Management of patients with carcinoma of the larynx should systematically include an appropriate treatment of lymph nodes according to the TNM stage. One of the most controversial points of the treatment in these patients is the management of the clinically negative neck (N(0)). A retrospective study of 295 patients with laryngeal carcinoma and N(0) neck undergoing treatment in our centre between 1983 and 1993 is presented. We observed a significant decrease in the survival of clinically N(0) patients with histologically affected lymph nodes. Lymphadenopathy was more frequently detected in patients with supraglottic tumours (38 per cent) when compared to glottic tumours (16 per cent). In our experience, routine bilateral and unilateral dissection of N(0) necks in all supraglottic tumours and in T3-T4 glottic tumours, respectively, is the most beneficial approach for patients in terms of survival.
La mayor complicación ambiental que presenta la disposición final de los residuos sólidos urbanos es como se desarrolle el método que se use; como es el caso de los rellenos sanitarios, cuya problemática radica en la excesiva producción de lixiviado los cuales son producidos por la degradación de los desechos y factores ambientales. En los últimos años los PAOs, se han convertido en una opción viable como tratamiento y se destacan debido a sus ventajas en la eficiencia de oxidación, velocidades de reacción rápidas y oxidación completa. No obstante tratamientos como peroxono necesita grandes cantidades de químicos para producir el aumento necesario de OH, lo cual hace que este proceso no sea económicamente tan viable; a diferencia del proceso Fenton, el cual en condiciones ambientales es una técnica muy eficiente. Con relación a los procesos Fenton y Photo-Fenton, los estudios demuestran que este último produce menos cantidad de lodos, consume menos hierro y la disminución de la DQO es más eficiente. Este artículo presenta los PAOs, basados en peróxido de hidrogeno donde se estudió la eficacia de estos tratamientos aplicados a lixiviados.
La lucha contra la infección, y sus implicaciones nocivas para la salud de los seres humanos, es una de las tareas más antiguas del cirujano. Como es bien sabido, las infecciones quirúrgicas pueden dividirse, en aquellas que el cirujano opera, aquellas que constituyen el objeto de la intervención quirúrgica, y las que el cirujano produce, a raíz de la actividad o procedimientos quirúrgicos. Entre las primeras destacan las infecciones intraabdominales, como la peritonitis terciaria, la apendicitis aguda, la diverticulitis, la colecistitis aguda o las debidas a perforación de una víscera hueca. Las infecciones graves de las partes blandas y pared abdominal constituyen un grupo importante de la patología quirúrgica infecciosa en el que son protagonistas patógenos específicos. En realidad, la gran mayoría de las infecciones en cirugía digestiva están causadas por microorganismos del tracto digestivo o por bacterias exógenas procedentes del personal sanitario o del medio ambiente, asociados a una menor capacidad de defensa del huésped frente a la agresión quirúrgica. La otra parte importante de las infecciones en cirugía están relacionadas con la incisión quirúrgica, infección de la herida quirúrgica o dehiscencias anastomóticas. Son estas últimas las que el cirujano provoca en el paciente.
