Amamentar é 'da natureza da mulher'? Então, por que profissionais de saúde, frequentemente, ouvem queixas como 'a mama empedrou', 'o leite secou'? Este livro tenta responder a todas estas perguntas. O autor vai buscar na história e na sociedade as origens para o paradigma atual da amamentação. Ao considerar a amamentação como um híbrido produzido pela natureza e pela cultura, sugere uma abordagem mais completa do fenômeno. Traz, ainda, um histórico dos bancos de leite humano, 'as amas-de-leite do século XX', destacando sua importância.

O paradoxo do desmame constitui-se no objeto deste ensaio, que traz como pressuposto o fato de o aleitamento materno ser formado por um tecido inteiriço que interliga natureza-cultura, em suas múltiplas formas de abordagem. Assim, parafraseando o que propõe LATOUR 10 , parte-se do princípio de que o termo aleitamento materno designa dois conjuntos de práticas totalmente distintas. O primeiro conjunto cria, por tradução, os híbridos entre natureza e cultura que se configuram em torno da amamentação. O segundo cria, por purificação, duas zonas ontológicas inteiramente distintas, que coloca, de um lado, as questões relacionadas à ciência e, de outro, as que se relacionam à sociedade. O primeiro conecta, através de um fio contínuo, a psicofisiologia da lactação; os mitos; as estratégias científicas; a política estatal de promoção da amamentação; as ONGs; o crescimento e desenvolvimento do lactente; a mulher-mãe; os profissionais de saúde; a indústria de laticínios; enfim, todas as questões plurais que permeiam o aleitamento materno. O segundo, tende a estabelecer uma partição entre o que pertence à natureza e à cultura, através do exercício da crítica. Nesse sentido, a construção das possíveis "verdades" na busca do conhecimento, dito eficiente, dependerá da capacidade de se retraçar a rede sócio-técnico-científica, que se estabelece em torno da amamentação, levando-se simultaneamente em consideração as leis transcendentes e as imanentes.Metodologicamente, este estudo, a partir de uma revisão bibliográfica, tem como eixo de discussão a relação entre a proposição de LATOUR 10 sobre naturezacultura e o aleitamento materno. Em torno desta proposição, tendo como referência uma visão epistemológica, procura-se estabelecer diálogo com diferentes obras no sentido de contribuir para o aprofundamento da temática em questão no âmbito da saúde pública. AMAMENTAR, POR QUE?O lidar com a promoção do aleitamento materno, em um cenário de saúde pública como o que se configura hoje no Brasil, assume um contorno de razoável complexidade. A depender da semiótica sob a qual o tema é focado, emergem questões, em princípio passíveis de serem consideradas como singulares, mas que ao se transformarem em objeto de reflexão revelam a pluralidade que permeia o tema. Tal fato pode ser facilmente comprovado, mediante uma incursão pela literatura no intuito de responder a uma indagação básica: amamentar, por que? O conhecimento científico oferece respostas de amplo espectro, que vão desde propriedades biológicas ímpares do leite até questões de cunho sócio-econômico. Objective: To study the reasons for weaning given by women receiving care at a Baby-Friendly Maternity in Teresina, state of Piauí, Brazil. Methods:The methodological principles of qualitative research were applied, supported by the social representation theory. Data were collected from 24 women who were in the process of weaning their babies before the 4 th month of life. Results:The decision-making process that leads women to wean their babies is complex and guilt-ridden. The following reasons for weaning were mentioned: having weak or little milk; puerperal problems affecting the breasts; lack of experience; disparity between the needs of the mother and the needs of the baby; external factors; work; ambiguity between wish/capacity to breastfeed and between burden/desire. Conclusions:A sense of solitude/isolation on the part of the mother and the need for support, not only from health services, but also from other segments of society, were conveyed in all interviews. ResumoObjetivo: Estudar as alegações para o desmame entre mulheres assistidas em uma maternidade Amiga da Criança, em Teresina, Piauí.Metodologia: Adotou-se os preceitos metodológicos da pesquisa qualitativa em saúde, utilizando como referencial teórico a teoria das representações sociais. Ao todo, entrevistou-se 24 mulheres que se encontravam em processo de desmame antes do 4º mês de vida do bebê.Resultados: A análise compreensiva permitiu revelar que a tomada de decisões que leva as mulheres ao desmame se dá de maneira complexa e carregada de culpa. Dentre os motivos alegados, figuraram leite fraco ou pouco, intercorrências de mama puerperal, falta de experiência, inadequação entre as suas necessidades e as do bebê, interferências externas, trabalho, ambigüidade entre o querer/ poder amamentar e entre o fardo/desejo. Conclusões:A solidão/isolamento da mulher-mãe e a necessidade de obter apoio para a consecução dessa prática, não só por parte do serviço de saúde como também dos outros segmentos da sociedade, se fizeram presentes de forma uniforme nas entrevistas. Objective: To describe the eating habits of breastfed and non-breastfed children through the analysis of food intake by children younger than one year of age in Feira de Santana in 2001.Methods: Cross-sectional study. The mothers of children younger than one year answered a questionnaire during the national vaccination day in 44 (71%) vaccination units selected by simple stratification. 2,319 children were evaluated, representing 24.3% of the estimated population. Prevalence ratio and 95% confidence interval were calculated. Statistical significance was determined using the chi-square test.Results: Non-breastfed children aged four months or younger were 8.2 and 6.7 times more likely to receive the family food (95% CI: 3.23-20.66) or vegetable soup (95% CI: 3.84-11.78), respectively. In the same age group, the prevalence of non-breastfed children receiving water, juice, and fruit was significantly higher in relation to breastfed children (70.7% vs 19.7%, 63.3% vs 26.7%, 33.3% vs 4.9% and 14.4% vs 1.4%, respectively). OBJETIVOS: conhecer a prevalência do aleitamento materno e os fatores associados ao desmame das crianças menores de um ano, em Feira de Santana, no ano 2001. MÉTODOS: estudo transversal com aplicação de questionários às 2319 mães presentes nas 44 (71,1%) unidades de vacinação selecionadas, por estratificação simples. Foi calculada a razão de prevalência e considerado como significante p < 0,05 e intervalo de 95% de confiança. RESULTADOS: a prevalência do aleitamento foi 69,2% nos menores de um ano. A ocorrência da amamentação exclusiva foi maior nas crianças que não usavam chupeta (49,4%) e que mamaram no primeiro dia de vida (40,4%). As variáveis estatisticamente significantes relacionadas com as maiores chances de amamentar foram: amamentação no primeiro dia (p <0,001), mães que não trabalhavam fora do lar (p <0,001), menor renda familiar (p <0,001), multiparidade (p = 0,03) e a não utilização de chupetas (p <0,001). CONCLUSÕES: as atividades assistenciais quanto à prevenção do desmame devem estar voltadas sobretudo para as mães que trabalham fora do lar, aquelas de maior renda familiar e primíparas. Quanto às crianças, para as que não foram aleitadas no peito no primeiro dia de vida e que usam chupetas.
