Resumen
A pesar de los avances técnicos y de reanimación postoperatoria, la cirugía pancreática sigue presentando una tasa de morbilidad media del 58% y de mortalidad del 4%. Esta tasa de complicaciones se resume fundamentalmente en cuatro inmediatas (la fístula pancreática, la hemorragia postoperatoria, el retraso del vaciamiento gástrico y la pancreatitis del remanente) y cuatro tardías (la insuficiencia pancreática exocrina, la endocrina, la aparición de colangitis a largo plazo y de nuevo la pancreatitis aguda). Se analizan en este artículo la definición actual de las mismas, su diagnóstico y su manejo clínico recogiendo la mejor evidencia disponible.
Resumen
La hidatidosis puede mostrar signos radiológicos atípicos que pueden inducir a un diagnóstico erróneo inicial. Presentamos el caso de un paciente alcohólico con distensión abdominal progresiva y hallazgo radiológico de un quiste gigante abdominal que ocupaba toda la cavidad abdominal y con características compatibles con un pseudoquiste pancreático. El drenaje ecoguiado permitió disminuir el contenido del quiste y observar en el control radiológico las vesículas hijas características del quiste hidatídico.
Resumen
Introducción: el estado de alarma por la actual pandemia por coronavirus 2019 (COVID-19) ha obligado a adoptar medidas especiales tanto en cirugía electiva como en cirugía urgente no demorable con el objeto de minimizar el riesgo de contagio. Sin embargo, dichas medidas difieren en numerosos aspectos entre centros hospitalarios y sociedades científicas. Además, la falta de experiencia y formación sobre el modo de actuar en estas situaciones aumentan la incertidumbre y el riesgo de contagio accidental.
Objetivo: presentamos nuestra experiencia tras la realización de la primera cirugía urgente abdominal realizada en nuestro centro sobre un paciente positivo para COVID-19.
Caso clínico: varón de 21 años con dolor en fosa iliaca derecha (FID) y síndrome emético de dos días compatible con apendicitis aguda. Comentaba anosmia desde hacía 2 semanas tras volver de un viaje a Londres. Se le practicó la prueba de detección de ARN del SARS-CoV-2 en cepillado nasofaringeo que resultó positiva. Se actívó el protocolo hospitalario para paciente quirúrgicos COVID-19 positivo y se realizó una apendicectomía abierta por apendicitis gangrenosa perforada.
Discusión: se trata de la primera experiencia quirúrgica urgente con una paciente positivo para COVID-19 realizada en el Hospital Virgen de Valme. Tras la activación del protocolo quirúrgico detectamos diversos problemas relacionados con la formación en este tipo de situaciones y con los equipos de protección individual. Creemos imprescindible que cada centro debe ofrecer formación teórica y práctica mediante simulación de las situaciones urgentes y en el manejo de los equipo de protección así como optimizar los equipos de protección individual para mejorar el confort y la visión del cirujano.
