civil, índice de masa corporal y la ubicación placentaria. Se consideró el desarrollo de preeclampsia como desenlace final. Principales medidas de resultados: Presencia de muesca protodiastólica o índice de resistencia >0,58 por velocimetría Doppler color y su relación con preeclampsia. Resultados: Entre las gestantes nulíparas con presencia de la MPD, el riesgo de preeclampsia fue 12 veces mayor (RR = 12; IC95%: 1,65 a 102,66) con respecto a gestantes nulíparas sin presencia de la MPD. Entre las gestantes con IR >0,58, el riesgo de preeclampsia fue 7 veces mayor con respecto a las gestantes nulíparas con IR ≤ 0,58 (RR = 7; IC95%: 0,88 a 55,25). Conclusiones: El riesgo de preeclampsia aumentó significativamente entre las gestantes nulíparas con presencia de la muesca protodiastólica entre las 24 y 26 semanas de gestación.
