On March 25, 1990 a large earthquake (Mw = 7.0, ML = 6.8) occurred at the entrance of the Nicoya Gulf, Costa Rica, at 1322:55.6 UTC, producing considerable damage in central Costa Rica and generating much interest about whether or not the Nicoya seismic gap (Nishenko, 1989) had broken. The local country‐wide seismographic network recorded 6 years of activity prior to this large earthquake, 16 hours of foreshocks, the mainshock, and its aftershocks. This network is operated jointly by the Costa Rica Volcanological and Seismological Observatory at the National University (OVSICORI‐UNA), and the Charles F. Richter Seismological Laboratory at the University of California, Santa Cruz (CFRSL‐UCSC). We obtained high resolution locations from this network and located the mainshock at 9°38.5′N, 84°55.6′W (depth is 20.0 km) and the largest foreshock (Mw = 6.0, March 25, 1990, at 1316:05.8 UTC) at 9°36.4′N, 84°57.1′W (depth is 22.4 km). We find that the aftershock zone abuts the southeast boundary of the Nicoya seismic gap, suggesting that the seismic gap did not rupture. Since the installation of the local network in April 1984 to March 24, 1990, nearly 1900 earthquakes with magnitudes from 1.7 to 4.8 (318 with magnitude 3.0 or larger) have been located at the entrance of the Nicoya Gulf, one of the most active regions in Costa Rica. The March 25 earthquake occurred at the northwest edge of this region, where a sequence of foreshocks began 16 hours prior to the mainshock. The spatial‐temporal distribution of aftershocks and directivity analysis of the mainshock rupture process using teleseismic records both indicate a southeast propagating rupture. The mainshock ruptured an asperity of approximately 600 km2 of area, with this area expanding to 4000 km2 after 7 days. We present evidence that suggests that the ruptured asperity is produced by the subduction of a seamount. Inversion of teleseismic broadband and long‐period P and SH waves yields a thrust faulting mechanism with the shallow plane striking 292°, dipping 26°, and with a rake of 88°, in agreement with the subduction of the Cocos plate under the Caribbean plate. Local first motions for the largest foreshock and the mainshock agree with this solution. We also present evidence suggesting that the March 25, 1990, earthquake triggered and reactivated several seismic swarms in central Costa Rica and temporally decreased the activity in the epicentral area of the July 3, 1983 (Ms = 6.2), Pérez Zeledón earthquake.

Background: Septic pulmonary embolism is a serious but uncommon syndrome posing diagnostic challenges because of its broad range of clinical presentation and etiologies.Objective: To understand the clinical and radiographic associations of septic pulmonary embolism in patients presenting to an acute care safety net hospital. Methods: We conducted a retrospective analysis of imaging and electronic health records of all patients diagnosed with septic pulmonary embolism in our hospital between January 2000 and January 2013. Results: 41 episodes of septic pulmonary embolism were identified in 40 patients aged 17 to 71 years (median 46); 29 (72%) were men. Presenting symptoms included: febrile illness (85%); pulmonary complaints (66%) including pleuritic chest pain (22%), cough (19%) and dyspnea (15%); and those related to the peripheral foci of infection (24%) and shock (19%). Sources of infection included: skin and soft tissue (44%); infective endocarditis (27%); and infected peripheral deep venous thrombosis (17%). 35/41 (85%) were bacteremic with staphylococcus aureus. All patients had peripheral nodular lesions on chest CT scan. Treatment included intravenous antibiotics in all patients. Twenty six (63%) patients required pleural drainage and/or drainage of peripheral abscesses. Seven (17%) patients received systemic anticoagulants. Eight (20%) patients died due to various complications. Conclusion: The epidemiology of septic pulmonary embolism has broadened over the past decade with an increase in identified extrapulmonary, non-cardiac sources. In the context of an extrapulmonary infection, clinical features of persistent fever, bacteremia and pulmonary complaints should raise suspicion for this syndrome, and typical findings on the chest CT scans confirm the diagnosis. Antibiotics, local drainage procedures and increasingly, anticoagulation are keys to successful outcomes.

Identification of unambiguous signals of volcanic unrest is crucial in hazard assessment. Processes leading to phreatic and phreatomagmatic eruptions remain poorly understood, inhibiting effective eruption forecasting. Our 5‐year gas record from Poás volcano, combined with geophysical data, reveals systematic behavior associated with hydrothermal‐magmatic eruptions. Three eruptive episodes are covered, each with distinct geochemical and geophysical characteristics. Periods with larger eruptions tend to be associated with stronger excursions in monitoring data, particularly in SO2/CO2 and SO2 flux. The explosive 2017 phreatomagmatic eruption was the largest eruption at Poás since 1953 and was preceded by dramatic changes in gas and geophysical parameters. The use of drones played a crucial role in gas monitoring during this eruptive period. Hydrothermal sealing and volatile accumulation, followed by top‐down reactivation of a shallow previously emplaced magma body upon seal failure, are proposed as important processes leading to and contributing to the explosivity of the 2017 eruption.

In the presence of an elevated cTn I, the ECG is not an independent predictor of long-term mortality after vascular surgery. These results support a strategy of routine surveillance of cTns after vascular surgery for the detection of cardiac events and postoperative risk stratification.

Background: There have been an increasing number of pediatric reports of septic pulmonary embolism in the setting of septic thrombophlebitis adjacent to a primary infectious source.Methods: Retrospective review at an urban hospital. A total of five adults with a documented primary infectious source, adjacent septic thrombophlebitis and septic pulmonary embolism were identified between 2000 and 2011.Results: The predominant symptoms on presentation were fever and pleuritic chest pain, followed by chills and cough. S. aureus was the pathogen in 4 patients. Only one case had echocardiographic evidence of endocarditis. All patients received IV antibiotics and anticoagulation therapy. No new embolic events or central nervous system complications were noted.Conclusions: The triad of extrapulmonary infection, contiguous septic thrombophlebitis and septic pulmonary embolism is present in adult as well as pediatric populations. The use of systemic anticoagulation with appropriate antibiotics resulted in clinical and radiologic improvement but no significant complications.

[1] New microgravity data from the active crater of Poás volcano, Costa Rica, collected in 2002-2004 extends the existing dataset to provide a unique 20-year time series. These data show that gravity has decreased monotonically in the north and east of the crater over the last 5 years, whilst it has increased to the west and remained approximately constant in the south. These changes are interpreted in terms of convective recharge within dendritic intrusions beneath the crater, with overall down-welling in the north and up-welling in the west. The data reveal a 5 -10 year periodicity in sub-crater mass movement, but overall, the upper part of the conduit system appears to have maintained a state of mass equilibrium. Citation: Rymer, H., C.
