Inclusão da pessoa com deficiência no trabalho: estudo transversal Inclusion of disable people in the labor: cross-sectional study
Objective: Given the epidemiological importance of malleolar fractures and differences in treatment choices, this study provides an update on the multiprofessional treatment of these fractures by mapping systematic reviews available in the scientific literature. Methods: The search was performed in the electronic databases Cochrane Database of Systematic Reviews (CDSR), Medline and Lilacs and in the PROSPERO international prospective register of systematic reviews, with no language or publication date restriction. The most recent search was performed on June 1, 2018. The term “ankle fracture” was used in those databases. Pubmed was also searched, using the [Mesh] term. The inclusion criteria were systematic reviews on the treatment of malleolar ankle fractures in adults. Results: Twenty-two systematic reviews were identified in the searched databases. Systematic reviews on the multiprofessional treatment of patients with malleolar fractures addressed the selection of metallic syndesmotic screw rather than absorbable screw without indicating its removal in the absence of symptoms. Furthermore, the Adelaide Fracture in the Diabetic Ankle (AFDA) algorithm was established for the diagnosis and treatment of diabetic patients. There is still no evidence that arthroscopically assisted open reduction and internal fixation (ORIF) is the best therapeutic method, nor is there biomechanical evidence that the locking plate is better than the conventional plate for treating lateral malleolar fracture in elderly people. There is strong evidence that mobility and early weight-bearing directly affect the functional prognosis of patients. Conclusion: This study was extremely important for identifying and selecting the most recent systematic reviews on the topic, thereby guiding practices regarding the best therapeutic regimen for patients with malleolar fractures.
Resumo Objetivo Avaliar a qualidade metodológica das revisões sistemáticas para tratamento cirúrgico e não cirúrgico de indivíduos com síndrome do manguito rotador; comparar, através do instrumento Assessingthemethodologicalqualityofsystematic reviews(AMSTAR, na sigla em inglês), a qualidade dos estudos encontrados nas bases de dados Cochrane Library, PubMed (Publisher Medline), EMBASE e Qinsight. Métodos Trata-se de um estudo transversal descritivo e comparativo, em que dois autores independentes analisaram, por meio do instrumento AMSTAR, a qualidade metodológica das revisões sistemáticas Cochrane e nãoCochrane sobre tratamento de indivíduos com diagnóstico de síndrome do manguito rotador. Resultados 76 revisões sistemáticas foram avaliadas pelo instrumento AMSTAR. O escore médio geral foi de 6,1(±2,1) e a média por base de dados foi 9,1(±0,9) para as revisões Cochrane e 5,7(±1,8) para as não Cochrane. O item de menor pontuação do AMSTAR foi 11, relacionada à exibição dos conflitos de interesse da publicação. Em uma análise comparativa do escore da variável final, houve uma diferença estatística entre os estudos Cochrane e nãoCochrane. Conclusão De acordo com o presenteestudo, revisões sistemáticas utilizando a metodologia Cochrane têm uma melhor qualidade metodológica em comparação com estudos nãoCochrane sobre o tratamento de disfunções do manguito rotador.
INTRODUÇÃOA urticária aguda são lesões maculopapulares, eritematosas e pruriginosas, com duração de até 6 semanas, que surgem de forma espontânea ou induzida. São manifestações da Hipersensibilidade do tipo I, e, por haver um mecanismo imunológico comum a reações imediatas, há uma tendência a se interpretar a causalidade do quadro com processos alérgicos. Sabe-se que são variadas as causas da urticária aguda induzida, desde alérgenos alimentares, medicamentos e picadas de insetos, até manifestação de um quadro infeccioso. OBJETIVOSInformar e evidenciar as principais causas de urticária aguda na infância. MATERIAIS E MÉTODOSTrata-se de uma revisão de literatura na base de dados bibliográficos Medline via Pubmed, usando como estratégia de busca "Urticaria AND acute". O critério de inclusão estabelecido foram artigos que abordassem urticárias em crianças, que ratificam a importância para a saúde do público infantil, com filtros em artigos em inglês, publicados nos últimos 15 anos. Foram excluídos artigos que focam somente em urticária crônica. Dos 400 artigos encontrados, foram excluídos 396 pela leitura do título, seguido pela leitura dos resumos, sendo usado os 4 restantes como materiais de análises. RESULTADOS E DISCUSSÃODe acordo com as análises dos estudos, os quadros de urticária aguda na infância possuem como principal causa, em até 48,4% dos casos, os processos infecciosos, com predominância de infecções virais. Segue-se como causas, com até 11,2%, alérgenos alimentares, com destaque para ovo, leite e nozes, e 9% picada de insetos. Menos comumente, intoxicações por drogas podem manifestar-se com urticária aguda em até 8% dos casos. CONCLUSÕESConclui-se que, predominantemente, as causas de urticária aguda na infância são processos infecciosos, seguidos de alérgenos alimentares e medicamentos. Destacando-se também a necessidade de investigação dos quadros, quando de forma induzida, a fim de evitar exclusão desnecessária de alimentos e medicamentos por diagnóstico falso de alergia a tais substâncias.
