El profesional en fonoaudiología identifica que la lactancia materna juega un papel determinante en la conformación estructural y funcional del sistema estomatognático; siendo la primera función que aporta información paratípica de crecimiento. Diversos autores mencionan que los movimientos de balanceo mandibular propios del amamantamiento, generan excitación sobre la articulación temporomandibular, proporcionando a la estructura estímulos paratípicos de crecimiento, generando acciones de crecimiento transversal del cráneo y la cara. Este estudio con paradigma cuantitativo, no experimental, enfocado en la búsqueda de una relación entre el tipo de lactancia o alimentación con el hábito de deglución atípica. Se utilizará como instrumento de recolección el protocolo de Evaluación Miofuncional, en sus secciones de evaluación cráneo-facial y funcional. Los resultados se centran en el análisis de la razón de momios de la prueba de regresión logística, que evidencia la existencia de un factor protector de la lactancia materna exclusiva, donde los niños que reciben lactancia artificial tienen 4,8 veces más probabilidades de presentar deglución atípica en comparación con los que reciben lactancia materna exclusiva, y 1,6 en el grupo de lactancia materna mixta, en comparación con los de lactancia exclusiva. Los aspectos preventivos de la lactancia materna exclusiva, denotan una gran importancia como abordaje de los procesos disfuncionales y parafuncionales, teniendo como sustento una regulación de las acciones fisiológicas vinculadas en principio a la alimentación inicial, que, para llevarse a cabo, necesita de estructuras anatómicas en estado óptimo, de acciones funcionales (sensitivas y motoras) efectivas y de una guía adecuada por parte del contexto.
Introducción: Las acciones terapéuticas del profesional de fonoaudiología, en la unidad de cuidados intensivos neonatal, están definidas por procedimientos de evaluación, diagnóstico y tratamiento de las funciones estomatognáticas u orofaciales. El equipo interdisciplinar de estas unidades, por lo general, no cuenta con la participación del fonoaudiólogo, siendo suplido por otros profesionales que no cuentan con una adecuada técnica terapéutica y sin las competencias para la ejecución de estos procedimientos.Métodos: Investigación aplicada con muestreo aleatorio. La muestra poblacional de estudio corresponde al personal médico vinculado a una unidad de cuidado intensivo. La naturaleza del estudio es cuantitativa, de corte transversal no experimental. Para la recolección de los datos se utilizó una encuesta tipo Likert. Análisis estadístico basado de chi-cuadrado.Resultados: Se destaca la necesidad de un mejor conocimiento del trabajo del fonoaudiólogo en la unidad.Conclusión: Mediante el instrumento aplicado, se pudo identificar la necesidad e importancia del rol de fonoaudiólogo en el equipo interdisciplinario. Se determinó que los médicos generales y nutricionistas son los que menos conocen la labor de este profesional en la unidad.
Introducción y marco teórico: La secuencia de acciones implica en el acto de deglutir complejos comandos neurofisiológicos para tornarse efectivo, de inicio voluntario y finalizando de manera involuntária. Didácticamente hablando, la deglución se divide en fase oral con característica de control voluntario, y es donde principalmente se da la formación del bolo alimenticio; las fases faríngea y esofágica se definen como automáticas o involuntarias. Metodología: Para el desarrollo de este estudio se acudió a la metodología de revisión sistemática de la literatura. La pregunta de investigación planteada fué ¿El uso de la videofluoroscopia ayuda en la caracterización de la deglución y en el diagnóstico de la disfagia? Resultados: Se hallaron 641 artículos, posteriormente se realizó una preselección, por medio de la lectura de los resúmenes de los trabajos. A partir de allí, con base en los criterios especificados se seleccionaron ocho (8) artículos los cuales contemplaron la investigación y la relación entre los descriptores. Conclusión: La Videofluoroscopia de la Deglución es el examen Gold Standard para la detección, tratamiento y/o investigaciones relacionadas con la Disfagia Orofaríngea y esofágica.
En el Síndrome de Enclaustramiento (Locked-In Syndrome, LIS) las manifestaciones clínicas son preponderantemente de orden motor, situación que requiere de la intervención del Fonoaudiólogo, dado el compromiso comunicativo y de la biomecánica deglutoria. La disfagia en el Locked-In Syndrome está dada por la desconexión sensorial y motriz de las estructuras anatómicas de la deglución, alterando la secuencia neuromotora y sensitiva del proceso deglutorio debido a la desconexión pontina. Dadas las características multicéntricas de la deglución, la intervención fonoaudiológica debe darse oportunamente, donde el eje fundamental de la intervención fonoaudiológica debe estar en la secuencialidad de los procesos neurofisiológicos; adoptando como estrategia fundamental el manejo de consistencias, para el fortalecimiento sensorial y motor de la deglución. Al intervenir las necesidades sensoriomotoras orales en pro de la alimentación, y usando como estrategias terapéuticas la modificación de consistencias, manejo de volúmen, postura, aditamento para suministro de alimento y tiempos de ingesta, se minimizan los riesgos en el proceso de alimentación y se pudo llegar a mantener un estado nutricional y de hidratación óptimo para la recuperación física general, desarrollado paralelamente con mejores procesos de interacción familiar que impactan positivamente la calidad de vida.
Introducción: una de las herramientas que actualmente comenzó a ser usada como complemento terapeutico en la rehabilitación de la disfagia neurogénica es la electroestimulación neuromuscular (EENM), considerada una tecnica no invasiva, aplicada por medio de electrodos en forma trascutanea. La electroestimulación neuromuscular tiene como finalidad promover el movimiento del complejo hiolaríngeo y favorecimiento de los grupos musculares envueltos directamente en la deglución, de ahí la importancia de sustentar conceptual, teórica y procedimentalmente la técnica para en la atención del Fonoaudiólogo. Metodo: la revisión sistemática fue basa en la metodología Prisma, se consultaron las bases de datos Scielo, Lilacs y PubMed, utilizando los descriptores “Deglución”, “Accidente Vascular Cerebral”, “Disfagia”, “Electroestimulación”, “Efectos” (Swallowing, Stroke, dysphagia, electrostimulation, Effects). Resultados: fueron encontrados 79 estudios relacionados con los descriptores utilizados, 13 estudios cuando los descriptores se relacionaban con la técnica de Electroestimulación Funcional. La mayoría de los estudios seleccionados y revisados mostraron efectos beneficiosos como resultado de la terapia tradicional combinada con la EENM, estos efectos fueron: cambios significativos a nivel de ingesta por via oral, mayor significancia en la escala de penetración y aspiración con disminución en el tiempo de transito oral y faríngeo de la deglución. Conclusiones: Los estudios indagados mostraron que la EENM es una herramienta eficaz cundo se utiliza de manera complementaria con la terapia tradicional para la disfagia en casos de ACV.
