Hierbij willen wij Angeline Donk danken voor haar reactie op ons artikel '…en wij proberen te luisteren…' in dit tijdschrift (Smith, Kleijn & Stevens, 2000). Haar reactie illustreert zowel het belang als de ingewikkeldheid van differentie¨le conceptualisering van het 'complex aan gevolgen van de door indirecte blootstelling aan traumatische situaties veroorzaakte stress en de veranderingen op cognitief en emotioneel gebied'. Donk wijst, met het oog op te nemen preventieve en curatieve maatregelen, op de noodzaak te differentie¨ren tussen burn-out en secundaire traumatisering, dat wil zeggen onderscheid te maken tussen meer aan werkomstandigheden gerelateerde factoren enerzijds en traumagerelateerde factoren anderzijds. Wij zijn het hiermee van harte eens. In ons recent in het Tijdschrift voor Psychiatrie gepubliceerde artikel (Smith, Kleijn & Stevens, 2001) staat dit verschil tussen traumagerelateerde emotionele belasting en burn-out centraal. Onze onderzoeksgegevens wijzen op verschillende processen, maar het is niet uitgesloten dat de aan traumatherapie verbonden emotionele belasting op den duur over kan gaan in verschijnselen van burn-out. Op grond van een eenmalige meting, zoals in het onderzoek dat wij hebben gedaan, is hierover geen definitieve uitspraak te doen. Wel was het duidelijk dat gunstige werkomstandigheden een matigende werking kunnen hebben op ervaren emotionele belasting.In ons artikel in dit tijdschrift (Smith, Kleijn & Stevens, 2000) hebben wij een lans gebroken voor een nadere differentiatie van traumagerelateerde therapeutreacties, juist omdat de verschillende reacties vragen om daarbij passende preventieve maatregelen -zoals wij in onze conclusie hebben verwoord.We maken graag van de gelegenheid gebruik om hier nader in te gaan op de kern van het commentaar van Donk op ons onderzoek, dat met name de operationalisering van secundaire traumatisering betreft en het moment waarop reacties als pathologisch moeten worden benoemd. Bij onze poging secundaire traumatisering te operationaliseren gingen wij uit van de meest voorkomende symptomen van de posttraumatische stressstoornis (PTSS). Dit zijn terugkerende gedachten en nachtmerries (als teken van intrusie) en slaapproblemen (als teken van hyperarousal). Vermijdingsgedrag wordt in onderzoek bij patie¨nten met PTSS relatief minder frequent gevonden en is gevarieerder van karakter (Kleijn, Hovens, Rodenburg & Rijnders, 1998). Gemeten over alle respondenten, en niet alleen psychotherapeuten zoals in het onderhavige artikel, bleek onrustige slaap samen te hangen met emotionele belasting en uitputting, maar de prevalentie van nachtmerries niet. Onrustige slaap was naar ons idee dan ook meer verbonden met burn-out dan met secundaire traumatisering, waarbij intrusieve verschijnselen te verwachten zouden zijn. Het ontbreken van een verhoogde frequentie van nachtmerries zou er mogelijk op kunnen wijzen dat geen van onze respondenten de grens van 'normale verwerking' naar 'pathologie' (secundaire traumatisering) had overschreden.