Neonatal nutrition is an important subject in health in the short, medium and long term. In preterm newborns, nutrition assumes a predominant role for the child's overall development. Babies with uncoordinated swallowing or respiration may not have the necessary oral abilities to suck the mother's breast and will need to implement different feeding practices; one of them is changing the consistency of the milk offered.ObjectivesDetermine viscosity variations of untreated human and pasteurized milk without and with thickening to adapt the diet to the needs of dysphagic infants hospitalized in the Neonatal Intensive Cara Unit (NICU). Material and MethodsThe authors altered the viscosity of natural infant powdered milk and, after thickening, determined and adopted a thickening standard for human milk. Untreated human and pasteurized milk was thickened in concentrations of 2%, 3%, 5% and 7% and the viscosity were determined every 20 minutes for a period of 60 minutes at a temperature of 37ºC. ResultsThe infant lactose formula thickened at concentrations of 2% and 3% produced viscosities of 8.97cP and 27.73 cP, respectively. The increases were significantly different after 1 hour. Inversely, untreated human milk at 2%, 3%, 5% and 7% produced diminished viscosity over time; the changes were more accentuated in the first 20 minutes. In pasteurized human milk, the 2% concentration had no variation in viscosity, but with the 3%, 5% and 7% concentrations, there was a significant decrease in the first 20 minutes with stability observed in the subsequent times. ConclusionIn powdered milk, the viscosity increases over time; the viscosity in human milk diminishes. The results point out the importance not only of considering the concentration of the thickener but also the time being administered after its addition to effectively treat dysphagic infants.
OBJETIVOS: analisar a gênese e evolução da Rede Nacional de Bancos de Leite Humano no Brasil. São destacados os elementos determinantes de sua reconhecida participação e influência na formulação da política estatal para o setor. MÉTODOS: foram realizadas análises de conteúdo de fontes documentais primárias geradas pelos órgãos oficiais e instituições mantenedoras de Bancos de Leite Humano, de teses, dissertações, livros e artigos científicos, que versam sobre a temática. Identificou-se que as percepções e construções sociais acerca destas unidades de serviço sofreram flutuações ao longo do tempo, e a depender do momento histórico que se considere, atores e grupos sociais lhes atribuíram diferentes significados. RESULTADOS: o conhecimento aparece como elemento que confere conectividade à Rede. Ou seja, a análise realizada, da gênese e evolução dos Bancos, permite identificar que além dos movimentos dos atores sociais envolvidos, há uma dinâmica da produção do conhecimento que deve ser entendida. CONCLUSÕES: é possível identificar a potencial contribuição da Rede para promoção de transformações sociais e para a formulação de políticas públicas voltadas para área da saúde da mulher e da criança.

AbstractObjective: To contribute to the creation of a new theoretical synthesis about the relationship between the biological and social dimensions of breastfeeding, categorizing it as a nature-nurture hybrid. Sources of data:The methodological approach of the present article was based on the qualitative health research frame, and data were analyzed on the basis of dialectic-hermeneutic principles. Primary sources, comprising historical documents, books, scientific articles and medical theses written in the 19th and 20th centuries were analyzed. Summary of the findings:Depending on the moment in history and on the expectations that surround the act of breastfeeding, the aspects related to nature and nurture, i.e., to biology and society, sometimes separate out and sometimes intermingle. A comprehensive approach to breastfeeding makes possible to see that it had to be considered instinctive, natural and biological in order to be assimilated as a cultural habit, and thus, it is not open to any kind of questioning. On other occasions, particularly in the course of the past decade, there has been a steady tendency to supplant traditional biological reductionism, which yielded cultural interpretations that do not reduce the human being to the status of a mammal like any other. Under this point of view, breastfeeding, in addition to being biologically determined, is socially conditioned, and thus constitutes an act embedded in ideologies and determinants resulting from the concrete conditions of life. In real life, however, a process apparently so natural, so loaded with affection and emotion is, in fact, affected by the most aggressive market interests, very often presented as scientific knowledge and decked out as health provisions. Conclusion:Breastfeeding focused as a nature-culture hybrid symbolizes the theoretical and methodological changes in the field, especially in the late 1990s.
